      高滲羥乙基淀粉對(duì)腦缺血大鼠顱內(nèi)壓的影響及機(jī)制探討

      【摘要】目的 探討高滲羥乙基淀粉200/0.5氯化鈉注射液對(duì)腦缺血-再灌注大鼠顱內(nèi)壓和腦水腫的影響及可能機(jī)制。方法 采用隨機(jī)對(duì)照動(dòng)物實(shí)驗(yàn)研究方法,實(shí)驗(yàn)在中山大學(xué)實(shí)驗(yàn)動(dòng)物中心進(jìn)行。取28只雄性SD大鼠,隨機(jī)(隨機(jī)數(shù)字法)分為高滲羥乙基淀粉組、羥乙基淀粉組、對(duì)照組和假手術(shù)組。右側(cè)大腦中動(dòng)脈阻塞(middle cerebral artery occlusion, MCAO)法建立腦梗死再灌注大鼠模型,造模不成功另選大鼠再行手術(shù)補(bǔ)足手術(shù)組,于再灌注開始時(shí)尾靜脈泵入高滲羥乙基淀粉200/0.5氯化鈉注射液和羥乙基淀粉130/0.4氯化鈉注射液,分別為206 mL/(kg·d);造模后0、2、6、12、18、24 h時(shí)間點(diǎn)分別測(cè)量血漿膠體滲透壓(plasma colloid osmotic pressure,COP)和顱內(nèi)壓(intracranial pressure,ICP);治療24 h后測(cè)量右側(cè)大腦半球含水量(brain water content,BWC)。結(jié)果 高滲羥乙基淀粉組、羥乙基淀粉組和對(duì)照組術(shù)后各時(shí)間點(diǎn)ICP均顯著高于假手術(shù)組;高滲羥乙基淀粉組術(shù)后ICP明顯低于對(duì)照組和羥乙基淀粉組, 但羥乙基淀粉組各時(shí)間點(diǎn)ICP和對(duì)照組比較差異無(wú)統(tǒng)計(jì)學(xué)意義。高滲羥乙基淀粉組和羥乙基淀粉組COP各時(shí)間點(diǎn)均顯著高于對(duì)照組和假手術(shù)組,高滲羥乙基淀粉組和羥乙基淀粉組間差異均無(wú)統(tǒng)計(jì)學(xué)意義。高滲羥乙基淀粉組、羥乙基淀粉組和對(duì)照組腦含水量均顯著高于假手術(shù)組[(81.24±0.36)%、(83.04±0.10)%、(83.14±0.41)% vs.(78.37±0.37)%,P=0.000];高滲羥乙基淀粉組腦含水率顯著低于對(duì)照組[(81.24±0.36)% vs.(83.14±0.41)%,P=0.000)]和羥乙基淀粉組[(81.24±0.36)% vs.(83.04±0.10)%,P=0.000];羥乙基淀粉組和對(duì)照組比較差異無(wú)統(tǒng)計(jì)學(xué)意義[(83.04±0.10)% vs.(83.14±0.41)%, P=0.578]。結(jié)論 高滲羥乙基淀粉可明顯改善腦缺血-再灌注大鼠急性期腦水腫、降低顱內(nèi)壓,但未能證實(shí)其提高COP對(duì)顱內(nèi)壓和腦水腫的影響。

      Effects and its mechanisms of hypertonic saline hydroxyethyl starch 200/0.5 solution in rats with ischemic cerebral edema

      【Abstract】Objective To investigate effects and its mechanisms of hypertonic saline hydroxyethyl starch 200/0.5 solution on intracranial pressure and brain water content in rats with ischemic cerebral edema. Methods All experiments were conducted in the animal experimental center of Sun Yat-sen University.The 28 male Sprague-Dawle(SD) rats were randomly(random number) divided into hypertonic saline hydroxyethyl starch group, hydroxyethyl starch group, control group and sham operation group, each n=7. Ischemic cerebral edema model was reproduced by middle cerebral artery occlusion(MCAO),followed by reperfusion after ischemia for 2 hours(If the moldel was not successful, other rats were operated to fill the missing models). Then reperfusion after ischemia 2 hours and received hypertonic saline hydroxyethyl starch and hydroxyethyl starch via tail vein at the beginning of reperfusion. The colloidal osmotic pressure (COP) and intracranial pressure(ICP) were evaluated on 0,2,6,12,18,24 hours after the surgery. The water content of the right hemisphere was measured on 24 h after the surgery. Results The ICP of hypertonic saline hydroxyethyl starch group,hydroxyethyl starch group and control group were significantly higher than that of sham operation group on 2,6,12,18,24 h after the surgery.The ICP of hypertonic saline hydroxyethyl starch group was significantly lower than those of hydroxyethyl starch group and control group on 2, 6,12,18 and 24 h.But there was no significant difference in ICP of the hydroxyethyl starch group compared with that of control group at all time points. The COP of hypertonic saline hydroxyethyl starch group and hydroxyethyl starch group were significantly higher than the control group and sham operation group at each time point; There was no significant difference in COP (mmHg)of the hydroxyethyl starch group compared with that of hypertonic saline hydroxyethyl starch group at all time points. The brain water content (BWC)of hypertonic saline hydroxyethyl starch group,hydroxyethyl starch group and control group were significantly higher than that of sham operation group on 24 hours after the surgery[(81.24±0.36)%,(83.04±0.10)%,(83.14±0.41)% vs. (78.37±0.37)%,all P=0.000], BWC of hypertonic saline hydroxyethyl starch group lower than these of hydroxyethyl starch group[(81.24±0.36)% vs. (83.04±0.10) %,P=0.000] and control group [(81.24±0.36)% vs.(83.14±0.41) %,P=0.000]. There was no significant difference in BWC of the hydroxyethyl starch group compared with that of control group [(83.04±0.10)% vs.(83.14±0.41) %,P=0.578]. Conclusion Hypertonic saline hydroxyethyl starch solution could significantly ameliorate ischemic cerebral edema and reduce ICP,but the relationship between its elevated COP and reduced ICP has not been confirmed.

