系統(tǒng)性炎癥反應(yīng)與新輔助放化療直腸癌患者的臨床預(yù)后研究

目的 分析系統(tǒng)性炎癥反應(yīng)(systemic inflammatory response, SIR)與晚期直腸癌患者的病理完全緩解(pathological complete response, pCR)及預(yù)后的關(guān)系。方法 檢測(cè)114例中低位直腸癌患者接受新輔助放化療前后的血漿白蛋白、中性粒細(xì)胞、淋巴細(xì)胞值、中性粒細(xì)胞淋巴細(xì)胞比值(neutrophil-lymphocyte ratio,NLR)、血小板淋巴細(xì)胞比值(platelet-lymphocyte ratio,PLR)、中性粒細(xì)胞白蛋白比值(neutrophil-albumin ratio,NAR),并計(jì)算放化療前后比值變化率,分析以上各指標(biāo)與患者pCR及預(yù)后的關(guān)系。結(jié)果 單因素與多因素分析發(fā)現(xiàn),NLR變化率(P=0.012)與NAR變化率(P=0.028)是患者pCR的獨(dú)立預(yù)測(cè)因子;NLR變化率是患者3年總生存[P=0.015]與3年無(wú)病生存[P=0.019]獨(dú)立預(yù)測(cè)因子。結(jié)論 NLR變化率是接受新輔助放化療的直腸癌患者pCR與生存的獨(dú)立預(yù)測(cè)因子,或可作為評(píng)估工具判斷治療反應(yīng)及決定治療選擇。

