      目的 采用131碘-間碘芐胍(131I-MIBG)心肌顯像方法評(píng)估帕金森病(PD)、多系統(tǒng)萎縮(MSA)及特發(fā)性震顫(ET)患者的心臟交感神經(jīng)功能,以期為上述疾病的鑒別探索新的方法。方法 收集PD患者25例,MSA患者18例,ET患者11例和正常對(duì)照者10例,所有受試者均行131I-MIBG心肌顯像,分別測(cè)定計(jì)算15 min和4 h兩個(gè)時(shí)間點(diǎn)的心臟/縱膈(H/M)放射性計(jì)數(shù)比值,作為131I-MIBG心肌攝取率。結(jié)果 PD組15 min和4 h兩個(gè)時(shí)間點(diǎn)的H/M值分別為1.65±0.36和1.50±0.43,MSA組分別為1.97±0.36和2.08±0.57,ET組分別為2.34±0.34和2.46±0.51,對(duì)照組分別為2.41±0.26和2.66±0.47。兩兩比較,PD組兩個(gè)時(shí)間點(diǎn)的H/M值均低于MSA組、ET組和對(duì)照組相應(yīng)值(均P<0.05),MSA組兩個(gè)時(shí)間點(diǎn)的H/M值均低于對(duì)照組相應(yīng)值(均P<0.05),ET組與對(duì)照組兩個(gè)時(shí)間點(diǎn)的H/M值差異均無(wú)統(tǒng)計(jì)學(xué)意義(均P>0.05)。結(jié)論 PD、MSA患者均可發(fā)生心臟交感神經(jīng)功能損害,且PD患者損害更著,而ET患者心臟交感神經(jīng)功能無(wú)明顯改變。131I-MIBG心肌顯像可能有助于PD、MSA及ET的鑒別診斷。

      帕金森病(PD)、多系統(tǒng)萎縮(MSA)、特發(fā)性震顫(ET)均為錐體外系疾病,且治療原則各異,但有時(shí)臨床鑒別困難,尤其在疾病早期。間碘芐胍(MIBG)是去甲腎上腺素(NE)功能類似物,可被體內(nèi)交感神經(jīng)末梢攝取,其攝取、儲(chǔ)存和釋放機(jī)制與NE相同,MIBG心肌顯像可以直觀且定量的觀察心臟交感神經(jīng)完整性及功能狀態(tài),成為評(píng)估心臟交感神經(jīng)功能的敏感指標(biāo)[1~4]。本研究采用131碘標(biāo)記的MIBG(131I-MIBG)心肌顯像方法評(píng)估PD、MSA及ET患者的心臟交感神經(jīng)功能,并比較其131I-MIBG心肌攝取率,以期為上述疾病的鑒別探索新的方法。

      1 資料和方法

      1.1 研究對(duì)象 選取2013年1月-2015年7月就診于北京大學(xué)人民醫(yī)院和中日友好醫(yī)院神經(jīng)內(nèi)科的PD患者25例,MSA患者18例,ET患者11例和正常對(duì)照者10例。入組標(biāo)準(zhǔn):PD組符合1992年英國(guó)腦庫(kù)PD診斷標(biāo)準(zhǔn)[5],MSA組符合2008年Gilman等制定的第二版MSA診斷標(biāo)準(zhǔn)[6],ET組診斷標(biāo)準(zhǔn)參照1998年國(guó)際運(yùn)動(dòng)障礙協(xié)會(huì)制定的震顫共識(shí)[7],對(duì)照組:性別和年齡與病例組相匹配,神經(jīng)系統(tǒng)查體無(wú)異常的健康志愿者。排除標(biāo)準(zhǔn):(1)腦卒中、腦外傷、腦炎、顱內(nèi)腫瘤等中樞神經(jīng)系統(tǒng)疾?。?2)心肌梗死、心肌病、心律失常、充血性心力衰竭等心臟疾?。?3)正在服用影響131I-MIBG攝取的藥物[8],包括利血平、拉貝洛爾、鈣通道阻滯劑、三環(huán)類抗抑郁藥、可卡因、麻黃堿、苯丙胺等;(4)甲狀腺疾病、碘過敏史。向所有受試者詳細(xì)告知本研究的目的和流程,并簽署知情同意書。

