摘要:目的:探究自動(dòng)管電流調(diào)制技術(shù)(ATCM)在頸部CT檢查中的圖像質(zhì)量及輻射劑量控制中的價(jià)值。方法:選擇本院于2014年12月-2016年11月間收治的87例頸部CT檢查患者為研究主體。劃分為A組和B組,分別是44例與43例。結(jié)果:A組的上頸部圖像質(zhì)量評(píng)分低于B組,下頸部圖像質(zhì)量評(píng)分高于B組,對(duì)比差異明顯,有統(tǒng)計(jì)學(xué)意義(P<0 05),且87例患者的圖像質(zhì)量均在診斷要求范圍內(nèi)。A組的上頸部客觀噪聲水平高于B組,下頸部客觀噪聲水平低于B組,對(duì)比差異顯著,有統(tǒng)計(jì)學(xué)意義(P<0 05)。B組的管電流大小并無(wú)明顯變化,A組的上、下頸部管電流對(duì)比,有差異,有統(tǒng)計(jì)學(xué)意義(P<0 05)。A組的CTDIvol(容積CT劑量指數(shù))低于B組,對(duì)比差異顯著,有統(tǒng)計(jì)學(xué)意義(P<0 05)。結(jié)論:ATCM技術(shù)與固定管電流技術(shù)相比,可以明顯控制患者頸部CT檢查中的輻射劑量,且圖像質(zhì)量較佳,值得推廣。

The Value of Automatic Tube Current Modulation Technique in Image Quality and
Radiation Dose Control of Neck CT

Abstract: Objective: To explore the value of automatic tube current modulation (ATCM) technology in the image quality and radiation dose control of neck CT examination Methods: 87 cases of cervical CT patients admitted to our hospital from December 2014 to November 2016 were selected as research subjects The patients were divided into A group and B group, 44 cases and 43 cases, respectively The image quality and radiation dose were compared Results: The image quality score of A group was lower than that B group, the image quality score of lower neck was higher than that B group, and the difference was statistically significant(P<0 05) The image quality of 87 patients was within the range of diagnostic requirements The A group had higher level of noise on the upper neck than in the B group, and the lower cervical noise level was lower than that in the B group, and the difference was statistically significant(P<0 05) There was no significant change in the tube current in the B group, and there was a significant difference between the upper and lower cervical canal currents in the A group (P<0 05) The CTDIvol (volume CT dose index) of A group was lower than that B group, and the difference was statistically significant(P<0 05) Conclusion: Compared with the fixed tube current technique, ATCM technique can obviously control the radiation dose in patients with cervical CT examination, and the image quality is better, so it is worth popularizing

頸部CT檢查具有掃描時(shí)間偏短、速度較快等優(yōu)勢(shì),被廣泛用于臨床頸部疾病的治療中[1]。CT檢查的圖像分辨率由射線的劑量值決定,所以常通過(guò)加大輻射量的方式,獲取更為清晰的圖像[2]。本文旨在分析ATCM技術(shù)在頸部CT檢查中的圖像質(zhì)量及輻射劑量控制中的價(jià)值,詳細(xì)如下:
1 資料與方法
1 1 一般資料
選擇本院于2014年12月-2016年11月間收治的87例頸部CT檢查患者為研究主體。劃分為A組和B組,分別是44例與43例。A組中,男26例,女18例;年齡范圍是31-75歲,平均(55 24±3 11)歲;體重范圍是47-62kg,平均(58 21±3 61)kg。B組中,男25例,女18例;年齡范圍是33-74歲,平均(55 12±3 08)歲;體重范圍是48-67kg,平均(58 27±3 17)kg。對(duì)比以上數(shù)據(jù),差異不明顯,無(wú)統(tǒng)計(jì)學(xué)意義(P>0 05),可比較。
1 2 方法
兩組均利用多層螺旋CT進(jìn)行頸部檢查。A組給予ATCM技術(shù)處理,且NI(噪聲指數(shù))為10。管電壓為120kv,管電流的調(diào)制范圍75至500mA,Pitch為0 984,探測(cè)器的準(zhǔn)直為64×0 625mm,機(jī)架轉(zhuǎn)速為0 6s/圈,DFOV為20至25cm,SFOV為32cm。
1 3 觀察指標(biāo)
圖像質(zhì)量評(píng)價(jià):將圖像傳至處理工作站,并有效去除圖像表面的技術(shù)信息。將上頸部與下頸部的平掃圖像作為質(zhì)量評(píng)估主體,所有操作由1名醫(yī)生+1名技師完成。
圖像質(zhì)量診斷要求:通過(guò)5分法進(jìn)行評(píng)估,不能接受記為1分,平均以下記為2分,能夠接受記為3分,平均以上記為4分,可很好接受記為5分。
1 4 統(tǒng)計(jì)學(xué)分析
數(shù)據(jù)通過(guò)SPSS16 0軟件加以處理,圖像質(zhì)量評(píng)分、客觀噪聲水平、管電流值和CTDIvol數(shù)值均用( )表示,經(jīng)t檢驗(yàn),若P<0 05,則說(shuō)明差異明顯,有統(tǒng)計(jì)學(xué)意義。
2 結(jié)果
2 1 對(duì)比圖像質(zhì)量
兩組的圖像質(zhì)量評(píng)分對(duì)比,有差異,有統(tǒng)計(jì)學(xué)意義(P<0 05)。此外,87例患者的圖像質(zhì)量評(píng)分均高于3分,因此均在診斷要求范圍內(nèi),結(jié)果見(jiàn)表1。
3 討論
數(shù)據(jù)表明,CT檢查占據(jù)臨床放射學(xué)檢查總數(shù)的5%左右,而照射劑量在放射學(xué)檢查中的所占比率約為35%[3]。在我國(guó),CT裝機(jī)量占據(jù)世界總量的第3位,且呈增長(zhǎng)態(tài)勢(shì)。由此,CT檢查中的輻射劑量是我國(guó)醫(yī)療照射源的主要研究課題[4]。
結(jié)果顯示,A組的上頸部圖像質(zhì)量評(píng)分低于B組,下頸部圖像質(zhì)量評(píng)分高于B組(P<0 05)。且87例患者的圖像質(zhì)量均在診斷要求范圍內(nèi)。A組的CTDIvol低于B組(P<0 05)。因此,ATCM技術(shù)與固定管電流技術(shù)相比,可以明顯控制患者頸部CT檢查中的輻射劑量,且圖像質(zhì)量較佳,可推廣。
