      阻塞性睡眠呼吸暫停綜合征患者血清同型半胱氨酸、胱抑素C表達(dá)水平及臨床價值

      [摘要] 目的 探討阻塞性睡眠呼吸暫停綜合征(OSAHS)患者血清同型半胱氨酸、胱抑素C表達(dá)水平及其臨床意義。 方法 選擇2015年1月~2017年7月在山東省臨朐縣人民醫(yī)院(以下簡稱“我院”)就診的174例OSAHS患者作為OSAHS組,同時選取我院健康體檢者90例作為對照組。比較兩組血清同型半胱氨酸、胱抑素C水平,采用Pearson相關(guān)分析對同型半胱氨酸、胱抑素C與呼吸暫停低通氣指數(shù)(AHI)進(jìn)行相關(guān)分析。根據(jù)病情輕重將OSAHS患者分為輕、中、重三組,比較三組同型半胱氨酸、胱抑素C水平。 結(jié)果 OSAHS組血清同型半胱氨酸、胱抑素C均高于對照組,差異有高度統(tǒng)計學(xué)意義(P < 0.01)。Pearson相關(guān)性分析顯示,血清同型半胱氨,、胱抑素C與AHI呈高度正相關(guān)(r = 0.79、0.74,P < 0.01)。輕、中、重度OSAHS組同型半胱氨酸、胱抑素C比較,差異有統(tǒng)計學(xué)意義(P < 0.05)。進(jìn)一步兩兩比較,中、重度OSAHS組高于輕度OSAHS組,重度OSAHS組高于中度OSAHS組,差異均有統(tǒng)計學(xué)意義(P < 0.05)。 結(jié)論 OSAHS患者血清同型半胱氨酸、胱抑素C表達(dá)水平高于正常人群,并與患者AHI呈正相關(guān),可作為輔助診斷及評估OSAHS病情的敏感指標(biāo)。

      Levels and clinical value of serum homocysteine and cystatin C in patients with obstructive sleep apnea syndrome

      [Abstract] Objective To explore the expression level of homocysteine and cystatin C in patients with obstructive sleep apnea syndrome and its clinical value. Methods From January 2015 to July 2017, 174 patients with OSAHS in People′s Hospital of Linqu County were selected as the OSAHS group. At the same time, 90 healthy people in the same hospital were selected as the control group. The levels of serum homocysteine and cystatin C in the two groups were compared. Pearson correlation analysis was used to analyze the correlation of homocysteine, cystatin C and apnea hypopnea index (AHI). Patients in OSAHS group were divided into mild, moderate and severe OSAHS group according to the severity of the disease and the homocysteine, Cystatin C in the three groups were compared. Results The homocysteine and cystatin C in the OSAHS group were higher than those of control group, with highly statistically significant differences (P < 0.01). Pearson correlation analysis showed that serum homocysteine and cystatin C were positively correlated with AHI, with highly statistically significant differences (r = 0.79,0.74,P < 0.01). There were statistically significant differences in homocysteine and cystatin C among the three OSAHS groups (P < 0.05), the above indicators in moderate and severe OSAHS group were higher than those of mild OSAHS group and those in the severe OSAHS group were higher than those of moderate OSAHS group, with statistically significant differences (P < 0.05). Conclusions The level of serum homocysteine and cystatin C in patients with OSAHS were significantly higher than those of normal population and are positively correlated with AHI, which can be used as a sensitive indicator for the diagnosis and evaluation of OSAHS.

      阻塞性睡眠呼吸暫停綜合征(obstructive sleep apnea hypopnea syndrome,OSAHS)是一種常見的睡眠呼吸障礙疾病,其病因十分復(fù)雜,具體發(fā)病機(jī)制尚未完全闡明[1]。主要表現(xiàn)為夜間睡眠打鼾并伴有呼吸暫停,每次呼吸暫停在10 s以上,并伴有血氧飽和度下降。目前臨床上對于OSAHS的診斷主要以患者的臨床表現(xiàn)為主,尚缺乏客觀的檢驗指標(biāo)。有研究表明,由于反復(fù)發(fā)作的呼吸暫停可導(dǎo)致機(jī)體缺氧和高碳酸血癥,交感神經(jīng)興奮性升高,炎性反應(yīng)及氧化應(yīng)激反應(yīng)增強[2]。血清同型半胱氨酸、胱抑素C與炎性反應(yīng)及氧化應(yīng)激關(guān)系密切,本研究通過對OSAHS患者血清同型半胱氨酸、胱抑素C表達(dá)水平的研究,探討血清同型半胱氨酸、胱抑素C與OSAHS的關(guān)系及臨床價值。

