      神經(jīng)內(nèi)科住院醫(yī)師腦電圖教學(xué)與培訓(xùn)現(xiàn)狀的分析

      [摘要]目的分析國(guó)內(nèi)外神經(jīng)內(nèi)科住院醫(yī)師腦電圖教學(xué)與培訓(xùn)的現(xiàn)狀。方法檢索中國(guó)知網(wǎng)和Pubmed數(shù)據(jù)庫(kù)中神經(jīng)內(nèi)科住院醫(yī)師腦電圖教學(xué)與培訓(xùn)的文獻(xiàn),至2021年8月16日。同時(shí)檢索中國(guó)和美國(guó)官方網(wǎng)站,查找神經(jīng)內(nèi)科住院醫(yī)師培訓(xùn)中腦電圖的內(nèi)容。結(jié)果最終納入20篇文獻(xiàn),分析國(guó)內(nèi)外腦電圖教學(xué)與培訓(xùn)的要求、目標(biāo)、評(píng)估和效果;新冠疫情下的腦電圖培訓(xùn)。參考中國(guó)住院醫(yī)師規(guī)范化培訓(xùn)內(nèi)容與標(biāo)準(zhǔn)以及美國(guó)醫(yī)學(xué)畢業(yè)生教育評(píng)審委員會(huì)神經(jīng)內(nèi)科住院醫(yī)師培訓(xùn)里程碑計(jì)劃,比較兩國(guó)腦電圖培訓(xùn)之異同。結(jié)論隨著技術(shù)的進(jìn)步,腦電圖教學(xué)模式的變革取得較好效果。但即使在美國(guó),住院醫(yī)師腦電圖教學(xué)與培訓(xùn)也仍然存在問(wèn)題,有待改進(jìn)。

      Analysis of the current situation of electroencephalogram teaching and training for neurology residents

      [Abstract] Objective To analyze the current situation of electroencephalogram (EEG) teaching and training for domestic and foreign neurology residents. Methods The pieces of literature on EEG teaching and training of neurology residents in CNKI and PubMed database were searched, with the date of search results up to August 16, 2021. Moreover, the official websites of China and the United States were searched to obtain the contents of EEG training for neurology residents. Results A total of 20 pieces of literature were included to analyze the requirements, objectives, evaluation and effects of domestic and foreign EEG teaching and training. EEG training was studied under the COVID-19. The contents and standards of standardized resident training in China and the milestone plan of neurology resident training of the American Medical Graduate Education Review Committee were referred and the similarities and differences of EEG training between the two countries were compared. Conclusion With the progress of technology, the reform of EEG teaching mode has achieved good results . However, even in the United States, there are still some problems in EEG teaching and training for residents, which need to be improved.

      中國(guó)于2014年啟動(dòng)住院醫(yī)師規(guī)范化培訓(xùn)制度建設(shè)工作,實(shí)現(xiàn)與國(guó)際醫(yī)學(xué)教育模式接軌[1]。中國(guó)醫(yī)師協(xié)會(huì)于2015年發(fā)布住院醫(yī)師規(guī)范化培訓(xùn)內(nèi)容與標(biāo)準(zhǔn)(試行)[2]。因?yàn)樯窠?jīng)內(nèi)科的疾病具有臨床表現(xiàn)多樣、病情復(fù)雜和預(yù)后不佳等特點(diǎn),輔助檢查的專(zhuān)業(yè)性強(qiáng),包括腦電圖;所以神經(jīng)內(nèi)科住院醫(yī)師培訓(xùn)對(duì)于神經(jīng)電生理輪轉(zhuǎn)時(shí)間,學(xué)習(xí)目的、基本要求都有相應(yīng)的規(guī)定[2]。腦電圖教學(xué)與培訓(xùn)在實(shí)踐中存在不足之處。研究顯示經(jīng)過(guò)正規(guī)腦電圖培訓(xùn),神經(jīng)內(nèi)科住院醫(yī)師對(duì)于腦電圖閱讀的正確率仍不高,近半數(shù)住院醫(yī)師即使在指導(dǎo)下也無(wú)法正確閱讀腦電圖[3]。調(diào)查還發(fā)現(xiàn)住院醫(yī)師培訓(xùn)中存在腦電圖接觸少、無(wú)效教學(xué)和住院醫(yī)師缺乏興趣等問(wèn)題[4]。

      本研究檢索國(guó)內(nèi)外相關(guān)文獻(xiàn),分析神經(jīng)內(nèi)科住院醫(yī)師腦電圖教學(xué)與培訓(xùn)的現(xiàn)狀,為今后教學(xué)改革提供借鑒。

