海綿竇位于蝶鞍兩側(cè),頸內(nèi)動(dòng)脈和第Ⅲ~ Ⅵ對(duì)腦神經(jīng)(動(dòng)眼神經(jīng)、滑車神經(jīng)、三叉神經(jīng)和外展神經(jīng))穿行其中,動(dòng)眼神經(jīng)、滑車神經(jīng)和椎動(dòng)脈V1段、V2段走行于其外側(cè)壁[1]。該區(qū)域病變手術(shù)難度大、風(fēng)險(xiǎn)高,術(shù)后可能出現(xiàn)海綿竇內(nèi)腦神經(jīng)功能障礙等較嚴(yán)重并發(fā)癥,因此,術(shù)中腦神經(jīng)解剖和功能保護(hù)尤為重要。神經(jīng)電生理監(jiān)測(cè)是識(shí)別海綿竇內(nèi)腦神經(jīng)和降低醫(yī)原性損傷的有效方法[2?4],術(shù)中監(jiān)測(cè)腦神經(jīng)可以使神經(jīng)外科醫(yī)師在切除腫瘤過程中調(diào)整手術(shù)操作和策略,減少腦神經(jīng)損傷。目前常用的神經(jīng)電生理監(jiān)測(cè)方法包括自發(fā)肌電圖(f?EMG)和觸發(fā)肌電圖(t?EMG)監(jiān)測(cè)[5]。f?EMG 監(jiān)測(cè)是一種連續(xù)記錄某一神經(jīng)支配肌肉或肌群的肌電活動(dòng),特征是捕捉術(shù)中某一腦神經(jīng)受牽拉、擠壓等刺激產(chǎn)生的神經(jīng)張力放電;t?EMG監(jiān)測(cè)記錄對(duì)神經(jīng)干進(jìn)行電刺激時(shí)對(duì)應(yīng)肌群出現(xiàn)的相應(yīng)電生理信號(hào),可獲得復(fù)合肌肉動(dòng)作電位(CMAP)波形,以評(píng)估神經(jīng)功能完整性。本研究通過術(shù)中監(jiān)測(cè)動(dòng)眼神經(jīng)、滑車神經(jīng)、三叉神經(jīng)和外展神經(jīng)f?EMG和t?EMG,探討術(shù)中神經(jīng)電生理監(jiān)測(cè)在內(nèi)鏡下擴(kuò)大經(jīng)鼻蝶入路手術(shù)治療海綿竇斜坡區(qū)腫瘤中的價(jià)值。
1.納入標(biāo)準(zhǔn) (1)術(shù)前頭部MRI顯示海綿竇斜坡區(qū)占位性病變,Knosp分級(jí)3~ 4級(jí)和(或)腫瘤侵犯斜坡。(2)均行內(nèi)鏡下擴(kuò)大經(jīng)鼻蝶入路手術(shù),并經(jīng)術(shù)后病理證實(shí)診斷。(3)術(shù)中均行神經(jīng)電生理監(jiān)測(cè)。(4)18~ 80歲。(5)所有患者及其家屬均對(duì)手術(shù)方案和術(shù)中監(jiān)測(cè)知情并簽署知情同意書。
2.排除標(biāo)準(zhǔn) (1)術(shù)前影像學(xué)顯示海綿竇斜坡區(qū)占位性病變,且內(nèi)分泌功能測(cè)定血清催乳素(PRL)>200 ng/ml。(2)術(shù)前評(píng)估不宜行內(nèi)鏡下經(jīng)鼻蝶入路手術(shù)。(3)伴嚴(yán)重全身性疾病無法耐受手術(shù)。
