      大連石城島原駐老年居民PON1rs662基因多態(tài)性與高血壓及其相關(guān)因素的關(guān)系研究

      [摘要] 目的 探討大連石城島原駐老年居民高血壓的患病率及相關(guān)因素,血清對氧磷酶1(rs662)基因(PON1rs662)與高血壓的關(guān)系。 方法 選取2016年6—8月石城島654名65歲以上原駐居民對其進行高血壓及相關(guān)因素流行病學(xué)調(diào)查,分別采用調(diào)整的世界衛(wèi)生組織(WHO)-MONICA研究問卷進行問卷調(diào)查,進行生化及心臟超聲相關(guān)檢查;采用離心柱法提取脫氧核糖核酸(DNA),實時定量熒光聚合酶鏈反應(yīng)(PCR)技術(shù)(TaqMan-PCR)對PON1rs662進行基因分型,確定所有研究對象PON1rs662基因型分布是否符合哈溫伯格平衡定律,探討PON1rs662基因與高血壓的關(guān)系。 結(jié)果 調(diào)查對象總?cè)巳焊哐獕夯疾÷蕿?3.1%,女性患病率為77.5%,男性患病率為68.3%;高血壓與非高血壓人群心率、體重指數(shù)(BMI)、腰圍、尿酸(UA)、三酰甘油(TG)、左房前后徑(LA)比較,差異有統(tǒng)計學(xué)意義(P < 0.05);研究對象PON1rs662基因型分布符合哈溫伯格平衡(P > 0.05);等位基因模型和相加模型檢測研究對象PON1rs662 SNP位點與高血壓無相關(guān)性(P > 0.05)。 結(jié)論 大連石城島原駐老年居民高血壓患病率為73.1%;心率、BMI、腰圍、UA、TG與島上高血壓患病率有關(guān);PON1rs662基因與研究對象高血壓患病率可能無相關(guān)性。

      Study on the relationship between PON1rs662 gene polymorphism and hypertension and its related factors in Shicheng Island of Dalian

      [Abstract] Objective To investigate the prevalence and related factors of hypertension in the elderly in Shicheng Island of Dalian, and the relationship between serum P-oxophosphase 1(rs662) gene(PON1rs662) and hypertension. Methods The epidemiological survey on hypertension and related factors was conducted among 654 residents aged more than 65 from June to August 2016. The adjusted WHO-MONICA research questionnaire was used for questionnaire survey, biochemical and echocardiographic examinations were carried out. DNA was extracted by centrifugation column, and gene typing of PON1rs662 was carried out by TaqMan PCR. All the studies were determined whether the genotype distribution of PONIrs662 accord with Hardy-Weinberg Law, and the relationship between PON1rs662 gene and hypertension was investigated. Results The prevalence of hypertension was 73.1%, 77.5% in women and 68.3% in men. There were statistically significant differences in heart rate, body mass index (BMI), waist circumference, uric acid (UA), triglyceride (TG) and left atrial anteropostery (LA) between hypertensive and non-hypertensive subjects (P < 0.05). The genotype distribution of PON1rs662 was in line with Hardy-Weinberg Law (P > 0.05). Both the allele model and the additive moded showed no correlation between the PON1rs662 SNP and hypertension (P > 0.05). Conclusion The prevalence rate of hypertension in the old residents of Shicheng Island was 73.1%. Heart rate, BMI, waist circumference, UA and TG were associated with the prevalence of hypertension on the island. PON1rs662 gene may not be correlated with the prevalence of hypertension in these study objects.

      3.2 PON1rs662SNP分布情況及其與高血壓的關(guān)系

      PON基因多態(tài)性在不同民族、不同地區(qū)、不同國家背景下與多種代謝性疾病相關(guān),有重要的研究意義,本研究選取人類基因組中PON1rs662位點在該島上進行深入研究,PON1rs662位于人類7號染色體的長臂上[11],PON1rs662基因的多態(tài)性影響PON1的酶活性[17],PON1主要在肝細胞合成和分泌,是一種鈣離子依賴血清脂酶。在人類血清中PON1與HDL-C結(jié)合保護LDL-C氧化修飾,減少LDL-C的氧化作用而保護血管[18]。PON1活性下降可加速動脈粥樣硬化,增加高血壓、糖尿病和冠心病等多種心血管疾病患病率[19]。由于SNP與疾病的關(guān)系在個體或人群之間關(guān)聯(lián)不一致,而且疾病的發(fā)生是遺傳和理化因素共同作用的結(jié)果,因此本研究選擇北方海島石城島,該島上出生并連續(xù)居住40年以上的原駐老年居民,從遺傳學(xué)角度屬于孤立人群,更有利于探討PON1rs662與高血壓及其相關(guān)危險因素的關(guān)系,本研究該島上PON1rs662位點SNP分布情況,通過Allelic model和Additive model兩個模型來分析PON1rs662與該島高血壓的關(guān)系,結(jié)果顯示該島上參與調(diào)查的老年人群中PON1rs662基因單核苷酸多態(tài)性與高血壓差異無統(tǒng)計學(xué)意義(P > 0.05),但這一結(jié)果并不能說明PON1rs662基因與該島高血壓無相關(guān)性,從人類遺傳學(xué)角度來說,有可能PON1rs662等位基因和致病基因相距很近,存在連鎖不平衡,也有可能因樣本量不足,需要更多大樣本的數(shù)據(jù)支持。

      探討高血壓發(fā)病機制,有效規(guī)避遺傳風(fēng)險,做到高血壓精準診斷與治療有著十分重要的意義,該研究有可能為未來高血壓的個體化、精準預(yù)防與治療提供相關(guān)研究數(shù)據(jù),在精準醫(yī)學(xué)、精準健康時代的趨勢下,做一些建設(shè)性的探索。

      3.3 結(jié)論

      大連石城島65歲以上老年原駐居民高血壓患病率為73.1%;該島上高血壓患病率可能與心率、BMI、腰圍、UA及TG有關(guān),或許是該島上高血壓相關(guān)危險因素;PON1rs662基因與研究對象高血壓患病率可能無關(guān)。

      3.4 研究不足

      島上原駐老年居民數(shù)量偏少,可能產(chǎn)生統(tǒng)計學(xué)偏倚,PON1rs662基因與高血壓是真的不相關(guān)還是樣本量不夠,有待于更多的研究和證實。
