摘要:目的 ?分析IL-6基因多態(tài)性與心房顫動(dòng)發(fā)病風(fēng)險(xiǎn)的相關(guān)性研究。方法 ?計(jì)算機(jī)檢索萬(wàn)方、知網(wǎng)、維普、PubMed等數(shù)據(jù)庫(kù),檢索關(guān)于IL-6基因多態(tài)性與心房顫動(dòng)的病例對(duì)照研究,檢索時(shí)間區(qū)間均從建庫(kù)至2019年12月。對(duì)于納入的研究,應(yīng)用Rev Man5.3軟件進(jìn)行分析。結(jié)果 ?共納入7項(xiàng)研究,Meta分析結(jié)果顯示,IL-6-572C/G、IL-6-634C/G多態(tài)性與心房顫動(dòng)明顯相關(guān)(P<0.01),其中IL-6-572C/G基因多態(tài)性中,攜帶GG基因型的個(gè)體患者心房顫動(dòng)的風(fēng)險(xiǎn)是攜帶CC個(gè)體的3.39倍[95%CI(2.18,5.29),P<0.00001],攜帶GC基因型的個(gè)體患者心房顫動(dòng)的風(fēng)險(xiǎn)是攜帶CC個(gè)體的1.58倍[95%CI(1.30,1.92),P<0.00001];IL-6-634C/G基因多態(tài)性中,攜帶GG基因型的個(gè)體患者心房顫動(dòng)的風(fēng)險(xiǎn)是攜帶CC個(gè)體的5.80倍[95%CI(1.15,29.39),P=0.03]。而IL-6-174G/C、IL-6-597G/A多態(tài)性與心房顫動(dòng)無(wú)關(guān)。結(jié)論 ?IL-6-572C/G基因型GG、GC和IL-6-634C/G基因型GG可能是心房顫動(dòng)發(fā)生的風(fēng)險(xiǎn)基因型。

Abstract:Objective ?To study the correlation between IL-6 gene polymorphism and atrial fibrillation risk.Methods ?The computer searched Wanfang, HowNet, Weipu, PubMed and other databases to search for case-control studies on IL-6 gene polymorphism and atrial fibrillation. The search time interval was from the establishment of the database to December 2019. For the included studies, Rev Man5.3 software was used for analysis.Results ?A total of 7 studies were included. The results of Meta analysis showed that IL-6-572C/G and IL-6-634C/G polymorphisms were significantly related to atrial fibrillation (P<0.01), among which IL-6-572C/G genes were the morphology, the risk of atrial fibrillation in individuals with GG genotype is 3.39 times that of individuals with CC [95%CI(2.18, 5.29),P<0.00001]. The risk of atrial fibrillation in individuals with GC genotype is that of carrying 1.58 times that of individuals with CC [95%CI(1.30, 1.92),P<0.00001]; among IL-6-634C/G gene polymorphisms, the risk of atrial fibrillation in individuals with GG genotype is 5.80 compared with individuals with CC Times [95%CI(1.15, 29.39),P=0.03]. The IL-6-174G/C and IL-6-597G/A polymorphisms have nothing to do with atrial fibrillation.Conclusion ?IL-6-572C/G genotype GG, GC and IL-6-634C/G genotype GG may be risk genotypes for atrial fibrillation.

