肝細(xì)胞生長(zhǎng)因子用于抑制動(dòng)脈旁路轉(zhuǎn)流術(shù)后吻合口內(nèi)膜增生的觀察

【摘要】目的 探討肝細(xì)胞生長(zhǎng)因子用于抑制動(dòng)脈旁路轉(zhuǎn)流術(shù)后吻合口內(nèi)膜增生的效果。方法 將32只實(shí)驗(yàn)兔,隨機(jī)分為對(duì)照組與觀察組各16只。常規(guī)方法建立兔血管旁路轉(zhuǎn)流術(shù)模型,對(duì)照組單純游離移植靜脈,觀察組術(shù)中以一定壓力灌注含肝細(xì)胞生長(zhǎng)因子的肝素生理鹽水,分別于術(shù)后2周、4周切除移植靜脈并進(jìn)行染色、閱片,以全自動(dòng)圖像分析系統(tǒng)測(cè)量?jī)?nèi)膜、中膜厚度,并計(jì)算I/M值。同時(shí)觀察實(shí)驗(yàn)兔術(shù)中術(shù)后大體情況。結(jié)果 觀察組術(shù)后2周、4周內(nèi)膜厚度、中膜厚度、I/M均小于對(duì)照組(P<0.01)。本組實(shí)驗(yàn)兔術(shù)中觀察移植血管彈性和搏動(dòng)均良好、無(wú)滲血;術(shù)后動(dòng)脈搏動(dòng)良好。結(jié)論 肝細(xì)胞生長(zhǎng)因子用于抑制動(dòng)脈旁路轉(zhuǎn)流術(shù)后吻合口內(nèi)膜增生的效果顯著。

Observation of the Effects of Hepatocyte Growth Factor on Inhibit Intimal Hyperplasia of the Anastomotic Stoma After Carotid Artery Bypass Grafting

[Abstract]Objective Study of the effects of hepatocyte growth factor on inhibit Intimal hyperplasia of the anastomotic stoma after carotid artery bypass grafting. Methods Thirty-two rabbits were randomly divided into control group and the observation group. Build the model of rabbit blood vessel bypass grafting by the conventional methods. The veins were pretreated with saline solution(control group)only or pretreated with heparin-saline included hepatocyte growth factor. The vein grafts were harvested at 2 weeks, 4 weeks after operation for staining and reading. The thickness of Intima and media in the vein grafts, I/M was calculated by computer image analysis system. Then observe the basic case in intraoperation and postoperation. Results The thickness of Intima, media in the vein grafts and I/M in observation group were lower than control group(P<0.01). In this group, the elasticity of vein grafts and pulse was good without errhysis in intraoperation, and the pulse was good in postoperation. Conclusion Treatment of veins grafts with hepatocyte growth factor can significantly inhibit intimal hyperplasia in a rabbit carotid artery bypass grafting model.