      1 資料與方法

      1.1 一般資料

      選取2015年1月~2017年7月在山東省臨朐縣人民醫(yī)院(以下簡稱“我院”)就診的174例OSAHS患者作為OSAHS組。OSAHS診斷標(biāo)準(zhǔn)參考中華醫(yī)學(xué)會呼吸病學(xué)分會制訂的《阻塞性睡眠呼吸暫停低通氣綜合征診治指南》(2011年修訂版)[3]:①睡眠中口鼻呼吸氣流完全停止>10 s;②呼吸氣流強度與幅度比值較基礎(chǔ)值降低>50%,同時血氧飽和度較基礎(chǔ)值下降>4%;③呼吸暫停低通氣指數(shù)(apnea hypopnea index,AHI)每小時≥5次。排除惡性腫瘤、嚴(yán)重心肺疾患、腦卒中、炎癥性疾病及嚴(yán)重肝腎功能異常者。同時選取同時期我院非OSAHS的健康體檢者90例作為對照組。本研究經(jīng)我院醫(yī)學(xué)倫理委員會批準(zhǔn),患者或其家屬知情同意并簽署知情同意書。兩組年齡、性別、吸煙史、高血壓、糖尿病、體重指數(shù)(BMI)等一般資料比較,差異無統(tǒng)計學(xué)意義(P > 0.01),具有可比性。見表1。

      1.2 OSAHS患者嚴(yán)重程度分組

      對所有入選OSAHS組的患者采用美國Embla多導(dǎo)睡眠監(jiān)測儀進(jìn)行多導(dǎo)睡眠監(jiān)測,記錄患者在7 h睡眠過程中的呼吸暫停次數(shù)及低通氣次數(shù),計算呼吸暫停低通氣指數(shù)(apnea hypopnea index,AHI)。AHI是指平均每小時睡眠中呼吸暫停與低通氣次數(shù)之和。依據(jù)《阻塞性睡眠呼吸暫停低通氣綜合征診治指南》(2011年修訂版)[3],根據(jù)OSAHS患者病情嚴(yán)重程度,以AHI為標(biāo)準(zhǔn)對OSAHS患者進(jìn)行再分組:AHI 5~<15次為輕度OSAHS組,AHI 15~<30次為中度OSAHS組;AHI>30次為重度OSAHS組。

      1.3 觀察指標(biāo)

      所有入選者于清晨空腹,抽取肘靜脈血6 mL,應(yīng)用離心機(jī)3000 r/min離心15 min,收集血清進(jìn)行血清同型半胱氨酸、胱抑素C測定。血清同型半胱氨酸試劑盒購自北京愛必信生物技術(shù)有限公司,檢測方法為酶聯(lián)免疫吸附法,儀器為羅氏P800全自動生化分析儀。胱抑素C測定試劑盒購自常州愛復(fù)康科技有限公司,采用免疫比濁法,由日本AU-640全自動生化分析儀進(jìn)行檢測。

      1.4 統(tǒng)計學(xué)方法

      采用SPSS 17.0對所有數(shù)據(jù)進(jìn)行統(tǒng)計分析,計數(shù)資料采用卡方檢驗。計量資料采用均數(shù)±標(biāo)準(zhǔn)差(x±s)表示,多組計量資料比較采用單因素方差分析。相關(guān)性研究采用Pearson相關(guān)性分析。P < 0.05表示差異有統(tǒng)計學(xué)意義。

      2 結(jié)果

      2.1 兩組同型半胱氨酸、胱抑素C及常規(guī)生化指標(biāo)比較

      OSAHS組血清同型半胱氨酸、胱抑素C,明顯高于對照組,差異有高度統(tǒng)計學(xué)意義(P < 0.01)。見表2。

      2.2 同型半胱氨酸、胱抑素C與OSAHS的Pearson相關(guān)性分析

      將OSAHS組患者血清同型半胱氨酸、胱抑素C統(tǒng)計數(shù)據(jù)與AHI進(jìn)行Pearson相關(guān)性分析,結(jié)果顯示,血清同型半胱氨酸、胱抑素C均與AHI呈高度正相關(guān)(r = 0.79、0.74,P < 0.01)。見圖1。

      2.3 不同嚴(yán)重程度OSAHS組患者同型半胱氨酸、胱抑素C比較

      輕、中、重度OSAHS組患者血清同型半胱氨酸、胱抑素C水平比較,差異有高度統(tǒng)計學(xué)意義(P < 0.01)。隨著OSAHS嚴(yán)重程度的加重,血清同型半胱氨酸、胱抑素C水平逐漸升高,進(jìn)一步兩兩比較,中、重度OSAHS組高于輕度OSAHS組,重度OSAHS組高于中度OSAHS組,差異均有統(tǒng)計學(xué)意義(P < 0.05)。見表3。