3.一般資料 選擇2019年8月至2020年12月在中山大學(xué)附屬第一醫(yī)院神經(jīng)外科住院治療的海綿竇斜坡區(qū)腫瘤患者共18例,男性5例,女性13例;年齡21~ 69歲,平均44.82歲;臨床表現(xiàn)為頭暈和(或)頭痛占8/18,視力下降或喪失占7/18,視野缺損占3/18,月經(jīng)周期紊亂或閉經(jīng)占2/18,腦神經(jīng)(外展功能)功能障礙占2/18,肢端肥大占1/18,嘔吐占1/18,鼻腔堵塞占1/18;腫瘤位于海綿竇占14/18例,蝶竇、蝶骨、篩竇、上頜竇、翼腭窩占10/18,鞍上(包括鞍區(qū))占9/18,斜坡占8/18,鞍內(nèi)占4/18,腦橋小腦角占2/18,鞍背占1/18,巖部尖占1/18,后床突占1/18,頸內(nèi)動(dòng)脈管占1/18;均行內(nèi)鏡下擴(kuò)大經(jīng)鼻蝶入路手術(shù),術(shù)中監(jiān)測(cè)動(dòng)眼神經(jīng)、滑車神經(jīng)、三叉神經(jīng)和外展神經(jīng)。18例海綿竇斜坡區(qū)腫瘤患者的臨床資料參見表1。

1.內(nèi)鏡下擴(kuò)大經(jīng)鼻蝶入路手術(shù) (1)麻醉方法:采用靶控輸注(TCI)異丙酚(1.40μg/ml)和瑞芬太尼(8 ng/ml)靜脈注射誘導(dǎo)麻醉,僅氣管插管時(shí)予短效肌松藥羅庫溴銨(0.90 mg/kg)靜脈滴注,手術(shù)期間靶控輸注丙泊酚(2~ 3μg/ml)和瑞芬太尼(4~ 6 ng/ml)持續(xù)靜脈泵入維持麻醉。監(jiān)測(cè)體溫和血壓于正常值范圍。(2)手術(shù)方法:均行術(shù)中神經(jīng)導(dǎo)航和MRI輔助的內(nèi)鏡下擴(kuò)大經(jīng)鼻蝶入路海綿竇斜坡區(qū)腫瘤切除術(shù),先以單級(jí)電刺激器(PNG2.3/90型,西安富德醫(yī)療電子有限公司)探查腦神經(jīng)走行,于神經(jīng)電生理監(jiān)測(cè)下逐步切除腫瘤,如果切除過程中出現(xiàn)異常自發(fā)式肌電波形,以單級(jí)電刺激器再次確認(rèn)神經(jīng)部位和走行,再采取更加精細(xì)的操作,避免損傷相關(guān)神經(jīng);如果腫瘤侵犯或包繞腦神經(jīng),則在不加重神經(jīng)損傷的情況下盡可能切除腫瘤。腫瘤切除后,以單級(jí)電刺激器評(píng)估神經(jīng)功能。
2.術(shù)中神經(jīng)電生理監(jiān)測(cè) 麻醉后手術(shù)前,由專業(yè)的神經(jīng)電生理科醫(yī)師徒手將一次性成對(duì)雙絞線針狀電極置入皮下,接地電極置于肩部,記錄上瞼提肌、上斜肌、咀嚼肌、外直肌肌電圖。術(shù)中記錄上瞼提肌、上斜肌、咀嚼肌、外直肌 f?EMG和 t?EMG,電反應(yīng)放大倍數(shù)為5000,濾波帶寬為30~ 1500 Hz;自發(fā)式肌電反應(yīng)(SMA)分析時(shí)程和波幅靈敏度分別為5 ms/D和100μV,復(fù)合肌肉動(dòng)作電位分析時(shí)程和波幅靈敏度分別為2 ms/D和50μV。術(shù)中以單級(jí)電刺激器刺激動(dòng)眼神經(jīng)、滑車神經(jīng)、三叉神經(jīng)和外展神經(jīng),刺激強(qiáng)度0.50~ 2.00 mA、刺激頻率1 Hz、刺激時(shí)間 0.20 ms。(1)f?EMG:術(shù)中持續(xù)記錄 f?EMG,觀察異常自發(fā)式肌電波形。術(shù)中出現(xiàn)的f?EMG包括6種波形[6],①靜息態(tài)下正常肌電波形。