獨家對話重慶海扶醫(yī)院院長汪煉“捍衛(wèi)子宮，不分國界！”

記者：我看了下你們的資料，你們醫(yī)院采用的核心技術(shù)是婦產(chǎn)科教授王智彪及團隊發(fā)明的海扶超聲聚焦刀治療骨癌、肝癌、乳腺癌及子宮肌瘤等。海扶超聲聚焦刀的學(xué)術(shù)名稱叫“高強度聚焦腫瘤治療系統(tǒng)”。這個“腫瘤”與我們常說的“癌”是什么關(guān)系？

汪煉：腫瘤分惡性和良性，而“癌”通常是指惡性腫瘤。

記者：海扶超聲聚焦刀我早就聽說過，10幾年前新聞報道過，當時在世界上引起很大震動。

汪煉：是的。王智彪是一位婦產(chǎn)科教授、博士生導(dǎo)師、海扶超聲聚焦刀的第一發(fā)明人。他立志于解除婦女的痛苦發(fā)明了聚焦刀，而聚焦刀發(fā)明后不僅能解除婦女的痛苦，也能解除腫瘤患者的痛苦。

記者：聽說人在你們的機器上一“照”病就好了？

汪煉：“照”只是一個形象的說法。聚焦刀類似放大鏡聚焦的原理。它利用超聲波的組織穿透性和可聚焦性，將低能量的超聲波聚焦，在焦點處瞬間產(chǎn)生高溫，從而殺滅體內(nèi)的腫瘤組織，而在焦點外沒有組織的損傷，實現(xiàn)了醫(yī)學(xué)一直向往的不開刀、不流血的手術(shù)。其專業(yè)名詞叫“超聲消融”。

記者：“照”一下就好了，聽起來像神話，真有點不可思議？

汪煉：但這確實是真實的。上世紀40年代，美國科學(xué)家Fry兄弟曾夢想“能否將超聲波從體外聚焦到體內(nèi)只殺死腫瘤，而不傷害周圍組織和機體的正常功能”，上世紀90年代末，中國重慶的王智彪教授及團隊把這個世界性的重大科學(xué)問題變成了現(xiàn)實。

迄今，該技術(shù)已從重慶走向了世界，先后落戶于英國、德國、意大利、西班牙、俄羅斯、韓國、日本等19個國家25個地區(qū)的醫(yī)科大學(xué)和醫(yī)院，治療子宮肌瘤、子宮腺肌病逾2萬例、惡性實體腫瘤逾5萬例，中國原始自主創(chuàng)新的高科技術(shù)在臨床上得到了成功運用。

子宮肌瘤婦女發(fā)病率30—50%，生育年齡高達50%。目前多數(shù)是切除患者子宮?，F(xiàn)在，美國每年有62.2萬婦女子宮被切除，中國約有200萬婦女子宮被切除。據(jù)了解，切除子宮對婦女危害巨大。那么，子宮肌瘤患者應(yīng)如何應(yīng)對呢？日前，本刊記者獨家對話全球首家子宮肌瘤專病醫(yī)院——重慶醫(yī)科大學(xué)附一院海扶醫(yī)院院長汪煉。

記者：這么牛呀？說來還是現(xiàn)實不是神話？

汪煉：牛或不牛，這都是現(xiàn)實。160多年前，發(fā)明麻醉技術(shù)開啟了人體手術(shù)治療史；半個世紀前腔鏡技術(shù)發(fā)明，開啟了微創(chuàng)醫(yī)療。15年前，王智彪及團隊發(fā)明了超聲聚焦刀技術(shù)，由此開啟了微無創(chuàng)醫(yī)療。這成為世界醫(yī)療史上新的里程碑。

記者：你們這個“聚焦刀”針對所有腫瘤，不分惡性和良性嗎？

汪煉：是的。良性腫瘤包括：子宮肌瘤、子宮腺肌病、乳腺纖維瘤；惡性腫瘤包括：肝癌、乳腺癌、骨腫瘤、軟組織肉瘤、腎癌、胰腺癌等?！熬劢沟丁倍脊苡?，只要是實體腫瘤，不分良性、惡性。