      1方法

      1.1檢索策略

      檢索日期至2021年8月16日。檢索方法:①Pubmed數(shù)據(jù)庫(kù)檢索:檢索公式為“((((Electroencephalography? OR electroencephalogram OREEGORelectroencephalograms) OR (Electroencephalography[MeSH Terms])) A N D ((((Education[MeSH Subheading]) OR (teaching [ M e S H Terms])) OR (Learning[MeSH? Terms]))OR (Education? OR teaching OR learning OR training))) AND (resident)) AND ((neurology) OR (neurology[MeSH Terms]))”;②中國(guó)知網(wǎng)數(shù)據(jù)庫(kù)檢索:檢索公式為(全文:神經(jīng)內(nèi)科)AND(全文:住院醫(yī)師)AND(全文:腦電圖)AND(全文:培訓(xùn))。以上檢索語(yǔ)言分別為英語(yǔ)和中文。③互聯(lián)網(wǎng)檢索中國(guó)和美國(guó)官方網(wǎng)站,查找神經(jīng)內(nèi)科住院醫(yī)師培訓(xùn)中腦電圖的內(nèi)容。

      1.2納入及排除標(biāo)準(zhǔn)

      1.2.1納入標(biāo)準(zhǔn)文獻(xiàn)中需同時(shí)包含以下內(nèi)容:①神經(jīng)內(nèi)科(neurology);②住院醫(yī)師(resident);③腦電圖(electroencephalography);④教學(xué)或培訓(xùn)(education or training)。

      1.2.2排除標(biāo)準(zhǔn)包含以下任何一條:①專(zhuān)科醫(yī)師培訓(xùn);②非英文或中文文獻(xiàn)。通過(guò)納入文獻(xiàn)的全文或參考文獻(xiàn)追溯符合標(biāo)準(zhǔn)的其他文獻(xiàn)。兩名研究者獨(dú)立檢索、篩選和納入文獻(xiàn),如有分歧則由一名高級(jí)研究者進(jìn)行裁定。

      2結(jié)果

      Pubmed數(shù)據(jù)庫(kù)檢索結(jié)果為70篇文獻(xiàn),標(biāo)題和摘要篩查后24篇符合,全文篩選最后納入12篇[3-14];另外追溯到4篇文獻(xiàn)[15-18]。中國(guó)知網(wǎng)數(shù)據(jù)庫(kù)檢索結(jié)果為358篇,期刊論文146篇,學(xué)位論文169篇,會(huì)議23篇。標(biāo)題和摘要篩查后3篇期刊論文符合納入和排除標(biāo)準(zhǔn)[19-21]。另追溯到1篇文獻(xiàn)并納入[22]。共納入20篇文獻(xiàn)。時(shí)間跨度為2012年至2021年,其中來(lái)自美國(guó)的文獻(xiàn)13篇,中國(guó)4篇,加拿大2篇,土耳其1篇。調(diào)查研究的對(duì)象以神經(jīng)內(nèi)科住院醫(yī)師為主,兼顧培訓(xùn)項(xiàng)目主管和指導(dǎo)老師。報(bào)道的主題包括對(duì)神經(jīng)內(nèi)科住院醫(yī)師腦電圖教學(xué)與培訓(xùn)現(xiàn)狀的問(wèn)卷調(diào)查,互聯(lián)網(wǎng)時(shí)代的腦電圖培訓(xùn)課程設(shè)置,新冠疫情下腦電圖培訓(xùn)的應(yīng)對(duì)等,見(jiàn)表1。

      其次于中國(guó)和美國(guó)官方文件中,提取神經(jīng)內(nèi)科住院醫(yī)師培訓(xùn)中腦電圖的內(nèi)容,中國(guó)為住院醫(yī)師規(guī)范化培訓(xùn)內(nèi)容與標(biāo)準(zhǔn)(試行)[2];美國(guó)為醫(yī)學(xué)畢業(yè)生教育評(píng)審委員會(huì)神經(jīng)內(nèi)科住院醫(yī)師培訓(xùn)里程碑計(jì)劃[23-24];中美神經(jīng)內(nèi)科住院醫(yī)師培訓(xùn)中腦電圖內(nèi)容比較見(jiàn)表2。

      3討論

      本研究通過(guò)對(duì)Pubmed和中國(guó)知網(wǎng)的系統(tǒng)檢索,多數(shù)為來(lái)自美國(guó)和中國(guó)的研究,結(jié)合中美兩國(guó)官方指導(dǎo)文件,比較全面地闡述了神經(jīng)內(nèi)科住院醫(yī)師腦電圖教學(xué)與培訓(xùn)的現(xiàn)狀。國(guó)內(nèi)外腦電圖教學(xué)與培訓(xùn)均卓有成效。盡管如此,即使在美國(guó),住院醫(yī)師腦電圖教學(xué)與培訓(xùn)也仍然存在問(wèn)題,均有待改進(jìn)。

