[摘要] 目的 探究在對(duì)小兒尿道下裂術(shù)后出現(xiàn)小尿瘺患兒的治療中，結(jié)扎法和切開縫合法的不同修復(fù)效果。 方法 方便選取2015年1月—2017年6月在該院進(jìn)行治療的小兒尿道下裂術(shù)后出現(xiàn)小尿瘺患兒40例，分為研究組和對(duì)照組，各20例，其中研究組患兒通過(guò)結(jié)扎法治療，對(duì)照組患兒通過(guò)切開縫合法治療。 結(jié)果 研究組術(shù)后出現(xiàn)再瘺情況患兒3例，其中再瘺瘺口1處2例，2處1例，對(duì)照組出現(xiàn)再瘺情況患兒5例，其中再瘺瘺口1處4例，2處1例，兩組患兒術(shù)后修復(fù)成功率分別為100.00%和90.00%，差異無(wú)統(tǒng)計(jì)學(xué)意義（χ2=0.10；P=0.749 3）。 結(jié)論 在小兒尿道下裂術(shù)后出現(xiàn)小尿瘺患兒的治療中，結(jié)扎法和切開縫合法對(duì)患兒的修復(fù)效果并無(wú)明顯差異，如果患兒的瘺管形態(tài)較為清晰明顯，則推薦使用操作簡(jiǎn)單的結(jié)扎法；如果患兒瘺管形態(tài)不清晰，則推薦使用切開縫合法進(jìn)行修復(fù)。

Comparative Analysis of Clinical Effects of Ligation and Incision Suture in the Treatment of Small Urinary Fistula after Hypospadias in Children

[Abstract] Objective To investigate the different repairing effects of ligation and incision suture in the treatment of children with small urinary fistula after hypospadias. Methods A total of 40 children with small urinary fistula after pediatric hypospadias were convenient treated in the hospital from January 2015 to June 2017. The study group and control group had 20 cases each. The study group was treated with ligation treatment， the control group of children treated by incision suture. Results There were 3 cases of recurrence in the study group， including 2 cases of 1 recurrence， 1 case of 2 recurrences， and 5 cases in the control group， including 4 cases in one part and 1 case in 4 parts， the success rate of postoperative repair was 100.00% and 90.00%， respectively， and there was no significant difference （χ2=0.10； P=0.749 3）. Conclusion In the treatment of children with small urinary fistula after hypospadias in children， the effect of ligation and incision suture on the repair of children is not significant. If the shape of the fistula is clear and obvious， it is recommended to use simple operation ligation method； if the shape of the fistula is not clear， it is recommended to use the incision suture for repair.

在小兒泌尿生殖系統(tǒng)的病癥中，尿道下裂是其中十分常見的一種，在男性新生兒中的發(fā)生率在1%～8%之間，是除隱睪外發(fā)生率最高的一種?，F(xiàn)階段，有幾百種手術(shù)方式可以對(duì)尿道下裂進(jìn)行治療，但在術(shù)后常出現(xiàn)尿瘺的并發(fā)癥，此并發(fā)癥發(fā)生率可達(dá)35%，在目前的手術(shù)方式中尚無(wú)一種方式能夠完全防止尿瘺的出現(xiàn)[1]。皮瓣血運(yùn)障礙、術(shù)后感染以及切口開裂等都是術(shù)后尿瘺出現(xiàn)的原因，其治療也主要以手術(shù)方式為主，但術(shù)后仍存在復(fù)發(fā)的可能性。該文為探究在對(duì)小兒尿道下裂術(shù)后出現(xiàn)小尿瘺患兒的治療中，結(jié)扎法和切開縫合法的不同修復(fù)效果，方便選取了2015年1月—2017年6月在該院進(jìn)行治療的小兒尿道下裂術(shù)后出現(xiàn)小尿瘺患兒40例作為研究對(duì)象，現(xiàn)報(bào)道如下。

1 資料與方法

1.1 一般資料

方便選取該院進(jìn)行治療的小兒尿道下裂術(shù)后出現(xiàn)小尿瘺患兒40例，分為研究組和對(duì)照組，各20例。研究組患兒年齡為2～10個(gè)月，平均年齡為（4.67±1.33）個(gè)月；1處瘺口患兒14例，2處2例，3處2例，共包含瘺口24處；距上次手術(shù)間隔時(shí)間為5～9個(gè)月，平均時(shí)間為（7.88±1.35）個(gè)月。對(duì)照組患兒年齡為3～10個(gè)月，平均年齡為（4.81±1.26）個(gè)月；1處瘺口患兒12例，2處3例，3處2例，共包含瘺口24處；距上次手術(shù)間隔時(shí)間為6～9個(gè)月，平均時(shí)間為（7.91±1.34）個(gè)月。所有患兒的性別、年齡均差異無(wú)統(tǒng)計(jì)學(xué)意義（P>0.05），具有可比性。所有患兒家屬均簽署知情同意書，且該方案通過(guò)倫理委員會(huì)批準(zhǔn)。