      1.2 研究方法

      1.2.1 基線資料采集 記錄所有受試者的基本特征(年齡、性別),由神經(jīng)科??漆t(yī)師對(duì)其進(jìn)行病史采集,詳細(xì)詢問病史,包括起病年齡、病程、首發(fā)及主要癥狀、病情進(jìn)展順序、診療經(jīng)過,既往是否有中樞神經(jīng)系統(tǒng)、心臟等病史,特殊藥物服用、毒物接觸等病史,家族史等,仔細(xì)閱讀頭部CT或MRI,并進(jìn)行詳細(xì)的神經(jīng)系統(tǒng)查體。采用Hoehn-Yahr(H-Y)分級(jí)[9]對(duì)PD患者進(jìn)行病情評(píng)估。

      1.2.2131I-MIBG心肌顯像

      (1)受試者于檢查前3 d開始口服復(fù)方碘溶液0.3~0.5 ml 3次/日以封閉甲狀腺,并持續(xù)至檢查后1 w。(2)受試者在靜脈注射131I-MIBG 3 mCi后,使用PHILIPS Precedence6 SPECT/CT成像儀,分別于15 min和4 h采集胸部前位平面像,通過勾畫感興趣區(qū)(ROI),計(jì)算心臟/縱膈(heart/mediastinum,H/M)放射性計(jì)數(shù)比值,以此作為131I-MIBG心肌攝取率。

      2 結(jié) 果

      2.1 組間基線資料比較 PD組共25例(其中H-YⅠ級(jí)8例,Ⅱ級(jí)11例,Ⅲ級(jí)4例,Ⅳ級(jí)1級(jí),Ⅴ級(jí)1例),男14例,女11例,年齡43~80歲,平均65.40±9.18歲;MSA組共18例,男12例,女6例,年齡48~80歲,平均62.50±8.73歲;ET組共11例,男5例,女6例,年齡53~81歲,平均66.64±9.64歲;對(duì)照組共10例,男2例,女8例,年齡43~76歲,平均59.00±9.06歲(見表1)。4組間在性別(P>0.05,χ2檢驗(yàn))和年齡(P>0.05,單因素方差分析)上相匹配。

      2.2 組間131I-MIBG心肌攝取率(H/M值)比較 PD組15 min和4 h兩個(gè)時(shí)間點(diǎn)的H/M值分別為1.65±0.36和1.50±0.43,MSA組分別為1.97±0.36和2.08±0.57,ET組分別為2.34±0.34和2.46±0.51,對(duì)照組分別為2.41±0.26和2.66±0.47(見表2、圖1、圖2、圖3、圖4)。兩兩比較,PD組兩個(gè)時(shí)間點(diǎn)的H/M值均顯著低于MSA組、ET組和對(duì)照組相應(yīng)值(均P<0.05),MSA組兩個(gè)時(shí)間點(diǎn)的H/M值均顯著低于對(duì)照組相應(yīng)值(均P<0.05),ET組與對(duì)照組兩個(gè)時(shí)間點(diǎn)的H/M值差異均無(wú)統(tǒng)計(jì)學(xué)意義(均P>0.05)。

      3 討 論

      MIBG是NE的功能類似物,MIBG的攝取主要包括Ⅰ型攝取(神經(jīng)攝取)和Ⅱ型攝取(非神經(jīng)攝取)兩種機(jī)制,Ⅰ型攝取需依賴去甲腎上腺素轉(zhuǎn)運(yùn)體(NET),Ⅱ型攝取為被動(dòng)擴(kuò)散機(jī)制。通常采集早期(15 min)與延遲期(4 h)兩次顯像,計(jì)算H/M值,以此作為MIBG心肌攝取率。早期顯像主要反映突觸前心臟交感神經(jīng)末梢的密度,大約3~4 h后,非神經(jīng)攝取部分幾乎完全被清除,而神經(jīng)攝取部分清除率較低,因此延遲期顯像可同時(shí)反映心臟交感神經(jīng)的功能活性,更能準(zhǔn)確的評(píng)估心臟交感神經(jīng)功能[1~4]。