      腦電圖培訓(xùn)的時(shí)間是最基本的要素。自1999—2021年,美國(guó)住院醫(yī)師培訓(xùn)期間用于腦電圖的時(shí)間保持不變,平均約為2個(gè)月(0~4月)[4,25-26]。美國(guó)住院醫(yī)師培訓(xùn)需要4年完成,調(diào)查發(fā)現(xiàn)要在培訓(xùn)第3~ 4年才能達(dá)到美國(guó) ACGME 神經(jīng)內(nèi)科住院醫(yī)師培訓(xùn)里程碑計(jì)劃設(shè)定的腦電圖教學(xué)目標(biāo)[18]。在此基礎(chǔ)上,即使是短期腦電圖強(qiáng)化訓(xùn)練也是有必要的[6]。只有經(jīng)過(guò)持續(xù)培訓(xùn)才有可能讓神經(jīng)內(nèi)科住院醫(yī)師勝任重癥監(jiān)護(hù)室等特殊環(huán)境中的腦電圖閱讀工作[14]。中國(guó)神經(jīng)內(nèi)科住院醫(yī)師培訓(xùn)細(xì)則規(guī)定神經(jīng)電生理室輪轉(zhuǎn)2個(gè)月,但是培訓(xùn)項(xiàng)目不僅包括腦電圖,還涵蓋肌電圖和誘發(fā)電位[2]。

      腦電圖培訓(xùn)目標(biāo)有必要設(shè)定得更為明確。中國(guó)的腦電圖培訓(xùn)目標(biāo)要求熟悉檢查結(jié)果的臨床意義[2],而臨床工作中住院醫(yī)師承擔(dān)的角色是獨(dú)立或在上級(jí)醫(yī)師指導(dǎo)下正確解讀腦電圖。美國(guó) ACGME 神經(jīng)內(nèi)科住院醫(yī)師培訓(xùn)里程碑計(jì)劃要求住院醫(yī)師在畢業(yè)時(shí)應(yīng)該能夠“解釋常見(jiàn)的腦電圖異常,識(shí)別正常的腦電圖變異,并創(chuàng)建報(bào)告”[23]。其中“常見(jiàn)腦電圖異?!睙o(wú)明確所指,且缺乏關(guān)于評(píng)估腦電圖報(bào)告準(zhǔn)確性和質(zhì)量的任何指導(dǎo)。最新一項(xiàng)對(duì)美國(guó)住院醫(yī)師項(xiàng)目主管的調(diào)查發(fā)現(xiàn),腦電圖教學(xué)效果的評(píng)估缺乏客觀和統(tǒng)一的標(biāo)準(zhǔn)[4]。因此培訓(xùn)目標(biāo)的明確有助于考核的實(shí)施。

      腦電圖培訓(xùn)效果在報(bào)道中并不一致。新的培訓(xùn)模式和方法提高了腦電圖培訓(xùn)的質(zhì)量[5,7-8,12-13,19,21]。盡管如此,在美國(guó),近50%住院醫(yī)師不能正確解讀腦電圖[3,10],約25%的住院醫(yī)師畢業(yè)時(shí)未達(dá)到腦電圖培訓(xùn)的目標(biāo)[4,9],住院醫(yī)師認(rèn)為腦電圖培訓(xùn)不充分[17]。這些問(wèn)題的解決需要經(jīng)過(guò)不斷的探索和調(diào)整,優(yōu)化腦電圖教學(xué)內(nèi)容和教學(xué)模式[20]。腦電圖教學(xué)和培訓(xùn)障礙的具體解決方案包括:增加對(duì)腦電圖的接觸,延長(zhǎng)輪轉(zhuǎn)時(shí)間和次數(shù),而且是專(zhuān)門(mén)在腦電圖室的輪轉(zhuǎn),增加癲癇單元的輪轉(zhuǎn),及時(shí)有效地評(píng)估教學(xué)效果[4]。

      總之,腦電圖教學(xué)模式的變革取得了較好的效果,但住院醫(yī)師腦電圖教學(xué)與培訓(xùn)仍然存在問(wèn)題,本研究可供住院醫(yī)師、教師和管理部門(mén)借鑒,未來(lái)優(yōu)化實(shí)施,進(jìn)一步提高腦電圖教學(xué)與培訓(xùn)的質(zhì)量。