新輔助放化療是低位尤其是伴有淋巴結(jié)轉(zhuǎn)移的直腸癌患者的首選治療方式,放化療后病理完全緩解率(pathological complete response, pCR)可達(dá)15%~27%[1]。對(duì)于放療效果欠佳的患者,新輔助放化療可能延誤手術(shù)時(shí)間,增加手術(shù)并發(fā)癥等[2]。雖然有很多分子生物學(xué)指標(biāo)與病理緩解相關(guān),但其可操作性差、費(fèi)用高,很難為臨床所用。
系統(tǒng)炎癥反應(yīng)(systemic inflammatory response, SIR)與多種腫瘤的預(yù)后相關(guān)[3];中性粒細(xì)胞、淋巴細(xì)胞、白蛋白等作為SIR的標(biāo)志物,與腫瘤治療反應(yīng)密切相關(guān)。Walsh等[4]研究發(fā)現(xiàn)中性粒細(xì)胞淋巴細(xì)胞比值(neutrophil-lymphocyte ratio,NLR)升高的患者具有高復(fù)發(fā)風(fēng)險(xiǎn),并能預(yù)測(cè)結(jié)直腸癌患者術(shù)后的生存。此外,中性粒細(xì)胞增高、白蛋白降低能夠作為結(jié)直腸癌肝轉(zhuǎn)移預(yù)后的獨(dú)立預(yù)測(cè)因子[5]。本研究回顧性分析我院114例接受新輔助放化療的直腸癌患者術(shù)前與術(shù)后各項(xiàng)血液指標(biāo),并分析與直腸癌患者病理緩解與預(yù)后的關(guān)系。
1.1 一般資料 選取2010年1月—2014年12月金華市中心醫(yī)院收治的114例經(jīng)病理證實(shí)為直腸癌的患者,腫瘤位置位于距肛緣12cm以下,伴有或不伴有淋巴結(jié)轉(zhuǎn)移,確診時(shí)所有患者均無(wú)遠(yuǎn)處轉(zhuǎn)移。臨床T及N分期依據(jù)體格檢查、結(jié)腸鏡檢查、盆腔磁共振檢查、胸部及腹部CT檢查;術(shù)后隨訪自放化療結(jié)束第1天開(kāi)始,患者中位隨訪時(shí)間36(10~62)個(gè)月,其中有8人失訪。本研究經(jīng)醫(yī)院倫理審查委員會(huì)審核通過(guò)。
1.2 治療 患者術(shù)前均接受總量45Gy/28期長(zhǎng)療程放療,同步口服希羅達(dá)(825 mg/m2/d)。放化療結(jié)束后6~8周行全直腸系膜切除治療,手術(shù)方式包括低位直腸前切除術(shù)、腹會(huì)陰聯(lián)合切除術(shù)及經(jīng)括約肌間切除術(shù),術(shù)中均行預(yù)防性橫結(jié)腸造口或永久造口。術(shù)后標(biāo)本經(jīng)有經(jīng)驗(yàn)的病理醫(yī)生分析,pCR定義為術(shù)后標(biāo)本中無(wú)腫瘤細(xì)胞殘存,標(biāo)記為ypT0N0。術(shù)后患者均在1月內(nèi)接受一線輔助化療,化療方案為XELOX或mFOLFOX6。
1.3 血細(xì)胞計(jì)數(shù) 新輔助放療前及術(shù)前7天抽取患者血液標(biāo)本檢測(cè),記錄中性粒細(xì)胞數(shù)、淋巴細(xì)胞數(shù)、血小板、血漿白蛋白。根據(jù)術(shù)前與術(shù)后相應(yīng)記錄值計(jì)算NLR、血小板淋巴細(xì)胞比值(platelet-lymphocyte ratio,PLR)、中性粒細(xì)胞白蛋白比值(neutrophil-albumin ratio,NAR),計(jì)算相應(yīng)比值變化率:(術(shù)后比值-術(shù)前比值)/術(shù)前比值。PLR分為≥160與<160兩組[5];CEA根據(jù)參考值分為≥5 ug/L組與<5 ug/L組,白蛋白分為≥3.5 g/dL組與<3.5 g/dL組。中性粒細(xì)胞、淋巴細(xì)胞、血小板計(jì)數(shù)、NLR、PLR、NAR及比值變化率根據(jù)中位數(shù)分為計(jì)數(shù)高組與計(jì)數(shù)低組。
2.1 患者臨床病理特征 本研究一共納入患者114例,其中男82例,女32例,年齡61.4±12.2歲;BMI 26.4±6.9 kg/m2;腫瘤距肛緣距離5.4±1.9 cm;放化療結(jié)束至手術(shù)間隔平均8±2周。72例患者接受直腸前切除術(shù),其中24例獲pCR,男18例,女6例;直腸前切除72例,腹會(huì)陰聯(lián)合切除與經(jīng)括約肌間切除42例;非完全患者術(shù)后病理證實(shí)68例為中低分化腺癌。術(shù)后隨訪期間33例(28.9%)患者出現(xiàn)復(fù)發(fā)。
2.2 pCR預(yù)測(cè)因子 單因素分析顯示放療前白蛋白、PLR、NAR、NLR變化率、NAR變化率與pCR顯著相關(guān)(P<0.05);多因素分析進(jìn)一步發(fā)現(xiàn),NLR變化率(P=0.012)與NAR變化率(P=0.028)是pCR的獨(dú)立預(yù)測(cè)因子。見(jiàn)表1。

2.3 生存分析 單因素生存分析發(fā)現(xiàn),放療前CEA、放療前NLR、NLR變化率與NAR變化率與患者無(wú)病生存及總生存率顯著相關(guān),見(jiàn)表2;多因素生存分析發(fā)現(xiàn),PLR變化率是患者預(yù)后的獨(dú)立預(yù)測(cè)因子[1.717(1.285~2.317),P=0.019],見(jiàn)圖1。

