【摘要】 目的:對(duì)常見頭孢類抗生素進(jìn)行體外抑菌活性研究。方法:利用頭孢氨芐、頭孢唑啉、頭孢呋辛、頭孢替安、頭孢他啶、頭孢哌酮6種常見頭孢類抗生素對(duì)我院臨床分離的562株菌(革蘭氏陽(yáng)性菌206株,革蘭氏陰性菌356株)進(jìn)行體外活性檢測(cè)。結(jié)果:頭孢氨芐、頭孢唑啉(1代)對(duì)革蘭氏陽(yáng)性菌的敏感率為93.2%、96.1%,對(duì)革蘭氏陰性菌的敏感率為52.3%、55.1%;頭孢呋辛、頭孢替安(2代)對(duì)革蘭氏陽(yáng)性菌的敏感率為95.6%、97.1%,對(duì)革蘭氏陰性菌的敏感率為75.3%、76.4%;頭孢他啶、頭孢哌酮(3代)對(duì)革蘭氏陽(yáng)性菌的敏感率為85.4%、87.4%,對(duì)革蘭氏陰性菌的敏感率為90.2%、92.1%。1代、2代頭孢類抗生素對(duì)革蘭氏陽(yáng)性菌的抑菌活性高于3代,而3代頭孢類抗生素對(duì)革蘭氏陰性菌的抑菌活性高于1代、2代,且差異具有統(tǒng)計(jì)學(xué)意義(P<0.05);1代和2代對(duì)革蘭氏陽(yáng)性菌的抑菌活性差異無(wú)統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論:1代、2代頭孢類抗生素體外抑制革蘭氏陽(yáng)性菌效果好于3代頭孢,3代頭孢體外抑制革蘭氏陰性菌效果好于1代、2代。

Study of common cephalosporin antibiotics on the antibacterial activity in vitro

Abstract:[WTHZ]Objective[WTBZ] To study the antibacterial activity in vitro of common cephalosporin antibiotics. [WTHZ]Methods [WTBZ] 562 strains of clinical isolates, 206 strains of gram positive bacteria and 356 strains of gram negative bacteria were selected in our hospital and the above bacterial strains were used to detect and compare the antibacterial activity in vitro of the common cephalosporin antibiotics (cephalosporin, ampicillin, cefazolin, cefuroxime, cefotiam, ceftazidime, cefoperazone). [WTHZ]Results[WTBZ] The sensitive rates of cefalexin and cefazolin (1 st generation) against gram positive bacteria were 93.2% and 96.1 ,The sensitivity rates of those medicines against gram negative bacteria were 52.3% and 55.1%. The sensitive rates of cefuroxime and cefotiam (2 rd generation) the gram positive bacteria were 95.6% and 97.1% while the sensitive rates of those medicines against the gram negative bacteria were 75.3% and 76.4%. The sensitive rates of ceftazidime and cefoperazone (3 rd generation) against the gram positive bacteria were 85.4% while87.4% while the sensitive rate of those medicines against the gram negative bacteria were 90.2% and 92.1%. The antimicrobial activity of 1 st generation and 2 rd generation cephalosporin against gram positive bacteria were higher than that of the 3 rd generation cephalosporin while the antimicrobial activity of the 3 rd generation cephalosporin against gram negative bacteria is higher than those of the 1 st generation and the 2 rd generation. The differences of the antibacterial activity above were (P<0.05). The antimicrobial activity of 1 st generation and 2 rd generation of cephalosporin against gram positive bacteria was not significant difference (P>0.05). [WTHZ]Conclusion[WTBZ] The effect of the 1 st generation and 2 rd generation cephalosporin in vitro against gram positive bacteria is better than that of the 3 rd generation cephalosporin while the effect of the 3 rd generation cephalosporin in vitro against gram negative bacteria is better than that of the 1 st generation and the 2 rd generation cephalosporin.