      缺血性腦卒中早期出現(xiàn)的顱內(nèi)壓升高的處理最常使用方法是脫水療法,即通過(guò)提高血管內(nèi)外滲透壓差達(dá)到治療效果,代表性藥物有甘露醇和高滲鹽水等。除了血漿晶體滲透壓對(duì)顱內(nèi)壓的影響外,研究還發(fā)現(xiàn)調(diào)節(jié)膠體滲透壓可能同樣對(duì)顱內(nèi)壓有調(diào)控作用[1];高滲羥乙基淀粉200/0.5氯化鈉作為一種高滲晶體和人工膠體的有機(jī)組合對(duì)缺血性腦水腫、顱高壓的影響如何?其具體通過(guò)什么機(jī)制發(fā)揮作用?目前尚沒(méi)有相關(guān)的研究報(bào)道。本研究擬采用腦缺血-再灌注大鼠模型觀察應(yīng)用高滲羥乙基淀粉200/0.5氯化鈉和羥乙基淀粉130/0.4氯化鈉對(duì)顱內(nèi)壓和血漿膠體滲透壓的影響。

      1 材料與方法

      1.1 實(shí)驗(yàn)動(dòng)物分組與動(dòng)物模型的制作

      28只6~8周成年SD雄鼠,實(shí)驗(yàn)動(dòng)物由中山大學(xué)實(shí)驗(yàn)動(dòng)物中心提供,體質(zhì)量230~250 g。隨機(jī)(隨機(jī)數(shù)字法)分為高滲羥乙基淀粉組、羥乙基淀粉組、對(duì)照組和假手術(shù)組,每組7只。大鼠禁食12 h、禁水6 h后,氯胺酮100 mg/kg肌注麻醉。⑴左側(cè)顱頂鉆孔硬腦膜下置入Camino Parenchymal catheter (110-4B)型顱內(nèi)壓監(jiān)測(cè)探頭,外接Camino MPM-1型顱內(nèi)壓監(jiān)測(cè)儀(Integra-neurosciences,英國(guó)),以速凝型牙托粉封閉骨孔,縫合皮膚;⑵采用改良MCAO法[4],將預(yù)先經(jīng)多聚賴氨酸處理過(guò)的直徑0.26 mm的線栓經(jīng)大鼠右側(cè)頸內(nèi)動(dòng)脈置入約18~22 mm(根據(jù)動(dòng)物大小調(diào)整插入線栓的深度),阻塞右側(cè)大腦中動(dòng)脈,記錄梗阻開始時(shí)間,并以此時(shí)間點(diǎn)為記錄起點(diǎn);固定外端的線栓并標(biāo)記留在皮膚外面線栓的長(zhǎng)度,縫合皮膚。大鼠麻醉未蘇醒時(shí)應(yīng)注意保暖。頸總動(dòng)脈近心端置入16號(hào)靜脈留置管,肝素鹽水封閉抗凝;血漿膠體滲透壓測(cè)量采用BMT-923型膠體滲透壓測(cè)量?jī)x(Okometer,德國(guó)),肝素化微量注射器通過(guò)頸動(dòng)脈留置管每次抽取100 μL動(dòng)脈血,并注意肝素鹽水封閉。所用線栓均術(shù)前浸漬多聚賴氨酸,60 ℃烘干備用。缺血2 h后,拔出線栓約10 mm,使線栓退至頸總動(dòng)脈杈處,實(shí)現(xiàn)腦缺血-再灌注。并進(jìn)行動(dòng)物模型評(píng)分(Zea longa評(píng)分標(biāo)準(zhǔn)[4]);評(píng)分1~3分為符合實(shí)驗(yàn)要求,第一批21只造模,成功16只,另選大鼠再行手術(shù)補(bǔ)足手術(shù)組(11只造模成功5只);21只造模成功大鼠分為A、B、C組:A組經(jīng)鼠尾靜脈泵入高滲羥乙基淀粉200/0.5氯化鈉注射液(森尤斯卡比公司,德國(guó)),速度為1 mL/h,為高滲羥乙基淀粉組;B組經(jīng)鼠尾靜脈泵入羥乙基淀粉130/0.4氯化鈉注射液費(fèi)(森尤斯卡比公司,德國(guó)),速度為1 mL/h,為羥乙基淀粉組;C組不給予任何處理,為對(duì)照組;假手術(shù)組:除不插線栓外,其余操作與對(duì)照組相同。