記者：那現(xiàn)在得了癌癥還有必要這么害怕嗎？

汪煉：“聚焦刀”是癌癥的克星。1998年，小熊（化名）被確診為骨癌，跑遍全國許多大醫(yī)院，得到的回答都是“截肢”。當時，她還是一個僅有9歲的小姑娘。1999年8月，海扶醫(yī)療團隊用“海扶超聲聚焦刀”為她進行了治療，而且是“無創(chuàng)”，不僅保住了患肢，而且健康地活了下來，后來她還上了大學(xué)，并成家立業(yè)。這樣的例子有很多。我們醫(yī)院可以治療各種骨癌、肝癌等癌癥。人們大可不必得癌色變。

記者：現(xiàn)在，很多醫(yī)院治療子宮肌瘤都是手術(shù)挖出或子宮切除。你們的這個做法與傳統(tǒng)的做法有什么不同？

汪煉：子宮肌瘤婦女發(fā)病率達30—50%，生育年齡高達50%。目前多數(shù)是手術(shù)挖出或子宮切除?，F(xiàn)在，美國每年有62.2萬婦女子宮切除，中國約有200萬婦女子宮切除。挽救子宮比什么都重要。作為醫(yī)生，關(guān)愛生命，就應(yīng)該從尊重器官開始。

現(xiàn)在，在傳統(tǒng)醫(yī)院做子宮肌瘤手術(shù)，一般大概要住院一周，休息1—2個月。我們約有70%的患者有機會選擇不開刀的無創(chuàng)治療。因為沒有傷口，恢復(fù)快，住院2天左右，回家休息1-2天即可，也可以不住院，以“日間手術(shù)”在門診完成。我們很多的外地病人當天治療后，第2天就在重慶參加一日游了。

記者：這么簡單？我平時感覺到的是，女人們只要一檢查到自己得了子宮肌瘤，總是害怕得不得了，甚至以為是得了絕癥。沒想到操作這么簡單。

汪煉：子宮肌瘤是女性最常見的良性腫瘤，育齡期女性發(fā)生率可高達50%左右，發(fā)展成惡性腫瘤的概率非常小。

記者：有沒有典型案例？

汪煉：多了。我剛才說，迄今為止，我們已在全球治療各種良性腫瘤逾2萬例，有國內(nèi)的，也有國外患者慕名而來。2009年，澳大利亞華人“在水一方”（網(wǎng)名）體檢發(fā)現(xiàn)多發(fā)性子宮肌瘤，其中最大的肌瘤直徑7.6公分，此后的時間里肌瘤一直不斷長大、增多。到2011年初，肌瘤已經(jīng)長大至接近11.5公分。在澳大利亞，多家醫(yī)院的醫(yī)生建議其直接切除子宮，一勞永逸，然而“在水一方”捍衛(wèi)子宮的決心從未動搖。直到2011年4月，“在水一方”通過網(wǎng)絡(luò)了解到海扶超聲消融無創(chuàng)治療子宮肌瘤的方式。2011年9月，“在水一方”從澳大利亞飛往重慶，在我們醫(yī)院接受了超聲消融治療，成功消融15個肌瘤。手術(shù)后三個月，“在水一方”在澳洲復(fù)查B超，結(jié)果顯示最大的肌瘤已從11.50cm縮小到3.9cm（肌瘤體積縮小了70%），其它等癥狀也得到了明顯的改善。B超結(jié)果讓澳洲醫(yī)生也為之震撼。如果要想了解更多的案例，可以上我們的網(wǎng)站（www.haifuhospital.com/）查詢。

記者：現(xiàn)在，因子宮肌瘤失去子宮的女性越來越多，請問汪院長：子宮完成生育后還有保留的必要嗎？

汪煉：在我們的傳統(tǒng)認識里，子宮的功能幾乎只和生孩子有關(guān)，而生完孩子以后，子宮就成了一個沒用的廢置器官，可以把它隨便切除。其實，子宮對人類有重要的作用：子宮是女性重要的生殖器官，是我們每個人第一個溫暖的家；子宮是女性生殖健康的重要角色，子宮是女性的尊嚴。有的女性切除子宮后，會覺得自己是“空腹女人”“半個女人”。據(jù)美國赫氏基金會報道，切除子宮后的女性：77.8%易怒，76.4%個性改變，66.0%喪失性欲，53.8%喪失性高潮，51.9%喪失性能力，53.3%有自殺念頭，57.5%失眠，40.6%喪失母性，35.4%頭痛，22.2%心悸，33.5%尿頻……切除給女性帶來巨大的身心傷害，甚至影響到家庭的幸福和穩(wěn)定。

