HBV相關(guān)慢加急性肝衰竭患者Th17、Treg變化及其與臨床相關(guān)性

【摘要】目的研究Th17、Treg及Th17/Treg在乙型肝炎相關(guān)慢加急性肝衰竭(ACHBLF)患者外周血的變化,及其與病情進(jìn)展及預(yù)后的相關(guān)性。方法選取33例ACHBLF患者,30例慢性乙型肝炎患者及11名健康對照。用流式細(xì)胞技術(shù)檢測血清中Th17、Treg頻數(shù),分析ACHBLF患者Th17、Treg頻數(shù)的變化及其與TBil、ALT、PT等指標(biāo)的相關(guān)性。結(jié)果ACBLF患者的Th17(1.96±0.99)%比CHB患者(0.59±0.40)%及健康對照者(0.26±0.20)%高(P<0.05);ACHBLF的Th17/Treg(0.46±0.32)比CHB患者(0.12±0.11)及健康對照者(0.07±0.06)的高(P<0.05);ACHBLF患者中,死亡者的Th17比存活者高;Th17細(xì)胞頻數(shù)及Th17/Treg與PT、ALT及AST呈正相關(guān)(P<0.05),與總膽紅素?zé)o顯著相關(guān)性,Th17/Treg與AFP呈正相關(guān)(P<0.05)。結(jié)論ACHBLF患者存在不同程度的免疫功能紊亂,Th17及Th17/Treg與病情進(jìn)展相關(guān),且對患者預(yù)后有重要價(jià)值,Th17越高預(yù)后越差。

每年全世界約有100萬人死于HBV感染引起的肝硬化、肝衰竭和肝細(xì)胞癌[1,2]。慢加急性(亞急性)肝衰竭(ACLF)是在已知或未知的慢性肝病的基礎(chǔ)上,某些誘因下出現(xiàn)的肝功能的急劇惡化,在中國,超過80%的ACLF是由HBV感染引起,即ACHBLF[3]。
ACHBLF進(jìn)展的具體分子機(jī)制尚不清楚,目前的研究提示宿主的免疫在肝臟損傷的進(jìn)展中發(fā)揮重要作用[4]。T輔助細(xì)胞17(Th17)是獨(dú)特的CD4+T淋巴細(xì)胞亞群,在HBV感染的病理過程中發(fā)揮重要作用。調(diào)節(jié)性T細(xì)胞(Treg)是CD4+T淋巴細(xì)胞的另一亞群,血液或肝臟中Treg細(xì)胞的失衡是HBV持續(xù)感染及疾病進(jìn)展的一個(gè)原因[5.6]。本研究觀察了ACHBLF患者的Th17、Treg頻數(shù)變化,并分析其與TBil、ALT、PT、AFP等指標(biāo)的相關(guān)性,為評估病情及預(yù)后提供線索。
資料和方法
一、研究對象
選擇2013年1月至2015年1月在蘇州大學(xué)附屬一院感染科住院的ACHBLF患者33例,其中男性30例,女性3例,年齡30~65歲。診斷符合肝衰竭診療指南(2012版),并排除合并甲、丙、戊型肝炎,酒精性肝病,自身免疫性肝病,肝癌等。所有患者近6個(gè)月未抗病毒治療。慢性乙型肝炎(CHB)患者30例,男21例,女9例,年齡26~60歲。健康對照11例,男15例,女5例,年齡25~45歲。所有患者簽署入組知情同意書。
二、儀器、試劑及檢測方法
流式檢測儀器:Beckman.Coulter XLAT09010流式細(xì)胞儀。
試劑:ECD標(biāo)記的CD3單抗,PC5標(biāo)記的CD25及CD8單抗,F(xiàn)ITC標(biāo)記的CD4單抗,PE標(biāo)記的CD279及CD127單抗,紅細(xì)胞溶解劑及白細(xì)胞穩(wěn)定劑,以及其他的清洗液、鞘液、刺激液等。
Th17及Treg檢測:采集患者入院時(shí)的空腹血2 mL,置于肝素抗凝管中,加入刺激液,放入37 ℃體積分?jǐn)?shù)為0.05的CO2培養(yǎng)箱中孵育5 h后,加入抗體標(biāo)記,并混勻離心后,加入破膜劑,再加入二抗,洗滌離心后上機(jī)檢測。
肝功能檢測:應(yīng)用全自動(dòng)生化儀及血凝儀檢測TBil、ALT、Alb、PT等指標(biāo)。
三、統(tǒng)計(jì)學(xué)處理
采用SPSS 17.0軟件系統(tǒng),定量資料用(均數(shù)±標(biāo)準(zhǔn)差)表示,多組間比較采用方差分析,兩組比較用t檢驗(yàn),正態(tài)分布資料相關(guān)性分析采用Pearson分析,偏態(tài)分布資料用Spearman分析,P<0.05為差異有統(tǒng)計(jì)學(xué)意義。
結(jié)果
一、ACHBLF患者與CHB、健康對照者的Th17、Treg及Th17/Treg的比較
