1神經(jīng)性理性疼痛的概述
神經(jīng)性理性疼痛是一種軀體感覺神經(jīng)系統(tǒng)損傷或者疾病引起的疼痛。糖尿病、帶狀皰疹病毒、腫瘤和獲得性免疫缺陷綜合征等都可以導致外周神經(jīng)發(fā)生損傷,當損傷持續(xù)存在較長時間后,就會產(chǎn)生痛覺異?;蛘咄从X過敏的現(xiàn)象。在世界范圍內(nèi),我國人群的糖尿病患病率較高,約為10%,大概50%的糖尿病病人會發(fā)展成糖尿病神經(jīng)病變,其中又高達25%的病人會發(fā)生糖尿病神經(jīng)病理性疼痛[1-2]。
2糖尿病神經(jīng)病理性疼痛的概述
糖尿病神經(jīng)病理性疼痛的主要癥狀為麻木、疼痛和感覺異常,癥狀嚴重程度一般為中等到重度的程度,容易在夜間惡化,嚴重影響睡眠質(zhì)量。病痛會對病人的日常生活和情緒產(chǎn)生負面作用,嚴重影響病人的生活質(zhì)量。糖尿病神經(jīng)病理性疼痛給病人和社會帶來了沉重的經(jīng)濟負擔[3-5]。
3糖尿病神經(jīng)病理性疼痛的發(fā)病機制
在糖尿病神經(jīng)病變的研究領域,科學家們一直在不斷地探索。目前認為,糖尿病神經(jīng)病變的發(fā)生涉及到多種途徑的綜合作用,這些途徑包括多元醇途徑、己糖胺途徑、蛋白激酶C途徑、晚期糖基化終末產(chǎn)物的聚集和能量代謝途徑。這些途徑的激活導致基因表達的改變、炎癥和氧化應激反應以及線粒體功能的紊亂,最終導致糖尿病神經(jīng)病變的發(fā)生。我們的治療原則在高血糖血脂代謝異常的基礎上得到非降脂機制,小膠質(zhì)細胞,白細胞,ROS,TGF等,具體見圖1[6-9]。