記者：對你們的技術(shù)我也不太懂。但恕我直言，根據(jù)我了解的情況，現(xiàn)在你們的這個技術(shù)似乎還沒有得到醫(yī)療界的普遍認同和運用。這是為什么呢？

汪煉：你說的確實也是事實。這項技術(shù)的先進性不容置疑，推廣運用也正是我們需要努力的方向。打響子宮保衛(wèi)戰(zhàn)，勢在必行！這需要全社會的輿論力量喚醒更多女性對保衛(wèi)子宮的重視，重拾女性的健康與自信。

為了喚起全社會對這方面的共識，今年在北京召開的全國“兩會”上，全國政協(xié)委員王智彪教授已向全國政協(xié)提交提案，建議設(shè)立“世界子宮日”，呼吁全社會關(guān)注女性子宮疾病，提高女性健康意識和水平?！瓣P(guān)愛生命，保衛(wèi)子宮?！蔽覀冃枰蛉澜绱舐晠群埃?/p>

汪煉院長簡介

汪煉，女，婦產(chǎn)科主任醫(yī)師，重慶醫(yī)科大學(xué)附屬第一醫(yī)院海扶醫(yī)院院長。從事婦產(chǎn)科臨床、科研、教學(xué)工作40余年。歷任重慶市子宮良性疾病微無創(chuàng)治療研究中心副主任、重慶市外陰疾病微無創(chuàng)治療究中心副主任、重慶市生殖健康學(xué)會生殖醫(yī)學(xué)專業(yè)委員會委員、重慶市性學(xué)會性醫(yī)學(xué)專委會委員等。在《中華婦產(chǎn)科雜志》等國家核心期刊上發(fā)表論文20余篇。承擔了《“十二五”國家科技支撐計劃“超聲消融子宮肌瘤的示范應(yīng)用與臨床再評價”項目》等多項科研項目。

王智彪教授簡介

王智彪，男，婦產(chǎn)科學(xué)教授、博士生導(dǎo)師，海扶超聲聚焦刀第一發(fā)明人。第10、11、12屆全國政協(xié)委員?，F(xiàn)任超聲醫(yī)療國家工程研究中心主任，中國超聲治療學(xué)會主任委員、國際超聲治療學(xué)會常務(wù)理事、中國生物醫(yī)學(xué)工程學(xué)會常務(wù)理事。先后承擔了50余項國家自然科學(xué)基金委等重點項目。從事婦產(chǎn)科、超聲治療學(xué)基礎(chǔ)和臨床應(yīng)用研究30年。

The morbidity of uterine fibroids among women is 30-50%， while for those at reproductive age， it reaches up to 50%. Currently， most of the doctors will resect the uterus of the patients. In USA， About 622，000 uteruses are resected every year， and the number in China is 2，000，000. However， it was known that hysterectomy is quite harmful to women. Thus， what shall women do with uterine f broids？ Regarding this question， we had an exclusive interview with Dr. Wang Lian，Director of the world's f rst hospital specialized in the treatment of uterine f broids-the Aff liated Haifu Hospital of Chongqing Medical University.

Reporter： From the material of the hospital I reviewed， I know the core technology of your hospital is Haifu Focused Ultrasound Knife for the treatment of bone tumor， liver cancer， breast cancer，uterine f broids and etc.， which was invented by Professor W ang Zhibiao， an Obstetrician and Gynaecologist and his team. The academic name of Haifu Knife is “Model JC Focused Ultrasound Tumor Therapeutic System”， What is the relationship between "tumor" and "cancer"？

Wang Lian： Tumor can be malignant and benign， but "cancer" usually refers to the malignant tumor.