心房顫動(dòng)(atrial fibrillation,AF)是人群中最常見(jiàn)的持續(xù)性心律失常,估計(jì)影響到全世界超過(guò)3000萬(wàn)人,但其發(fā)病機(jī)制仍不明確,近年來(lái),炎癥激活在AF發(fā)病機(jī)制中的關(guān)鍵作用日益被人們所重視[1]。白細(xì)胞介素-6(IL-6)是一種兼具促炎和抗炎作用的細(xì)胞因子。IL-6是一種多功能細(xì)胞因子,參與調(diào)節(jié)多個(gè)急性時(shí)相反應(yīng)、造血、細(xì)胞再生和免疫[2]。已知IL-6水平隨年齡增加而增加,而且IL-6與心房重構(gòu)有關(guān)[3]。目前已有研究表明,IL-6在心房顫動(dòng)患者血清中水平上升[4]、并可預(yù)測(cè)射頻消融術(shù)后房顫復(fù)發(fā)[5],也有研究表明IL-6基因多態(tài)性與心房顫動(dòng)發(fā)生風(fēng)險(xiǎn)相關(guān),但尚存在爭(zhēng)議。本研究通過(guò)對(duì)IL-6基因多態(tài)性與心房顫動(dòng)相關(guān)性Meta分析,旨在為臨床診療提供理論依據(jù)。
1資料與方法
1.1檢索策略 ?計(jì)算機(jī)檢索PubMed、萬(wàn)方、維普、知網(wǎng)、中國(guó)生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(kù);中文以“心房顫動(dòng)”“房顫”“房型心律失常”和“白介素-6”“IL-6”為主題詞;英文以“IL-6”“Interleukin-6”and“AF”“atrial fibrillation”“auricular fibrillation”為主題詞,并手工檢索可能的相關(guān)研究。
1.2納入與排除標(biāo)準(zhǔn) ?納入標(biāo)準(zhǔn):①病例對(duì)照研究;②基于中國(guó)人群的心房顫動(dòng)診斷明確的患者,心房顫動(dòng)類(lèi)型不限;③均分析了IL-6基因多態(tài)性與心房顫動(dòng)的關(guān)系;④有相應(yīng)等位基因分布的具體數(shù)據(jù)。排除標(biāo)準(zhǔn) ①未設(shè)立健康對(duì)照組;②重復(fù)文獻(xiàn)、未獲得全文、綜述、meta分析;③在對(duì)照組各基因型分布符合Hardy-Weinberg檢驗(yàn)。
1.3文獻(xiàn)篩選及數(shù)據(jù)提取 ?文獻(xiàn)篩選及數(shù)據(jù)提取由2名研究人員共同進(jìn)行,并交叉核對(duì),最后由第3名研究者校對(duì)。數(shù)據(jù)提取主要包括納入研究作者的姓名及年份、樣本量、各基因型的分布頻率等。
1.4統(tǒng)計(jì)學(xué)方法 ?使用Review Manager 5.3軟件對(duì)提取的數(shù)據(jù)進(jìn)行分析。各基因型在病例組及對(duì)照組分布頻率用OR值表示,區(qū)間估計(jì)均采用95%CI。研究間的異質(zhì)性用I2值表示,當(dāng)各研究間異質(zhì)性較小時(shí)(I2≤50%)采用固定效應(yīng)模型,當(dāng)各研究間異質(zhì)性明顯時(shí)(I2>50%)首先分析異質(zhì)性原因,若無(wú)臨床異質(zhì)性采用隨機(jī)效應(yīng)模型,并進(jìn)行敏感性分析。由于納入研究小于10篇所以本研究未進(jìn)行發(fā)表偏倚檢測(cè)。P<0.05表示差異有統(tǒng)計(jì)學(xué)意義。
2結(jié)果
2.1文獻(xiàn)檢索結(jié)果及質(zhì)量評(píng)分 ?使用制定的檢索策略共檢索到文獻(xiàn)1293篇通過(guò)去除重復(fù)文獻(xiàn)、閱讀標(biāo)題和摘要、通讀全文后最終納入7篇文獻(xiàn),見(jiàn)圖1。納入文獻(xiàn)的基線(xiàn)特征見(jiàn)表1。其中分析IL-6-174G/C多態(tài)性的有3篇;分析IL-6-572C/G基因多態(tài)性的有5篇;分析IL-6-597G/A、IL-6-634C/G基因多態(tài)性的各1篇。兩名研究者據(jù)NOS評(píng)分量表對(duì)納入文獻(xiàn)進(jìn)行評(píng)分,所有文獻(xiàn)評(píng)分均在3~6分。
2.2 Meta分析結(jié)果
2.2.1 IL-6-174G/C基因多態(tài)性與心房顫動(dòng)相關(guān)性分析 ?GG基因型分別與CC、GC、CC+GC基因型進(jìn)行比較,由于各研究間無(wú)明顯異質(zhì)性(I2=0)故采用固定效應(yīng)模型。由于GG基因型突變率較低,進(jìn)行調(diào)整后Meta分析結(jié)果無(wú)明顯統(tǒng)計(jì)學(xué)差異(P>0.05),見(jiàn)表2。