②偶發(fā)單個(gè)尖銳肌電波形。③連續(xù)爆發(fā)的數(shù)個(gè)肌電波形。④突發(fā)單個(gè)肌電活動(dòng)后的連續(xù)放電波形。⑤連續(xù)發(fā)生的肌電波形。⑥規(guī)律的自發(fā)式肌電波形。術(shù)中主要關(guān)注規(guī)律的自發(fā)式肌電波形,該肌電波形與術(shù)中對(duì)運(yùn)動(dòng) 神經(jīng)的機(jī)械牽拉和擠 壓等刺激直接相關(guān)[6,8?10],出現(xiàn)此種肌電波形時(shí),術(shù)者需及時(shí)改變手術(shù)操作甚至?xí)和J中g(shù)。(2)t?EMG:術(shù)中可疑神經(jīng)組織時(shí),根據(jù)t?EMG判斷其為神經(jīng)組織還是腫瘤組織,既可有效識(shí)別,又可清晰顯示被腫瘤侵犯或包繞的神經(jīng)形態(tài)。腫瘤切除后,電刺激動(dòng)眼神經(jīng)、滑車神經(jīng)、三叉神經(jīng)和外展神經(jīng),根據(jù)肌肉端記錄的t?EMG潛伏期和波幅,評(píng)估神經(jīng)結(jié)構(gòu)和功能完整性。根椐腫瘤侵襲頸內(nèi)動(dòng)脈海綿竇段的部位和范圍,分為上間隙、后間隙、前間隙和外側(cè)間隙,術(shù)中神經(jīng)生理監(jiān)測(cè)范圍取決于腫瘤對(duì)海綿竇和斜坡的侵犯程度,神經(jīng)功能分為4級(jí),Ⅰ級(jí),正常;Ⅱ級(jí),離散性輕癱或主觀報(bào)告的復(fù)視,但眼動(dòng)無明顯異常;Ⅲ級(jí),嚴(yán)重輕癱,眼動(dòng)受限但仍有輕微眼動(dòng);Ⅳ級(jí),癱瘓或麻痹[7]。
3.觀察指標(biāo) 分別于術(shù)后1周、出院時(shí)和術(shù)后3個(gè)月行眼科檢查、神經(jīng)電生理監(jiān)測(cè)和垂體MRI增強(qiáng)掃描,以評(píng)估視覺功能、神經(jīng)功能和腫瘤切除程度。所有病例均在門診完成隨訪,根椐術(shù)后3個(gè)月神經(jīng)功能和腫瘤切除程度決定后續(xù)隨訪時(shí)間和治療措施。
本研究18例患者均順利完成手術(shù),9例腫瘤全切除、4例次全切除、5例大部切除。術(shù)后經(jīng)病理證實(shí)垂體腺瘤10例(圖1),脊索瘤5例(圖2),顱咽管瘤2例(圖3),神經(jīng)鞘瘤囊性變1例。術(shù)中共監(jiān)測(cè)動(dòng)眼神經(jīng)20條,滑車神經(jīng)3條,三叉神經(jīng)27條,外展神經(jīng)26條(表1)。術(shù)后隨訪3~ 13個(gè)月,平均8.63個(gè)月 。術(shù)前1例左側(cè)外展神經(jīng)功能重度癱瘓(Ⅳ級(jí))患者,術(shù)后3個(gè)月為外展神經(jīng)輕癱(Ⅱ級(jí)),術(shù)后6個(gè)月功能恢復(fù)正常(Ⅰ級(jí));1例出現(xiàn)右側(cè)外展神經(jīng)輕癱(Ⅱ級(jí))患者,術(shù)后3個(gè)月功能恢復(fù)正常。術(shù)后有1例出現(xiàn)左側(cè)動(dòng)眼神經(jīng)重度癱瘓(Ⅳ級(jí))和1例出現(xiàn)左側(cè)外展神經(jīng)麻痹(Ⅱ級(jí)),均于術(shù)后3個(gè)月功能恢復(fù)正常;均未出現(xiàn)三叉神經(jīng)功能障礙;1例術(shù)后發(fā)生左蝶腭動(dòng)脈出血,內(nèi)鏡下止血后未再出血。