Reporter： It was heard that the patients could be treated just by one “irradiation” from your device？

Wang Lian： “Irradiation” is a kind of metaphor. The working principle of HIFU （high-Intensity Focused Ultrasound， also known as FUS or HIFUS） is similar to magni f er. As ultrasound can pass through tissues and be focused， the system focus the low intensity ultrasound and cause instant high temperature at the focus to ablate the tumor in vivo while cause no damage to tissues outside the focus. It is a treatment with no bleeding or open surgery， which is called “ultrasound ablation” in clinic.

Reporter： I have heard about Haifu Knife from news and reports more than 10 years ago， it caused a great stir around the world at that time.

Wang Lian： Yes. Professor Wang Zhibiao is a professor of obstetrics and gynecology， Doctoral advisor and the leading inventor of Haifu Knife. He devotes himself to alleviate the pain of women， and the Haifu Knife he invented can not only alleviate the pain of woman， but also patients with tumors.

Reporter： The patient will be treated just by one irradiation，this sounds like magic and unbelievable.

Wang Lian： But it is true. In 1940s， two American scientists，the Fry bothers dreamed about “focusing ultrasound in vitro to kill tumors in vivo without damage to the normal function of surrounding tissues and organs”. This dream， also a world scientif c conundrum was realized by Professor Wang Zhibiao and his team in the late 1990s here in Chongqing， China.Till now， the technology has gone global from Chongqing，with the system been applied in medical colleges and hospitals in about 20 countries and regions such as UK， Germany， Italy， Spain，Russia， South Korea， Japan. Over 20，000 cases of uterine f broids adenomyosis and more than 50，000 cases of malignant solid tumors have been treated by using HIFU， which demonstrated the successful clinical application of original innovative China high

tech.

Reporter： Really？ Unbelievable！ It sounds like myth.

Wang Lian： It is unbelievable， but it is real. More than 160 years ago， the invention of anesthetic opened the history of surgery on human； half a century ago， laparoscopy started the minimally invasive medicine. Fifteen years ago， Professor W ang Zhibiao and his team invented the focus ultrasound system，opened the door of minimally-invasive and noninvasive medicine. This is a new milestone in the world medical history.

Reporter： The focused ultrasound ablation can treat all tumors？ malignant and benign？

Wang Lian： Yes. Benign tumors include uterine fibroids，uterine adenomyosis， breast fibroma； malignant tumors include liver cancer， breast cancer， bone tumor， soft tissue sarcoma，kidney cancer， pancreas cancer and etc. All the solid tumors are suitable， no matter it is benign or malignant.

Reporter： So shall we not be so scared if we had cancer？

Wang Lian： HIFU can fight against cancer. In 1998， Xiao Xiong（a pseudonym） was diagnosed with bone tumor， she went to many large hospitals around the whole country， the answer she got from the doctors was "amputation”. At that time she was only 9 years old， just a little girl. In August 1999， haifu medical team treated her with HIFU， noninvasively. Her limb was saved and survived as a healthy girl. She went to college later， had her own career and family. We have seen many cases like this. Our hospital can treat cancers such as bone tumor， liver cancer and other cancers. Actually， we do not need to be so scared of cancer.

Reporter： Nowadays， the solutions for uterine fibroids in many hospitals are myomectomy or hysterectomy. What are the differences between HIFU and these traditional ways？

Wang Lian： The morbidity of uterine f broids among women is 30-50%， while for those at reproductive age， it reaches up to 50%.. Currently， myomectomy or hysterectomy are commonly used methods. In USA， About 622，000 uteruses are resected every year now， and the number in China is 2，000，000. Saving uterus is quite essential. As doctors， cherish life shall begin with the respect for the integrity of organs.

Patients with uterine fibroids who went through operation in traditional hospitals need to stay at hospital for about 1 week，and rest for 1～2 months for recovery. In our hospital， about 70% of our patients have the chance for non-invasive HIFU treatment. As there is no cut， the recovery time is short， about 1-2 day hospitalization and then rest for 2 days at home. The patient can also choose outpatient “day-treatment” which requires no hospitalization. We have some patients from other provinces got treatment here and the next day they went for one-day-tour in Chongqing.

