      卵泡液中細(xì)胞因子與卵泡發(fā)育及妊娠結(jié)局的相關(guān)性

      【摘要】隨著體外受精-胚胎移植(in vitro fertilization-embryo transfer,IVF-ET)技術(shù)水平的日益提高,卵巢微環(huán)境中調(diào)節(jié)卵母細(xì)胞發(fā)育的細(xì)胞因子的作用受到廣泛關(guān)注。細(xì)胞因子作為一類具有廣泛生物學(xué)活性的激素樣多肽調(diào)控生殖過程,它們通過調(diào)節(jié)卵巢內(nèi)分泌或旁分泌,進而影響卵泡成熟、胚胎發(fā)育及胚泡著床等環(huán)節(jié)來參與生殖調(diào)節(jié)。研究表明不孕可能與卵泡液中某些細(xì)胞因子(如血管內(nèi)皮生長因子、粒細(xì)胞集落刺激因子、活性氧簇、胰島素樣生長因子、白細(xì)胞介素、白血病抑制因子等)的異常表達(dá)引發(fā)的卵泡成熟障礙有關(guān)。因而,加強卵泡液中細(xì)胞因子的研究將有助于提高IVF-ET的臨床妊娠率。

      女性不孕的病因是現(xiàn)代生殖醫(yī)學(xué)研究的熱點之一。多項研究發(fā)現(xiàn),卵泡液中細(xì)胞因子表達(dá)異常與卵泡發(fā)育及排卵障礙有關(guān),現(xiàn)就近年發(fā)表的有關(guān)卵泡液中細(xì)胞因子表達(dá)水平與卵泡發(fā)育及輔助生殖臨床結(jié)局相關(guān)的文獻進行綜述。

      1 血管內(nèi)皮生長因子(vascular endothelial growth factor,VEGF)和內(nèi)分泌腺來源的VEGF(endocrine gland-derived VEGF,EG-VEGF)

      VEGF是一種肝素結(jié)合雙鏈糖蛋白,能特異地促進血管內(nèi)皮增生。正常婦女的VEGF在卵泡顆粒細(xì)胞和卵泡膜細(xì)胞呈周期性表達(dá),在始基卵泡和閉鎖卵泡中無表達(dá),隨著卵泡發(fā)育表達(dá)增加,促性腺激素和低氧也可刺激其表達(dá),在促進卵巢血管生成、維持卵巢功能方面起重要作用,且能夠促進排卵前卵泡向血管化黃體的轉(zhuǎn)變,是與著床和早期妊娠相關(guān)的重要生血管因子[1]。Babitha等[2]發(fā)現(xiàn)水牛卵泡液中的VEGF可通過促進卵巢組織毛細(xì)血管增生促進卵泡選擇、生長及成熟。金銳等[3]的研究結(jié)果顯示,在促排卵周期隨著卵泡生長,血清中VEGF水平呈上升趨勢,但獲卵數(shù)≥20個組的患者卵泡液中VEGF水平低于獲卵數(shù)≤5個組,認(rèn)為成熟卵泡數(shù)目增加可能會抑制單個卵泡中VEGF的生成;而卵巢低反應(yīng)者卵泡周圍微循環(huán)不足,缺氧導(dǎo)致紅細(xì)胞代償性增多,可促進VEGF mRNA的合成。

      EG-VEGF是一種組織特異性血管生成因子,作為內(nèi)分泌腺微血管床內(nèi)皮細(xì)胞的有絲分裂原,調(diào)節(jié)內(nèi)皮細(xì)胞的生長、結(jié)構(gòu)和功能,誘導(dǎo)內(nèi)分泌腺中新血管形成。卵巢是人體EG-VEGF合成量最高的器官,主要由卵泡的顆粒細(xì)胞和膜細(xì)胞表達(dá)。有學(xué)者證實,卵泡液中VEGF和EG-VEGF濃度與卵巢反應(yīng)性及卵母細(xì)胞成熟度呈負(fù)相關(guān),血清EG-VEGF的濃度與妊娠結(jié)局相關(guān),認(rèn)為由于卵巢顆粒細(xì)胞和膜細(xì)胞是人體合成EG-VEGF的主要細(xì)胞,EG-VEGF在卵泡發(fā)育中的作用可能較VEGF更為重要,EGVEGF可通過提高子宮內(nèi)膜容受性改善妊娠結(jié)局[4-5]。另有學(xué)者發(fā)現(xiàn)體外受精(IVF)患者VEGF的基因型(+405G>C型)與胚胎種植率相關(guān)[6]。

      VEGF和EG-VEGF的組織細(xì)胞定位和時空表達(dá)呈互補關(guān)系[7]。EG-VEGF無論在卵泡液還是血清中的濃度均與人絨毛膜促性腺激素(hCG)注射日血清雌二醇(E2)濃度呈負(fù)相關(guān),而VEGF僅在卵泡液中的濃度與hCG注射日血清E2濃度呈負(fù)相關(guān),提示EGVEGF與卵巢反應(yīng)性關(guān)系更加密切。黃體早期血清中而非卵泡液中EG-VEGF水平與IVF妊娠結(jié)局有關(guān),認(rèn)為血清EG-VEGF濃度可能與子宮內(nèi)膜容受性有關(guān),EG-VEGF可能通過調(diào)節(jié)子宮內(nèi)膜血管通透性和促進低密度脂蛋白轉(zhuǎn)運入黃體細(xì)胞合成孕酮支持妊娠[8]。