2.2.2 IL-6-572C/G基因多態(tài)性與心房顫動(dòng)相關(guān)性分析 ?CC基因型分別與GG、GC、GG+GC基因型進(jìn)行比較,由于各研究間無(wú)明顯異質(zhì)性(I2=0)故采用固定效應(yīng)模型。Meta分析結(jié)果示:攜帶GG基因型的個(gè)體患心房顫動(dòng)的風(fēng)險(xiǎn)是攜帶CC個(gè)體的3.39倍[95%CI(2.18,5.29),P<0.00001],攜帶GC基因型的個(gè)體患心房顫動(dòng)的風(fēng)險(xiǎn)是攜帶CC個(gè)體的1.58倍[95%CI(1.30,1.92),P<0.00001],攜帶GC+GG基因型的個(gè)體患心房顫動(dòng)的風(fēng)險(xiǎn)是攜帶CC基因型個(gè)體的1.78倍,結(jié)果見(jiàn)圖2~圖4。
2.2.3 597G/A、634C/G基因多態(tài)性與心房顫動(dòng)相關(guān)性分析 ?納入研究中只有1項(xiàng)研究報(bào)道了597G/A基因多態(tài)性,未發(fā)現(xiàn)G→C等位基因突變,故未行Meta分析。有1項(xiàng)研究報(bào)道634C/G基因多態(tài)性與心房顫動(dòng)的關(guān)系,結(jié)果顯示,攜帶GG基因型的個(gè)體患者心房顫動(dòng)的風(fēng)險(xiǎn)是攜帶CC個(gè)體的5.80倍,而攜帶GC基因型的個(gè)體與攜帶CC基因型的個(gè)體發(fā)生心房顫動(dòng)的風(fēng)險(xiǎn)性無(wú)明顯統(tǒng)計(jì)學(xué)差異(P=0.10)。
3討論
心房顫動(dòng)是一種發(fā)病率高、死亡率高的持續(xù)性心律失常。心房顫動(dòng)的總發(fā)病率可達(dá)5.5%,且隨年齡增加,發(fā)病率逐漸上升[6]。大量研究表明[7,8,9],CRP、IL、TNF與心房顫動(dòng)發(fā)生時(shí)炎癥水平相關(guān),其中IL-6在心房顫動(dòng)的發(fā)生發(fā)展及預(yù)后中扮演著重要角色[10,11]。人IL-6基因定位于第7p21染色體上[12,13],由4個(gè)內(nèi)含子和5個(gè)外顯子組成,形成一個(gè)由232個(gè)氨基酸組成的前體蛋白。IL-6是一種多功能性性細(xì)胞因子,可能是心房顫動(dòng)炎癥反應(yīng)的關(guān)鍵介質(zhì)。目前已在IL-6啟動(dòng)子區(qū)發(fā)現(xiàn)-174 G/C(Rs 1800795)和-572C/G(Rs 1800796)、-597G/A、-634C/G等多種功能啟動(dòng)子多態(tài)性,這幾種基因多態(tài)性可能與IL-6水平的升高有關(guān)。導(dǎo)致心房顫動(dòng)患者炎癥因子釋放增加的機(jī)制尚不清楚。目前只有少量研究表明IL-6(-174 G/C和-572 C/G、-597G/A、-634C/G)基因多態(tài)性與心房顫動(dòng)相關(guān),且無(wú)統(tǒng)一共識(shí)[14]。
IL-6-572C/G基因多態(tài)性與心房顫動(dòng)相關(guān)性的研究最多。本研究顯示,攜帶GG、GC基因型的個(gè)體更易發(fā)生心房顫動(dòng),這與既往研究結(jié)果一致。也有研究分析IL-6-174 G/C與心房顫動(dòng)相關(guān)性,但由于在中國(guó)人群中等位基因突變率較低,尚未發(fā)現(xiàn)二者相關(guān)性。鑒于本研究納入的人口均為中國(guó)人群,由于IL-6-174 G/C突變率較低,本研究通過(guò)對(duì)納入研究的數(shù)據(jù)調(diào)整后,也同樣發(fā)現(xiàn)IL-6-174 G/C與心房顫動(dòng)無(wú)明顯相關(guān)性。此外關(guān)于IL-6-597G/A、-634C/G由于目前研究較少,本研究各納入1篇文獻(xiàn)。Meta分析結(jié)果顯示,-597G/A在中國(guó)人群中未發(fā)現(xiàn)突變,而-634C/G基因中攜帶GG基因型的個(gè)體患者心房顫動(dòng)的風(fēng)險(xiǎn)是攜帶CC個(gè)體的5.80倍,雖然本研究結(jié)果表明攜帶CC基因型的個(gè)體患房顫的機(jī)率較高,但由于納入研究較少,結(jié)論不具有外推性,還需要進(jìn)一步數(shù)據(jù)支持。本研究局限性:關(guān)于IL-6基因多態(tài)性與心房顫動(dòng)相關(guān)性的研究仍然較少,而且缺乏大規(guī)模臨床對(duì)照實(shí)驗(yàn),因此納入研究較少、樣本量較少;由于樣本量較小一些潛在混雜因素?zé)o法得到有效的控制,無(wú)法通過(guò)亞組分析剔除混雜因素。
綜上所述,IL-6-572C/G、-634C/G基因多態(tài)性可能與我國(guó)人民心房顫動(dòng)風(fēng)險(xiǎn)性相關(guān);IL-6-174 G/C、-597G/A基因由于在中國(guó)人群中突變率較低,可能與心房顫動(dòng)風(fēng)險(xiǎn)性無(wú)關(guān)。鑒于樣本量較小,研究結(jié)論需進(jìn)一步臨床實(shí)驗(yàn)驗(yàn)證。