近年來(lái)的研究發(fā)現(xiàn)肝細(xì)胞生長(zhǎng)因子(HGF)能刺激多種上皮及內(nèi)皮細(xì)胞進(jìn)行有絲分裂、運(yùn)動(dòng),在組織器官的損傷修復(fù)、形態(tài)發(fā)生中發(fā)揮重要作用[1]。本研究以實(shí)驗(yàn)兔為研究對(duì)象,探討了肝細(xì)胞生長(zhǎng)因子用于抑制動(dòng)脈旁路轉(zhuǎn)流術(shù)后吻合口內(nèi)膜增生的效果,報(bào)告如下。
1.1研究材料
實(shí)驗(yàn)用新西蘭白兔32只,均為3~4月齡,體重2.2~2.6 kg,隨機(jī)分為對(duì)照組與觀察組,每組16只;彈力纖維染色試劑盒;0.5%~1%伊紅酒精溶液,蘇木素染液,1%鹽酸酒精分化液,二甲苯,無(wú)水乙醇,95%乙醇,80%乙醇,蒸餾水,中性樹(shù)膠,顯微鏡,全自動(dòng)組織脫水機(jī),石蠟包埋機(jī),切片機(jī),全自動(dòng)圖像分析系統(tǒng),手術(shù)刀,血管鉗,縫合針線,注射器,生理鹽水,肝細(xì)胞生長(zhǎng)因子等。
1.2方法
按照30 mg/kg劑量向?qū)嶒?yàn)兔耳緣靜脈緩慢注射3%戊巴比妥鈉,麻醉起效后將其仰臥固定于手術(shù)臺(tái),頸部去毛、消毒皮膚。將頸部皮膚沿正中線切開(kāi),鉗夾頸外靜脈兩端并截取3 cm,結(jié)扎斷端。以血管夾夾閉右頸總動(dòng)脈兩端后進(jìn)行離斷,兩斷端分別與截取的頸外靜脈兩端進(jìn)行吻合,然后松開(kāi)血管夾使血流恢復(fù)。觀察組在術(shù)中吻合移植靜脈后,鉗夾移植的頸外靜脈兩端,先以生理鹽水沖洗,再用注射器以一定壓力灌注含有肝細(xì)胞生長(zhǎng)因子(100 ng/ml)的肝素生理鹽水,60 s后吸出,重復(fù)3次后恢復(fù)血流;對(duì)照組按上述方式灌注生理鹽水。兩組均于術(shù)后2周、4周隨機(jī)各取材8只兔,麻醉后取出移植靜脈和部分動(dòng)脈留取吻合口周?chē)鷦?dòng)靜脈各0.5 cm進(jìn)行常規(guī)脫水-包埋-切片-HE染色以及彈力纖維染色-顯微鏡閱片,并以全自動(dòng)圖像分析系統(tǒng)測(cè)量?jī)?nèi)膜、中膜厚度,并計(jì)算內(nèi)膜/中膜厚度比值(I/M)。觀察記錄實(shí)驗(yàn)兔術(shù)中術(shù)后情況。
1.3統(tǒng)計(jì)學(xué)方法
采用SPSS 17.0軟件包分析,計(jì)量數(shù)據(jù)采用(x-±s)表示,組間比較采用t檢驗(yàn),以P<0.05為差異對(duì)比有統(tǒng)計(jì)學(xué)意義。
本組實(shí)驗(yàn)兔術(shù)中觀察移植血管彈性和搏動(dòng)均良好、無(wú)滲血;術(shù)后動(dòng)脈搏動(dòng)良好。觀察組術(shù)后2周、內(nèi)膜厚度、中膜厚度、I/M分別為(18.75±2.13)μm、(39.84±2.29)μm、(0.48±0.04),4周內(nèi)膜厚度、中膜厚度、I/M分別為(40.24±2.37)μm、(59.47±2.53)μm、(0.65±0.02),對(duì)照組術(shù)后2周、內(nèi)膜厚度、中膜厚度、I/M分別為(40.76±2.25)μm、(50.30±2.21)μm、(0.82±0.06),4周內(nèi)膜厚度、中膜厚度、I/M分別為(58.77±2.34)μm、(80.64±2.48)μm、(0.77±0.03),觀察組均小于對(duì)照組(P<0.01)。
動(dòng)脈旁路轉(zhuǎn)流術(shù)的轉(zhuǎn)流材料往往以自體靜脈作為首選[2]。近年來(lái)的研究發(fā)現(xiàn)HGF在組織器官的損傷修復(fù)、形態(tài)發(fā)生中發(fā)揮重要作用,能防止動(dòng)脈旁路轉(zhuǎn)流術(shù)后內(nèi)膜增生引起的管腔狹窄、閉塞[3]。本研究中采取的是局部灌注法,在術(shù)中采取肝細(xì)胞生長(zhǎng)因子對(duì)移植靜脈與動(dòng)脈吻合口進(jìn)行處理,通過(guò)觀察術(shù)后2周、4周的內(nèi)膜厚度、中膜厚度、I/M值,發(fā)現(xiàn)移植靜脈的內(nèi)膜厚度隨著時(shí)間的延長(zhǎng)而增加,且對(duì)照組內(nèi)膜厚度增加更為明顯,這表明應(yīng)用了HGF的觀察組內(nèi)膜增生受到明顯抑制。綜上所述,本研究結(jié)果表明HGF對(duì)于動(dòng)脈旁路轉(zhuǎn)流術(shù)后吻合口的內(nèi)膜增生可產(chǎn)生明顯抑制效應(yīng),這一結(jié)果為尋找對(duì)抗術(shù)后吻合口內(nèi)膜增生的方法提供了切實(shí)依據(jù)。