      2 粒細(xì)胞集落刺激因子(granulocyte colony stimulating factor,G-CSF)

      G-CSF屬于造血生長因子,主要由造血細(xì)胞產(chǎn)生,一些非造血細(xì)胞也可以產(chǎn)生G-CSF。人卵巢中的G-CSF主要由顆粒細(xì)胞、卵泡膜細(xì)胞和基底細(xì)胞產(chǎn)生,并通過自分泌或旁分泌發(fā)揮其生物學(xué)效能,與卵巢的微環(huán)境、儲備功能及卵母細(xì)胞質(zhì)量密切相關(guān),并可作為預(yù)測妊娠結(jié)局的有效指標(biāo)之一[9]。董毅飛等[10]發(fā)現(xiàn)IVF患者取卵日血清中G-CSF濃度與卵泡液中G-CSF濃度呈正相關(guān),但明顯低于卵泡液;妊娠組卵泡液中G-CSF濃度顯著高于未妊娠組,且與獲卵數(shù)呈正相關(guān),而兩組間血清G-CSF濃度差異無統(tǒng)計學(xué)意義。在IVF/胞漿內(nèi)單精子注射(ICSI)周期中聯(lián)合檢測卵泡液中G-CSF、白細(xì)胞介素33(IL-33)及其受體IL -1受體樣1(IL -1 receptor -like 1, IL1RL1,也稱為ST2)的濃度可預(yù)測IVF的成功率[11]。Lédée等[12]也證實,卵泡液中G-CSF濃度與胚胎移植成功率呈正相關(guān),能較準(zhǔn)確地預(yù)測妊娠率。Barad 等[13]則認(rèn)為卵泡液中G-CSF濃度與子宮內(nèi)膜厚度、胚胎著床率及臨床妊娠率無明顯相關(guān)性,但該研究患者平均年齡>39歲,結(jié)論并不適用于年輕患者。

      3 活性氧簇(reactive oxygen species,ROS)與總抗氧化能力(total antioxidant capacity,TAC)

      ROS是有氧代謝的產(chǎn)物,在細(xì)胞凋亡、損傷修復(fù)和細(xì)胞增殖等信號傳遞途徑中具有重要調(diào)控作用。過高水平的ROS會導(dǎo)致細(xì)胞損傷,機體抵抗ROS的能力稱為TAC,若ROS-TAC動態(tài)平衡被打破,則可能導(dǎo)致細(xì)胞功能異常。卵泡液內(nèi)ROS主要來源于巨噬細(xì)胞、顆粒細(xì)胞、上皮細(xì)胞以及卵母細(xì)胞的代謝,在卵泡發(fā)生和卵子發(fā)育等方面起重要調(diào)控作用[14],抗氧化酶則主要來自顆粒細(xì)胞和泡膜細(xì)胞。排卵前黃體生成激素(LH)峰的出現(xiàn)可引起多個炎癥相關(guān)細(xì)胞因子表達(dá)增強,并出現(xiàn)局部急性炎癥反應(yīng),巨噬細(xì)胞、中性粒細(xì)胞等釋放大量的ROS。牛宏志等[15]發(fā)現(xiàn),IVF患者取卵日血清ROS水平顯著高于卵泡刺激前,認(rèn)為排卵反應(yīng)可導(dǎo)致卵泡液內(nèi)ROS升高,并通過微循環(huán)進入血清中,導(dǎo)致血清ROS升高,而血清和卵泡液內(nèi)TAC并未隨排卵而升高,高水平的ROS雖不影響受精,但可能對胚胎的后續(xù)發(fā)育產(chǎn)生負(fù)面影響。Shkolnik等[16]的動物實驗發(fā)現(xiàn),注射hCG后卵泡內(nèi)ROS明顯升高,而使用大量抗氧化劑抑制ROS產(chǎn)生的同時排卵也受抑制,表明ROS水平與排卵密切相關(guān)。Singh等[17]發(fā)現(xiàn),子宮內(nèi)膜異位癥及輸卵管因素不孕患者中,妊娠組卵泡液中ROS水平低于非妊娠組,推測ROS水平可能與妊娠相關(guān),但其結(jié)論及機制尚待進一步研究證實。總之,保持ROS-TAC水平的平衡可維持健康卵泡的活性,ROS-TAC失衡將對卵母細(xì)胞成熟和后續(xù)胚胎發(fā)育造成不良影響。

      4 胰島素樣生長因子(insulin-like growth factor,IGF)