Reporter： So easy？ What we knew was that women felt so cared if they are diagnosed with uterine fibroids， some even regard it as fatal disease. We never thought it could be so easy in your hospital.

Wang Lian： Uterine f broid is the most common benign tumor in women， the morbidity of women at childbearing age can reach to 50%， the malignancy probability is very low.

Reporter： Any typical case？

Wang Lian： A lot. As I have mentioned， we have treated more than 20，000 cases of dif ferent benign tumors around the world，patients are from China and abroad， most patients from abroad are attracted by the fame of HIFU technology. In 2009， "Memory in Water" （nickname）， an Australian Chinese was diagnosed with multiple uterine fibroids and the largest one was 7.6 cm in diameter. The fibroid kept growing and by early 201 1， it have grown to 11.5 cm. In Australia， lots of doctors suggested her to receive hysterectomy， once for all； however， she determined to save the uterus. In April， 2011， "Memory in W ater" learned about noninvasive ultrasound ablation of uterine f broids from the website named“savewomb”. In September 201 1， she flew from Australia to Chongqing and underwent ultrasound ablation therapy in our hospital. Doctors ablated her 15 f broids successful. Three months after HIFU， "Memory in W ater" had B-mode ultrasound scanning for follow-up in Australia， the results showed that the biggest f broid have reduced from 1 1.50 cm to 3.9 cm （shrinkage rate is 70%）， and other symptoms also got obvious relieved. Reporter： Now， more and more women lost their uteruses for uterine fibroids， is it necessary for women to keep the uterus after having babies？

Wang Lian： Traditionally， the uterus function seems to be child-bearing， after that， uterus could be useless organ which can be removed randomly. Actually， the uterus is an important reproductive organ for women， the first house of every human being. It plays an important role in health system which stands for woman dignity. Some women without uterus feel so hollow inside. According to American HERS foundation， among women lost uteruses， 77.8% suffer from irritability， 76.4% suffer from personality change，66.0% lose their sexual desire， 53.8% lose orgasm， 51.9% lose sexual ability， 53.3% have suicidal thoughts， 57.5% suffer from insomnia， 40.6% lose their maternal， 35.4% suffer from headache， 22.2% suf fer from heart palpitations，33.5% suffer from frequent micturition…Removal of uterus does enormous physical and psychological harm to the female， and even affect the family's happiness and stability.

Reporter： I am not so familiar with your technology. To be frank， HIFU seems to have not been generally recognized and applied in the medical f eld. Why？

Wang Lian： What you mentioned is a matter of fact. There is no doubt about the technology's advancement. Promotion is what we need to work on. Protecting the uterus is imperative. This requires the power of public opinion of the whole society to awaken more women to realize the importance of saving the uterus， regain the women's health and conf dence.

To arouse the whole society， in People's Congress & The Chinese People's Political Consultative Conference held in Beijing this year， the CPPCC National Committee，Professor Wang Zhibiao has submitted a proposal to the CPPCC proposed "world uterus day"， for the purpose of making the whole society to pay attention to female uterus disease and improve women's health consciousness and level. "Caring life， saving uterus"， we need shout loudly to the world！

Director Wang Lian's brief introduction

Wang Lian， female， obstetrics and gynecology， director of the physician， the f rst aff liated hospital of the af filiated Haifu Hospital of Chongqing Medical University；engaged in the obstetrics and gynecology clinical work， scientific research and education for more than 40 years. Wang Zhibiao， male，professor， he is doctoral supervisor of obstetrics and gynecology， the first inventor of HIFU therapeutic system，the member of 10， 1 1， 12 session of the CPPCC National Committee. Currently， as director of the National Engineering Research Center Of Ultrasound Medicine， director of the institute committee，executive director of the International Society For Therapeutic Ultrasound， executive director of the Biomedical Engineering Society Of China； and he has successively undertaken more than 50 projects of National Natural Science Foundation and other key projects； engaged in gynecology and obstetrics，ultrasound therapeutic basics and clinical application research for 30 years.