頭孢類抗生素屬于半合成的廣譜抗生素,于20世紀(jì)60年代問世,目前產(chǎn)品多達(dá)60余種,臨床用于呼吸系統(tǒng)、泌尿系統(tǒng)、皮膚軟組織等感染性疾病。研究發(fā)現(xiàn)頭孢類抗生素可到達(dá)機(jī)體的各個(gè)組織器官,只要?jiǎng)┝孔銐騽t可將致病菌殺死,因此在危重感染中的治療效果好于青霉素、氯霉素和四環(huán)霉素[1-2] 。鑒于頭孢類抗生素的臨床實(shí)用價(jià)值,本文對(duì)頭孢類抗生素中常見的幾種藥物進(jìn)行體外抑菌活性探討,現(xiàn)報(bào)道如下。
1材料與方法
1.1菌株以我院2012年2月至2014年2月收集的562株分離菌為體外試驗(yàn)對(duì)象,其中,革蘭氏陽(yáng)性菌206株(金黃色葡萄球菌78株,肺炎鏈球菌86株,β溶血鏈球菌42株),革蘭氏陰性菌356株(肺炎克雷伯菌96株,大腸埃希菌124株,流感嗜血菌78株,產(chǎn)氣腸桿菌58株)。
1.2藥物頭孢1代:頭孢氨芐(批號(hào):20100929,哈藥集團(tuán)制藥總廠)、頭孢唑啉(批號(hào):20110107,樂普藥業(yè)股份有限公司);頭孢2代:頭孢呋辛(批號(hào):20110318,石藥集團(tuán)中諾藥業(yè)有限公司)、頭孢替安(批號(hào):20100907,上海新亞藥業(yè)有限公司);頭孢3代:頭孢他啶(批號(hào):20110113,齊魯制藥有限公司)、頭孢哌酮(批號(hào):20100925,哈藥集團(tuán)制藥總廠)。
1.3方法根據(jù)美國(guó)國(guó)家臨床實(shí)驗(yàn)室標(biāo)準(zhǔn)委員會(huì)(NCCLS)[3] 推薦瓊脂平皿二倍稀釋法,多點(diǎn)接種儀接種,測(cè)定抗生素MIC值,每1接種點(diǎn),含試驗(yàn)菌1×104個(gè),標(biāo)準(zhǔn)菌作對(duì)照。檢測(cè)6種頭孢抗生素藥物對(duì)受試菌株的最低抑菌濃度(MIC)。
1.4檢測(cè)指標(biāo)MIC即最低抑菌濃度,MIC90指在一批實(shí)驗(yàn)中可抑制90%受試菌所需MIC,藥敏試驗(yàn)MIC:高敏MIC90<1mg/L;中敏MIC90 1~4mg/L;較敏MIC90 4~32mg/L;耐藥MIC90>32mg/L。本研究將MIC90<2 mg/L視為試驗(yàn)菌對(duì)藥物敏感,統(tǒng)計(jì)6種頭孢抗生素對(duì)試驗(yàn)菌的敏感率。
1.5統(tǒng)計(jì)學(xué)分析使用SPSS17.0軟件分析數(shù)據(jù),計(jì)數(shù)資料用[例(%)]表示,χ2檢驗(yàn)比較分析,P<0.05表示具有統(tǒng)計(jì)學(xué)差異。
2結(jié)果
頭孢氨芐、頭孢唑啉(1代)對(duì)革蘭氏陽(yáng)性菌的敏感率為93.2%、96.1%;頭孢呋辛、頭孢替安(2代)對(duì)革蘭氏陽(yáng)性菌的敏感率為95.6%、971%;頭孢他啶、頭孢哌酮(3代)對(duì)革蘭氏陽(yáng)性菌的敏感率為85.4%、87.4%;頭孢氨芐、頭孢唑啉(1代)對(duì)革蘭氏陰性菌的敏感率為52.3%、55.1%;頭孢呋辛、頭孢替安(2代)對(duì)革蘭氏陰性菌的敏感率為75.3%、76.4%;頭孢他啶、頭孢哌酮(3代)對(duì)革蘭氏陰性菌的敏感率為90.2%、92.1%。1代和2代頭孢類抗生素對(duì)革蘭氏陽(yáng)性菌的抑菌活性高于3代(P<0.05),1代和2代對(duì)革蘭氏陽(yáng)性菌的抑菌活性差異無(wú)統(tǒng)計(jì)學(xué)意義,具體見表1。3代頭孢類抗生素對(duì)革蘭氏陰性菌的抑菌活性高于1代、2代(P<0.05),2代對(duì)革蘭氏陰性菌的抑菌活性高于1代,差異具有統(tǒng)計(jì)學(xué)意義(P<0.05),具體見表2。

3討論
頭孢類抗生素可分布于身體各個(gè)器官,主要作用于β內(nèi)酰胺酶來抑制病原菌的生長(zhǎng)[4-5] ,只要致病菌對(duì)其敏感就可以將其選為抗菌藥物使用。頭孢類抗生素的抗菌譜廣泛,無(wú)論是革蘭氏陽(yáng)性菌還是革蘭氏陰性菌均顯示出良好的抑制效果。另外,頭孢類藥物對(duì)細(xì)菌產(chǎn)生的破壞性酶的穩(wěn)定性好,胃酸對(duì)其破壞作用弱,因此藥效穩(wěn)定,且過敏反應(yīng)相對(duì)少于青霉素[6] ,提示頭孢類抗生素在臨床中有著重要的應(yīng)用價(jià)值。目前第4代頭孢抗生素在國(guó)外已經(jīng)廣泛應(yīng)用[7] ,我國(guó)部分醫(yī)院也使用,但多數(shù)基層醫(yī)院仍未普及。本文對(duì)我國(guó)常見的三代頭孢類抗生素進(jìn)行體外抑菌活性探討。頭孢氨芐、頭孢唑啉屬于1代頭孢,體外抗菌試驗(yàn)提示1代頭孢對(duì)革蘭氏陽(yáng)性細(xì)菌的敏感性良好,對(duì)金黃色葡萄球菌、肺炎鏈球菌、和β溶血鏈球菌的敏感率在85%以上,但對(duì)革蘭氏陰性細(xì)菌的敏感性相對(duì)較差,對(duì)肺炎克雷伯菌、大腸埃希菌、流感嗜血菌和產(chǎn)氣腸桿菌的敏感性在60%以下。頭孢呋辛、頭孢替安為2代頭孢,其對(duì)革蘭氏陽(yáng)性菌的抑制效果略好于1代,對(duì)革蘭氏陰性菌的敏感率超過70%,遠(yuǎn)遠(yuǎn)高于1代,有報(bào)道提示2代頭孢多用于病原菌不明確的感染治療[8]。3代頭孢在體外抗革蘭氏陽(yáng)性菌的效果較1代和2代要差,但對(duì)革蘭氏陰性細(xì)菌的抑制效果明顯,對(duì)嚴(yán)重腸道感染有著較好的治療效果。
通過體外抗菌活性試驗(yàn)可知,頭孢類抗菌素的抗菌譜相對(duì)廣泛,對(duì)革蘭氏陽(yáng)性菌和陰性菌均顯示出抑制作用,在臨床預(yù)防和治療感染有著重要的臨床價(jià)值。