      IGF是一種與胰島素結(jié)構(gòu)和功能相似的單鏈堿性蛋白,主要包括IGF-Ⅰ和IGF-Ⅱ。IGF-Ⅰ具有促進有絲分裂、參與新陳代謝等胰島素樣生物學(xué)效應(yīng),可以通過調(diào)節(jié)葡萄糖轉(zhuǎn)運蛋白8(Glut-8)促進糖吸收,在增加胰島素水平的同時引起IGF-Ⅰ升高,在生殖領(lǐng)域中發(fā)揮重要作用。多囊卵巢綜合征(PCOS)患者高水平的胰島素可通過IGF-Ⅰ影響卵泡生長[18]。Kafi等[19]發(fā)現(xiàn),卵泡液中升高的孕激素、IGF-Ⅰ和高水平的E2可促進卵母細(xì)胞最后階段的成熟。Mehta等[20]發(fā)現(xiàn),卵泡液中IGF-Ⅰ水平較高的IVF患者受精率、卵裂率、囊胚形成率、胚胎種植率及臨床妊娠率也顯著升高。IGF-Ⅱ的mRNA及相應(yīng)蛋白在優(yōu)勢卵泡的卵泡液及顆粒細(xì)胞中大量存在,而在PCOS患者卵泡及正常卵巢小竇狀卵泡的卵泡膜細(xì)胞上弱表達(dá),IGF-ⅡmRNA的表達(dá)與卵泡液中VEGF濃度呈正相關(guān),并與卵泡生長及優(yōu)勢卵泡的成熟相關(guān),妊娠患者血清及卵泡液中IGF-Ⅱ有高于未妊娠患者的趨勢[21-23]。IGF家族成員與妊娠結(jié)局的相關(guān)性優(yōu)于單一因子[24],但目前卵泡液中IGF表達(dá)與妊娠結(jié)局的相關(guān)性尚不明確。

      5 IL和白血病抑制因子(leukemia inhibitory factor,LIF)

      IL是一種在多種細(xì)胞中均有表達(dá)的糖蛋白,通過與其特異性受體結(jié)合傳遞信息,并影響生殖過程的各個環(huán)節(jié),包括卵細(xì)胞發(fā)育、成熟、排卵以及受精、著床、妊娠等。IL-1β是IL-1系統(tǒng)具有生物活性的亞型之一,與生殖活動關(guān)系最為密切,卵泡液中IL-1β水平是反映卵母細(xì)胞質(zhì)量及黃體功能的重要指標(biāo)[25]。有研究表明,PCOS患者血清中IL-1β的高表達(dá)與PCOS發(fā)生有一定相關(guān)性,且患者血清中IL-6基因啟動子區(qū)域的174 G/C單核苷酸多態(tài)性、基因型和等位基因分布與對照組間差異均有統(tǒng)計學(xué)意義,其中G等位基因頻率與PCOS發(fā)病顯著關(guān)聯(lián),卵泡液中IL-12表達(dá)水平與卵子受精及胚胎發(fā)育潛能呈正相關(guān)[26-28]。目前,炎癥細(xì)胞因子IL參與PCOS發(fā)生的機制尚未明確,但IL與PCOS的關(guān)聯(lián)性對PCOS的預(yù)防和治療具有重要參考價值。

      LIF是IL-6家族中的一個多效性細(xì)胞因子,是一種分泌型糖蛋白,LIF及其受體mRNA在卵泡發(fā)育不同階段的顆粒細(xì)胞均有表達(dá),隨卵泡發(fā)育而增加,在排卵前達(dá)高峰,在閉鎖卵泡表達(dá)消失,由此推測LIF可能參與排卵的局部調(diào)節(jié)。Haraguchi等[29]發(fā)現(xiàn),豬體內(nèi)存在LIF/信號轉(zhuǎn)導(dǎo)和轉(zhuǎn)錄活化因子3(STAT3)通路,是否可通過補充LIF激活該通路提高卵母細(xì)胞的發(fā)育潛能尚待進一步研究證實。

      綜上所述,卵泡液內(nèi)細(xì)胞因子的異常表達(dá)可影響卵泡的發(fā)育及成熟,但其機制尚不明確。隨著高通量測序技術(shù)的發(fā)展,通過檢測卵泡液中的相關(guān)細(xì)胞因子的基因表達(dá),有望為評價卵子質(zhì)量及促進卵泡發(fā)育成熟提供新的治療策略。

      【Abstract】With the increasing technology in vitro fertilization-embryo transfer(IVF-ET), the roles of cytokines in the regulation of oocyte development in ovarian microenvironment have been widely concerned. Cytokines as a class of hormone-like peptides with comprehensive biological activity participate in the regulation of reproduction, such as the follicular development, embryo development and blastocyst implantation and others, by regulating ovarian hormones and paracrine. It was found that the abnormal expression of cytokine profile in follicular fluid, such as vascular endothelial growth factor(VEGF), granulocyte colony stimulating factor(G-CSF), reactive oxygen species(ROS), insulin-like growth factor(IGF), interleukin(IL), and leukemia inhibitory factor (LIF)could be related to the follicular mature disturbance. The enhancing research in this area will be helpful to improve the clinical pregnancy outcomes of IVF-ET.