      本研究發(fā)現(xiàn),PD患者M(jìn)IBG心肌攝取率較MSA組、ET組、對(duì)照組均顯著下降,與國(guó)外研究結(jié)果相似[2~4,10~13]。既往研究同時(shí)提示,PD早期即可出現(xiàn)MIBG心肌攝取率下降,本研究納入PD患者25例,其中早期PD(H-Y Ⅰ級(jí)和Ⅱ級(jí))共19例,似乎支持上述觀點(diǎn)。目前認(rèn)為,PD、路易體癡呆(DLB)、純自主神經(jīng)功能衰竭(PAF)等疾病存在相同的病理特征[14,15],神經(jīng)元內(nèi)均有α-突觸核蛋白及路易小體形成,統(tǒng)稱為路易小體疾病(LBD)。PD患者自主神經(jīng)系統(tǒng)受累廣泛,在下丘腦、交感神經(jīng)系統(tǒng)(胸髓中間外側(cè)柱、交感神經(jīng)節(jié))、副交感神經(jīng)系統(tǒng)(動(dòng)眼神經(jīng)E-W核、泌涎核、迷走神經(jīng)背核、副交感神經(jīng)節(jié))、心臟神經(jīng)叢、消化道、盆神經(jīng)叢、腎上腺髓質(zhì)等部位均有路易小體發(fā)現(xiàn)[16~18]。尸檢已證實(shí),PD患者心臟中酪氨酸羥化酶(TH)免疫反應(yīng)陽(yáng)性交感神經(jīng)軸索數(shù)目降低[16~18]。近期一項(xiàng)研究納入了23例LBD患者,所有患者的診斷均經(jīng)尸檢病理證實(shí),且生前均曾行MIBG心肌顯像,收集患者心肌組織行免疫組化染色,研究顯示早期和延遲期分別有90.9%和95.7%的患者H/M值下降,心肌中TH免疫反應(yīng)陽(yáng)性軸索面積、神經(jīng)微絲蛋白(NF)免疫反應(yīng)陽(yáng)性軸索面積與早期及延遲期H/M值均顯著相關(guān)(均P<0.05),該研究首次確認(rèn)LBD患者M(jìn)IBG心肌攝取率下降與心臟交感神經(jīng)軸索缺失存在緊密關(guān)聯(lián)[19]。以上提示PD發(fā)病時(shí),心臟交感神經(jīng)系統(tǒng)可受累,心臟節(jié)后交感神經(jīng)末梢密度降低,PD患者M(jìn)IBG心肌攝取率下降與心臟交感失神經(jīng)密切相關(guān)。

      對(duì)于鑒別PD與MSA,Braune等[20]通過meta分析發(fā)現(xiàn),MIBG心肌顯像的靈敏度和特異度分別可達(dá)89.7%和94.6%。Orimo等[3]發(fā)現(xiàn),MIBG心肌顯像鑒別PD與MSA的靈敏度和特異度分別可達(dá)90.2%和81.9%。Tregla等[2]對(duì)19項(xiàng)研究、共計(jì)1972例患者進(jìn)行meta分析,發(fā)現(xiàn)MIBG心肌顯像鑒別PD與其他帕金森綜合征的靈敏度和特異度分別為88%和85%。Orimo等[3]對(duì)13項(xiàng)研究、共計(jì)845例患者進(jìn)行meta分析,發(fā)現(xiàn)延遲期H/M值鑒別PD與其他伴帕金森綜合征的神經(jīng)系統(tǒng)變性疾病的靈敏度和特異度分別為89.7%和82.6%,若將PD患者限定在病情早期(H-Y Ⅰ級(jí)和Ⅱ級(jí)),延遲期H/M值用于PD診斷的靈敏度和特異度分別可達(dá)94.1%和80.2%。以上提示,MIBG心肌顯像對(duì)PD的診斷及鑒別診斷有較高的靈敏度和特異度,對(duì)于早期PD患者,其靈敏度更高。