      3 討論

      OSAHS是一種以夜間睡眠呼吸暫停為主要表現(xiàn)的疾病,呼吸暫停是指睡眠中口鼻呼吸氣流完全停止>10 s。低通氣是指呼吸氣流在睡眠時較基礎(chǔ)水平降低超過30%以上,并伴血氧飽和度下降或覺醒。其發(fā)病率在40歲以上的人群中約為4%。OSAHS可導(dǎo)致患者低氧血癥和高碳酸血癥,使患者正常的睡眠狀態(tài)受到干擾,白天嗜睡、頭痛、記憶力下降等[4-6]。大量研究表明,OSAHS是糖尿病、高血壓、冠心病、腦卒中等多種疾病的獨立危險因素[7-11]。OSAHS具體發(fā)病機(jī)制尚未完全闡明,可能與氧化應(yīng)激、炎癥因子、內(nèi)皮細(xì)胞破壞等因素有關(guān)[12],在上述因素中,血清同型半胱氨酸及胱抑素C均發(fā)揮著重要作用。

      同型半胱氨酸是一種含硫氨基酸,是蛋氨酸的中間代謝產(chǎn)物。同型半胱氨酸是動脈粥樣硬化的危險因素之一,研究表明,同型半胱氨酸可促使巨噬細(xì)胞對低密度脂蛋白攝取增加,同時還能氧化低密度脂蛋白,使泡沫細(xì)胞增加,參與動脈粥樣硬化過程[13]。有研究發(fā)現(xiàn),同型半胱氨酸對內(nèi)皮有毒性作用,可導(dǎo)致內(nèi)皮細(xì)胞損傷及功能失調(diào),其機(jī)制可能是,在同型半胱氨酸氧化過程中產(chǎn)生了自由基[14]。同型半胱氨酸還能降低NO的活性,加速膠原堆積,并促使中性粒細(xì)胞和單核細(xì)胞黏附于內(nèi)皮[15]。國外研究表明,同型半胱氨酸水平在OSAHS患者體內(nèi)顯著升高,但具體機(jī)制尚不明確[16]。有研究認(rèn)為OSAHS患者體內(nèi)的同型半胱氨酸水平升高,可能與低氧及高碳酸血癥對同型半胱氨酸代謝過程中的某些酶產(chǎn)生影響有關(guān)[17]。本研究結(jié)果顯示,OSAHS患者血清同型半胱氨酸表達(dá)水平比對照組明顯升高,且隨病情進(jìn)展逐漸升高。

      血清胱抑素C是一種低分子量蛋白質(zhì),其編碼基因位于人類第20號染色體。于1983年被首次發(fā)現(xiàn),對維持細(xì)胞外基質(zhì)的平衡具有重要作用[18]。胱抑素C在體內(nèi)比較穩(wěn)定,不受年齡、性別等影響,能被腎小球濾過,然后在近曲小管再次重吸收,可反映腎小球濾過功能,一開始主要作為評估腎功能的指標(biāo),后來研究發(fā)現(xiàn),其表達(dá)水平與心血管疾病關(guān)系密切[19]。近期國外研究表明,血清胱抑素C在OSAHS患者中呈高水平表達(dá),并與病情的嚴(yán)重程度呈正相關(guān)[20]。OSAHS患者血清胱抑素C升高的具體機(jī)制尚不明確,有學(xué)者認(rèn)為OSAHS導(dǎo)致患者低氧血癥及高碳酸血癥,可促使機(jī)體交感神經(jīng)興奮,下丘腦-垂體-腎上腺軸功能紊亂[21]。也有學(xué)者認(rèn)為低氧血癥可導(dǎo)致氧化應(yīng)激和炎細(xì)胞因子表達(dá)異常,引起血管內(nèi)皮損傷,胱抑素C調(diào)控基因相關(guān)蛋白酶含量升高,進(jìn)而促使胱抑素C大量表達(dá)[22]。本研究結(jié)果顯示,OSAHS患者血清胱抑素C表達(dá)水平比對照組明顯升高,且其表達(dá)水平與AHI呈高度正相關(guān),并隨病情進(jìn)展逐漸升高。

      綜上所述,本研究認(rèn)為血清同型半胱氨酸及胱抑素C與OSAHS關(guān)系密切,臨床上可通過檢測血清同型半胱氨酸及胱抑素C水平變化來判斷療效及預(yù)后。本研究時間尚短,樣本量相對較少,尚需長時間、大樣本臨床觀察予以進(jìn)一步證實。
