[摘要] 目的 分析穩(wěn)心復(fù)脈湯對冠心病患者QT間期離散度的影響。 方法 方便選取該院2013年1月—2015年10月期間收治的95例冠心病患者分為兩組，對照組45例單純常規(guī)治療，觀察組50例在單純常規(guī)基礎(chǔ)上穩(wěn)心復(fù)脈湯治療。觀察兩組患者胸悶、胸痛、氣短、乏力等冠心病癥狀變化，監(jiān)測QT離散度變化。對比兩組患者治療效果。 結(jié)果 觀察組總有效率98%高于對照組總有效率80%，觀察組治療后總癥候評分（0.82±0.31）分及QT間期離散度（67.9±13.9）ms較對照組治療后總癥候評分（3.85±0.82）分及QT間期離散度（51.0±12.9）ms降低（P<0.05）。結(jié)論 穩(wěn)心復(fù)脈湯能降低冠心病患者QT間期離散度，改善患者冠心病心肌缺血癥狀，從而達(dá)到預(yù)防惡性心律失常發(fā)生的作用。

Effect of Wenxin Fumain Tang on the Dispersion of Patients with Coronary Heart Disease during the QT Interval

[Abstract] Objective To analyze the effect of Wenxin Fumain Tang on the dispersion of patients with coronary heart disease during the QT interval. Methods Convenient selection 95 cases of patients with coronary heart disease admitted and treated in our hospital from January 2013 to October 2015 were selected， the control group（n=45） adopted the routine treatment， while the observation group（n=50） adopted the Wenxin Fumain Tang on the basis of the routine treatment， and the symptoms of chest tightness， chest pain， shortness of breath and fatigue were compared between the two groups， and the QT dispersion change was monitored and the treatment effect was observed. Results The total effective rate in the observation group was higher than that in the control group（98% vs 80%）， and the total symptom score and QT interval dispersion in the observation group decreased compared with those in the control group after treatment， [（0.82±0.31）points， （67.9±13.9）ms vs （3.85±0.82）points， （51.0±12.9）ms]（P<0.05）. Conclusion The Wenxin Fumain Tang can decrease the dispersion of patients with coronary heart disease during the QT interval and improve the myocardial ischemic symptoms of patients with coronary heart disease thus preventing the occurrence of malignant arrhythmia.

冠心病多見于中老年人群，患者伴復(fù)雜性心律失常、胸悶、心悸等癥狀，病情呈進(jìn)行性發(fā)展，甚至?xí)T發(fā)患者猝死。QT間期離散度（QTd）表現(xiàn)為體表12導(dǎo)聯(lián)心電圖各導(dǎo)聯(lián)之間最長和最短QT間期的差異程度（最長的QT間期與最短的QT間期之間的差值），能間接性反映患者心室肌復(fù)極化的不一致性及不穩(wěn)定性的變化程度，逐漸成為預(yù)測病人發(fā)生嚴(yán)重室性心律失常和猝死的主要標(biāo)志[1]。通過對該院2013年1月—2015年10月期間收治的95例冠心病患者的臨床觀察，QT間期離散度的判斷，力求研究穩(wěn)心復(fù)脈湯對冠心病患者心電圖QTd的影響及對心肌缺血的改善作用，判斷患者病情恢復(fù)及預(yù)后程度。

1 資料與方法

1.1 一般資料

方便選取該院收治的冠心病患者95例，隨機雙盲法分為兩組，觀察組50例，男31例，女19例；年齡40～80歲，平均（58.9±13.0）歲；對照組45例，男29例，女16例；年齡40～82歲，平均（60.1±12.6）歲；與《實用內(nèi)科學(xué)》[2]中冠心病診斷標(biāo)準(zhǔn)相符合；患者每周心絞痛發(fā)作兩次以上次數(shù)；排除藥物過敏、不穩(wěn)定型心絞痛、急性心肌梗死、肝腎功能異常、認(rèn)知功能不全者；患者此次研究知情，簽署了研究同意書；兩組患者年齡、性別等資料利用統(tǒng)計學(xué)軟件處理（P>0.05），可進(jìn)行比較。