      傳統(tǒng)觀點(diǎn)認(rèn)為,PD主要為周圍節(jié)后交感神經(jīng)受累,而MSA主要為中樞節(jié)前交感神經(jīng)受累,其節(jié)后交感神經(jīng)通常幸免[21,22]。然而本研究發(fā)現(xiàn),MSA患者M(jìn)IBG心肌攝取率較對(duì)照組下降,與既往部分研究結(jié)果一致[12,23,24],提示MSA患者心臟節(jié)后交感神經(jīng)也可同時(shí)受累。Orimo等[25]對(duì)15例經(jīng)病理證實(shí)的MSA患者尸檢發(fā)現(xiàn),6例病程較長(zhǎng)的患者心臟中TH免疫反應(yīng)陽(yáng)性軸索數(shù)目輕度下降。Druschky等[12]提出,可能由于跨突觸效應(yīng)的作用,導(dǎo)致MSA患者節(jié)后交感神經(jīng)功能受損,影響心肌MIBG的攝取,且MIBG心肌攝取率可能與病程相關(guān)。

      ET是一種以姿勢(shì)性或動(dòng)作性震顫為主要特點(diǎn)的運(yùn)動(dòng)障礙性疾病,隨著研究的深入,發(fā)現(xiàn)18.8%的ET患者可出現(xiàn)靜止性震顫[26],而PD患者也可伴姿勢(shì)性震顫,二者有時(shí)鑒別困難。本研究顯示,ET患者M(jìn)IBG心肌攝取率與對(duì)照組相比,差異無(wú)統(tǒng)計(jì)學(xué)意義。在Orimo等[11]、Lee等[27]的研究中,均有同樣的發(fā)現(xiàn)。Rajput等[28]收集了9例ET尸檢病例,未發(fā)現(xiàn)ET患者存在與PD相一致的病理改變。以上提示,ET患者心臟交感神經(jīng)功能無(wú)明顯受累。

      本研究顯示,PD、MSA患者均可發(fā)生心臟交感神經(jīng)功能損害,且PD患者損害更著,而ET患者心臟交感神經(jīng)功能無(wú)明顯改變。131I-MIBG心肌顯像可能有助于PD、MSA及ET的鑒別診斷。下一步還需要進(jìn)一步擴(kuò)大樣本量,并對(duì)患者進(jìn)行長(zhǎng)期隨訪,以不斷完善該研究。

      Objective The present study is to assess the cardiac sympathetic nerve function in patients with Parkinson’s disease (PD),multiple system atrophy(MSA) and essential tremor(ET) using131I-MIBG myocardial scintigraphy and to explore a new method for differential diagnosis among them with expectation. Methods Twenty-five PD,eighteen MSA,eleven ET and ten normal controls were enrolled.131I-MIBG myocardial scintigraphy was performed on each subject and then heart to mediastinum (H/M) ratios were calculated at two sampled time (15 min and 4 h),which represented the131I-MIBG myocardial uptake ratios. Results The H/M ratios at two sampled time (15 min and 4 h) in the PD group were 1.65±0.36 and 1.50±0.43,and those of the MSA group were 1.97±0.36 and 2.08±0.57,the ET group were 2.34±0.34 and 2.46±0.51,and the control group were 2.41±0.26 and 2.66±0.47. The H/M ratios at two sampled time in the PD group were lower than those of the MSA,ET and control group respectively,with statistical significance (allP<0.05). The H/M ratios at two sampled time in the MSA group were significantly lower than those of the control group respectively (allP<0.05). There was no significant difference in H/M ratios at two sampled time between the ET and control groups (allP>0.05). Conclusion Cardiac sympathetic dysfunction could occur in both PD and MSA patients,especially in PD patients. In contrast,cardiac sympathetic function remains in ET patients. Above all,131I-MIBG myocardial scintigraphy may help realize differential diagnosis among PD,MSA and ET patients.