ACHBLF患者的Th17細(xì)胞頻數(shù)為(1.96±0.99)%,CHB及健康對照者的Th17細(xì)胞頻數(shù)分別為(0.59±0.40)%、(0.26±0.20)%,ACHBLF組的Th17細(xì)胞頻數(shù)比CHB組及健康對照者的高,且CHB組的Th17比健康對照者的亦高,差異均有統(tǒng)計(jì)學(xué)意義。ACHBLF及CHB組的Treg細(xì)胞頻數(shù)比正常人得高,但差別無顯著性。比較三組患者的Th17/Treg,ACHBLF組(0.46±0.32)比CHB組0.12±0.11)及健康對照者(0.07±0.06)的高,但CHB與HC之間無明顯差別。
二、ACHBLF死亡與存活患者Th17、Treg及Th17/Treg的比較
33例ACHBLF患者中,存活13例,死亡20例,比較兩組間Th17、Treg及Th17/Treg,死亡患者的Th17(2.26±0.95)%比存活者(1.50±0.90)%高,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。Th17/Treg在死亡者中(0.54±0.33)比存活者(0.31±0.23)高,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。而兩組的Treg無明顯區(qū)別。
三、Th17、Treg及Th17/Treg與肝功能的相關(guān)性
Th17、Treg及Th17/Treg與ALT、AST、TBil、AFP及PT均有相關(guān)性,但是Th17及Th17/Treg與ALT、AST有顯著相關(guān)性(P<0.05),且Th17/Treg與AFP呈正相關(guān)(表1)。

討論
ACHBLF的進(jìn)展涉及到天然免疫及適應(yīng)性免疫系統(tǒng)的紊亂,多種細(xì)胞包括DC、T淋巴細(xì)胞等參與了ACHBLF的疾病過程。Th17及Treg在免疫應(yīng)答中發(fā)揮顯著作用[7,8]。研究證實(shí)Th17細(xì)胞參與了許多肝臟疾病的發(fā)病過程,包括酒精性肝病,原發(fā)性膽汁性肝硬化,慢性乙型肝炎等,參與了宿主抵御細(xì)菌及真菌等病原體的免疫保護(hù)過程[9]。Treg細(xì)胞與Th17細(xì)胞來源于同樣的naive細(xì)胞,通過釋放抗炎因子而發(fā)揮抗炎作用,并且被認(rèn)為在維持宿主自身免疫耐受、免疫平衡中發(fā)揮重要作用[10]。Th17與Treg細(xì)胞之間緊密聯(lián)系,兩者之間的平衡可以影響自身免疫性,在多種自身免疫性疾病中發(fā)現(xiàn)Th17/Treg比例的失衡[11.12]。
本研究發(fā)現(xiàn),ACHBLF及CHB組的Th17及Treg細(xì)胞頻數(shù)比健康對照者高,與Niu等[13]研究相符,證實(shí)Th17、Treg參與了HBV感染的病理過程,而升高的Treg細(xì)胞可能為阻止由Th17引起的過度免疫應(yīng)答進(jìn)行負(fù)反饋性調(diào)節(jié),阻止HBV特異性的T細(xì)胞應(yīng)答,減輕肝臟炎性反應(yīng),控制肝臟的疾病進(jìn)展[14]。比較三組Th17/Treg發(fā)現(xiàn),ACHBLF組比CHB組及HC高,說明ACHBLF患者體內(nèi)存在著Th17/Treg的失衡,而該失衡是偏向于Th17,提示免疫功能亢進(jìn)引起的免疫病理損傷參與了ACHBLF的發(fā)病過程。進(jìn)一步比較ACHBLF死亡者與存活者的Th17、Treg及Th17/Treg,發(fā)現(xiàn)死亡者的Th17比存活者高。
一些研究證明,血液中增加的Th17、Treg細(xì)胞頻數(shù)與疾病進(jìn)展相關(guān),且與肝臟炎癥的嚴(yán)重性相平行,比如CHB進(jìn)展為ACLF[15]。ALT與肝臟炎性反應(yīng)密切相關(guān),TBil及AST與肝臟損傷程度有關(guān),而AFP提示肝臟壞死后再生的程度。本研究發(fā)現(xiàn),Th17、Treg與它們都有相關(guān)性,而Th17與ALT、AST有顯著正相關(guān)性,Th17/Treg不僅與AST、ALT相關(guān),且與AFP呈正相關(guān)。說明在ACHBLF發(fā)病過程中,促炎因子Th17高表達(dá)及Th17/Treg失衡參與肝細(xì)胞的炎性壞死的發(fā)生,提示Th17、Th17/Treg能夠反映肝臟的炎性反應(yīng)的程度,這與該組患者臨床上表現(xiàn)出的肝臟的合成、解毒、生化轉(zhuǎn)化等功能嚴(yán)重障礙相吻合。此結(jié)果與Wang[16]的研究相符,但未發(fā)現(xiàn)免疫指標(biāo)與膽紅素的顯著相關(guān)性,可能與病例數(shù)較少,入組患者有的已經(jīng)過相關(guān)治療有關(guān)。