      1.2 顱內(nèi)壓和血漿膠體滲透壓監(jiān)測(cè)

      于術(shù)后2、6、12、18和24 h時(shí)點(diǎn)分別采用Okometer BMT-923型膠體滲透壓測(cè)量?jī)x測(cè)量血漿膠體滲透壓(COP),Integraneurosciences Camino MPM-1型顱內(nèi)壓監(jiān)測(cè)儀監(jiān)測(cè)大鼠顱內(nèi)壓(ICP)變化。

      1.3 腦含水量測(cè)定

      大鼠治療24 h后處死,進(jìn)行腦含水量測(cè)定??焖贁囝^取右側(cè)大腦半球,蘸去表面血跡,測(cè)量濕質(zhì)量,將腦組織放入恒溫80 ℃干燥箱內(nèi)干燥72 h,取出稱干質(zhì)量(g)。腦含水率=(濕質(zhì)量-干質(zhì)量)/濕質(zhì)量×100%。

      1.4 統(tǒng)計(jì)學(xué)方法

      所有數(shù)據(jù)經(jīng)SPSS 13.0統(tǒng)計(jì)軟件處理,根據(jù)資料不同類型進(jìn)行相應(yīng)的統(tǒng)計(jì)處理。服從正態(tài)分布的計(jì)量資料以均數(shù)±標(biāo)準(zhǔn)差(x±s)表示;單因素組間比較采用one-way ANOVA方差分析。方差齊,組間多重比較采用LSD-t分析,方差不齊,組間多重比較采用Dunnetts T3分析;以P<0.05為差異具有統(tǒng)計(jì)學(xué)意義。

      2 結(jié)果

      2.1 24 h ICP變化

      0 h各組ICP比較差異無(wú)統(tǒng)計(jì)學(xué)意義(F=0.924,P=0.444),在術(shù)后2、6、12、18、24 h各時(shí)間點(diǎn),高滲羥乙基淀粉組ICP顯著低于羥乙基淀粉組(組間兩兩比較,P<0.05)和對(duì)照組(組間兩兩比較,P<0.05);但顯著高于假手術(shù)組,組間兩兩比較,P=0.000);羥乙基淀粉組和對(duì)照組各時(shí)間點(diǎn)ICP差異無(wú)統(tǒng)計(jì)學(xué)意義(組間兩兩比較,P>0.05),見表1。

      2.2 24 h COP變化

      0 h時(shí)間點(diǎn)COP各組間比較均差異無(wú)統(tǒng)計(jì)學(xué)意義(F=0.586,P=0.630),在術(shù)后2、6、12、18、24 h時(shí)間點(diǎn),高滲羥乙基淀粉組和羥乙基淀粉組COP比較均差異無(wú)統(tǒng)計(jì)學(xué)意義(組間兩兩比較,P>0.05);高滲羥乙基淀粉組和羥乙基淀粉組COP顯著高于對(duì)照組(高滲羥乙基淀粉組與對(duì)照組組間兩兩比較,P<0.05;羥乙基淀粉組與對(duì)照組組間兩兩比較,P<0.05)和假手術(shù)組(高滲羥乙基淀粉組與假手術(shù)組組間兩兩比較,P<0.05;羥乙基淀粉組與假手術(shù)組組間兩兩比較,P<0.05);對(duì)照組與假手術(shù)組在各時(shí)間點(diǎn)均差異無(wú)統(tǒng)計(jì)學(xué)意義(組間兩兩比較,P>0.05),結(jié)果見表2。

      2.3 腦含水量

      高滲羥乙基淀粉組、羥乙基淀粉組和對(duì)照組腦含水量均顯著高于假手術(shù)組[(81.24±0.36)%、(83.04±0.10)%、(83.14±0.41)%vs.(78.37±0.37)%,均P=0.000)];高滲羥乙基淀粉組腦含水率顯著低于對(duì)照組[(81.24±0.36)%vs.(83.14±0.41)%,P=0.000)]和羥乙基淀粉組[(81.24±0.36)%vs.(83.04±0.10)%,P=0.000)];羥乙基淀粉組和對(duì)照組比較差異無(wú)統(tǒng)計(jì)學(xué)意義[(83.04±0.10)%vs.(83.14±0.41)%, P=0.578]。結(jié)果見圖1。
