摘要：目的：探討對真菌性上頜竇炎診斷時使用CT的臨床及影像學(xué)分析。方法：在2017年9月-2019年9月的時間段內(nèi)，選擇91例真菌性上頜竇炎患者作為研究組，并選擇同時期進(jìn)行健康體檢的患者91例設(shè)為對照組，研究組經(jīng)過手術(shù)病理證實為真菌性上頜竇炎，使用CT掃描對所有患者進(jìn)行診斷，分析研究組的診斷準(zhǔn)確率，研究組的臨床表現(xiàn)和兩組的影像學(xué)對比。結(jié)果：研究組的診斷準(zhǔn)確率與手術(shù)病理結(jié)果沒有差異（P>0.05），研究組影像學(xué)的特征均比對照組明顯（P<0.05）。結(jié)論：對真菌性上頜竇炎診斷時使用CT，診斷準(zhǔn)確率較高，真菌性上頜竇炎臨床表現(xiàn)主要以膿涕、涕中帶血、壓痛、面部腫脹、鼻塞伴頭痛為主，影像學(xué)為高密度不均勻結(jié)節(jié)狀軟組織，斑點狀、細(xì)條狀團(tuán)塊鈣化灶，骨質(zhì)增生硬化等，因此診斷有臨床推廣的價值。

Clinical and CT Imaging Analysis of 91 Cases of Fungal Maxillary Sinusitis

Abstract： Objective： To investigate the clinical and CT imaging analysis in the diagnosis of fungal maxillary sinusitis. Methods： In the period from September 2017 to September 2019， 91 cases with fungal maxillary sinusitis were selected as study group， and 91 patients with healthy physical examination at the same time were selected as control group. The study group confirmed fungal maxillary sinusitis by surgical pathology， CT scan was used to diagnose all patients， and the diagnostic accuracy of the study group was analyzed. The study groups clinical manifestation was compared with the imaging of the two groups. Results： The study group was no difference between the diagnostic accuracy and surgical pathology（P>0.05）. The imaging characteristic of the study group were more obvious than the control group（P<0.05）. Conclusion： CT was used in the diagnosis of fungal maxillary sinusitis， and the diagnosis accuracy was high. The clinical manifestation of fungal maxillary sinusitis is mainly abscess， nostril with blood， tenderness， facial swelling， nasal congestion and headache. Imaging is a high-density and uneven nodular soft tissue， speckle， thin strips of calcified stove， bone hyperplasia and sclerosis， so the diagnosis has clinical value.

真菌性上頜竇炎是臨床常見的鼻科疾病，主要由于鼻腔鼻竇被真菌感染所導(dǎo)致的，容易與上頜竇癌混淆，影響治療[1]，因此臨床需要對其進(jìn)行準(zhǔn)確診斷，以提高對癥治療的效果。本研究探討對真菌性上頜竇炎診斷時使用CT的臨床及影像學(xué)分析。

1 一般資料

1.1 基本資料

在2017年9月-2019年9月的時間段內(nèi)，選擇91例真菌性上頜竇炎患者作為研究組，并選擇同時期進(jìn)行健康體檢的患者91例設(shè)為對照組，研究組49例女性，42例男性，平均年齡（42.26±8.91）歲。對照組43例女性，48例男性，平均年齡（42.97±9.02）歲?；颊咴谀挲g、性別上均可比（P>0.05），所有患者都已經(jīng)簽署了知情同意書，倫理委員會批準(zhǔn)，納入標(biāo)準(zhǔn)：研究組經(jīng)過手術(shù)病理證實為真菌性上頜竇炎，均自愿參與本次研究;排除標(biāo)準(zhǔn)：其他類型鼻炎、耳鼻疾病，惡性腫瘤，無法配合本次研究。

1.2 研究方法

CT診斷：采用進(jìn)口的多層螺旋CT儀器，患者實行仰臥方式，對患者的頭部進(jìn)行軸位和/或冠狀位掃描，層間和間隔5mm，觀察患者CT圖像。