腫瘤放化療敏感性不僅依賴于腫瘤本身的生物學(xué)特性,也依賴于腫瘤所處的微環(huán)境[6]。體內(nèi)研究發(fā)現(xiàn)放化療后死亡的腫瘤細(xì)胞能夠遞呈腫瘤相關(guān)抗原給宿主免疫細(xì)胞,激活機(jī)體腫瘤反應(yīng)。淋巴細(xì)胞能夠識(shí)別特異腫瘤抗原并消滅腫瘤細(xì)胞,從而在抗腫瘤免疫中發(fā)揮重要作用,并與腫瘤對(duì)放化療敏感性相關(guān)[6]。中性粒細(xì)胞、血小板能夠分泌生長(zhǎng)因子,形成促腫瘤生長(zhǎng)的微環(huán)境,促進(jìn)腫瘤進(jìn)展。Kitayama研究發(fā)現(xiàn)淋巴細(xì)胞數(shù)與患者完全緩解率顯著相關(guān)[6]。一項(xiàng)包含179例患者的研究發(fā)現(xiàn),外周血淋巴細(xì)胞數(shù)是接受新輔助放化療直腸癌患者pCR的獨(dú)立預(yù)測(cè)因子[7]。進(jìn)一步研究發(fā)現(xiàn)血紅蛋白、白蛋白及淋巴細(xì)胞與pCR呈正相關(guān),而中心粒細(xì)胞、C反應(yīng)蛋白呈負(fù)相關(guān)[8]。Trwfik等研究發(fā)現(xiàn)術(shù)前與術(shù)后NAR與pCR顯著相關(guān)[2],但在我們的研究中,放療前后淋巴細(xì)胞比率降低,但未見(jiàn)與pCR明顯相關(guān)。雖然放化療前NAR與PLR與pCR顯著相關(guān),但是放化療后NLR、PLR、NAR與pCR無(wú)明顯相關(guān)性。在機(jī)體內(nèi),促進(jìn)腫瘤與抑制腫瘤是相互制約的,兩者的比例或更能反應(yīng)機(jī)體對(duì)腫瘤的效應(yīng)。因此,我們進(jìn)一步研究發(fā)現(xiàn)新輔助放化療前后NLR、NAR變化率是患者pCR的獨(dú)立預(yù)測(cè)因子。因此動(dòng)態(tài)評(píng)估患者指標(biāo)變化,或能更準(zhǔn)確預(yù)測(cè)患者對(duì)放化療反應(yīng)。
SIR在機(jī)體對(duì)多種腫瘤的應(yīng)答中起重要作用,而作為SIR的標(biāo)志物,血細(xì)胞計(jì)數(shù)及白蛋白或許能夠預(yù)測(cè)惡性腫瘤患者對(duì)治療的應(yīng)答及預(yù)后。Shen等研究發(fā)現(xiàn)NLR≥2.8局部進(jìn)展期直腸癌患者總生存率與無(wú)病生存率較<2.8患者差[9]。Carruthers等研究發(fā)現(xiàn)NLR是新輔助放化療直腸癌患者預(yù)后的獨(dú)立預(yù)測(cè)因子[2]。在我們的研究中,單因素分析發(fā)現(xiàn)術(shù)前NLR、NLR變化率、NAR變化率與患者生存相關(guān)。進(jìn)一步多因素研究發(fā)現(xiàn)NLR變化率而非NLR是患者獨(dú)立預(yù)測(cè)因子。
綜上所述,直腸癌患者新輔助放化療前后NLR變化率pCR與預(yù)后的獨(dú)立預(yù)測(cè)因子。由于NLR較易獲取,或能在新輔助放化療患者選擇及評(píng)估療效中發(fā)揮重要作用。但是我們的研究是回顧性分析,同時(shí)分析范圍較窄,沒(méi)有將其他分子標(biāo)志物等納入分析,研究具有一定局限性,未來(lái)需更大樣本及范圍的研究進(jìn)一步證實(shí)。

‘Systemic Infla mmatory Response’ and the prognosis for patients of advanced rectal cancer undergoing preoperative chemoradiotherapy: a correlative study

Objective To explore the correlation of systemic infla mmatory response (SIR) to pathological complete response (pCR) and the prognosis of patients of advanced rectal cancer. Method Firstly, the plasma albumin, neutrophils, lymphocytes and neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio ratio (PLR), neutrophil-albumin ratio(NAR) of 114 rectal cancer patients as observed before and after neoadjuvant chemoradiotherapy were recorded and the ratio changes were calculated. Secondly, statistical methods were used to analyze the relationship between the above indexes and the prognosis for the patients. Findings The change rate of NLR (P=0.012) and NAR (P=0.028) were an independent predictor of pCR patients in univariate and multivariate analysis. The univariate and multivariate survival analysis showed that the change rate of NLR patients were independent predictor of 3 years overall survival [1.491 (1.259-2.857),P=0.015] and 3 years disease-free survival [1.717(1.285-2.317),P=0.019]. Conclusion The change rate of NLR rate was an independent predictor of pCR and survival in patients with rectal cancer undergoing neoadjuvant chemoradiotherapy. It can serve as an evaluation tool to determine therapeutic response and treatment seletions.