總之,Th17可以反映ACHBLF肝臟損傷的程度,Treg可以抑制免疫系統(tǒng)的過度反應(yīng),在ACHBLF患者體內(nèi)兩者均升高,且存在偏向于Th17一方的失衡。Th17與肝臟損傷的嚴(yán)重程度密切相關(guān),并且高的Th17提示預(yù)后較差。這些為ACHBLF的治療提供新的思路。然而,ACHBLF相關(guān)的免疫應(yīng)答是復(fù)雜的網(wǎng)絡(luò),同時(shí)存在抗炎反應(yīng)綜合征,因此進(jìn)一步動(dòng)態(tài)研究免疫的變化及與炎性因子的關(guān)系有助于指導(dǎo)治療。

【Abstract】ObjectiveTo investigate the frequency of peripheral Th17, Treg and Th17/Treg in patients with hepatitis B-related acute-on-chronic liver failure (HBV-ACLF), and its correlation with disease progression and prognosis. MethodsSeventy-four cases were enrolled, including 33 HBV-ACLF cases (HBV-ACLF group), 30 chronic hepatitis B cases (CHB group) and 11 healthy cases (control group). The frequency of Th17 and Treg cells and surface antigen quantity in peripheral blood were detected in three groups with flow cytometry technique, respectively. Correlation analysis between changes of Th17 and Treg frequency in the HBV-related ACLF cases and total bilirubin (TBiL), alanine aminotransferase (ALT) and prothrombin time (PT) was performed. ResultsFrequency of Th17, Treg cells and proportion of Th17/Treg were higher in HBV-ACLF patients than those in CHB and control group (P<0.05), respectively. In HBV-ACLF group, frequency of Th17 in dead patients were higher than that in survivors; frequency of Th17 and Th17/Treg were positively correlated with PT, ALT and AST (P<0.05) with no significant correlation with TBiL. Additionally, Th17/Treg was also positively correlated with alpha fetoprotein (AFP) (P<0.05). ConclusionThere were different degrees of immune dysfunction in HBV-ACLF patients. Frequency of Th17 and Th17/Treg were associated with disease progression. Higher frequency of Th17 predicted worse prognosis, which revealed it might be important indicators for those patients.