根椐腫瘤侵襲頸內(nèi)動(dòng)脈海綿竇段的部位和范圍,分為上間隙、后間隙、前間隙和外側(cè)間隙,上間隙主要走行動(dòng)眼神經(jīng),后間隙為外展神經(jīng),前間隙為外展神經(jīng)及其交感神經(jīng)節(jié)或神經(jīng)叢,外側(cè)間隙為動(dòng)眼神經(jīng)、滑車神經(jīng)、三叉神經(jīng)和外展神經(jīng)。根椐腫瘤侵襲部位和是否侵襲雙側(cè)海綿竇,選擇性監(jiān)測(cè)2~ 3對(duì)腦神經(jīng)。根椐解剖位置和神經(jīng)走行,術(shù)中易損傷外展神經(jīng),因此本組1 8例患者均需術(shù)中常規(guī)監(jiān)測(cè)外展神經(jīng)。腫瘤侵犯上間隙的比例較高,上間隙主要走行動(dòng)眼神經(jīng),故本研究術(shù)中亦監(jiān)測(cè)動(dòng)脈神經(jīng)。外側(cè)間隙走行動(dòng)眼神經(jīng)、滑車神經(jīng)、三叉神經(jīng)和外展神經(jīng),因此進(jìn)入外側(cè)間隙的手術(shù)需監(jiān)測(cè)上述腦神經(jīng)。本研究早期術(shù)中監(jiān)測(cè)滑車神經(jīng),但實(shí)際手術(shù)過程中較少涉及該神經(jīng),加之電極置入時(shí)亦有一定風(fēng)險(xiǎn),故后期未常規(guī)術(shù)中監(jiān)測(cè)滑車神經(jīng)。由專業(yè)的神經(jīng)電生理科技師將一次性成對(duì)雙絞線針狀電極置入皮下,將電極置入眼眶外的上瞼提肌、上斜肌、眼外直肌外表面,可以減少電極尖與角膜的接觸,從而避免角膜損傷,本研究未出現(xiàn)角膜損傷等并發(fā)癥;同時(shí),采用成對(duì)雙絞線針狀電極可以有效增加神經(jīng)電生理監(jiān)測(cè)的敏感性。
目前,f?EMG 和 t?EMG 監(jiān)測(cè)已常規(guī)應(yīng)用于其他腦神經(jīng)監(jiān)測(cè)并證實(shí)有效,而海綿竇腫瘤切除術(shù)中監(jiān)測(cè)動(dòng)眼神經(jīng)、滑車神經(jīng)、三叉神經(jīng)和外展神經(jīng)的研究相對(duì)較少且存有爭議。Kaspera等[7]發(fā)現(xiàn),腦橋小腦角腫瘤手術(shù)中,腦神經(jīng)通常位于腫瘤外,而非被腫瘤包裹,腦神經(jīng)被腫瘤壓迫成一束微小纖維;而海綿竇內(nèi)腦神經(jīng),可能被腫瘤浸潤或侵襲,或者獨(dú)立于腫瘤之外,因此認(rèn)為,雖然無法避免腫瘤侵襲或浸潤造成的神經(jīng)損傷,但術(shù)中可見的腦神經(jīng)無需神經(jīng)電生理監(jiān)測(cè)。
海綿竇內(nèi)有頸內(nèi)動(dòng)脈和動(dòng)眼神經(jīng)、滑車神經(jīng)、三叉神經(jīng)、外展神經(jīng)等重要解剖結(jié)構(gòu),即使微創(chuàng)手術(shù)也可能出現(xiàn)血管神經(jīng)損傷,影響患者生活質(zhì)量。而且,腦神經(jīng)無神經(jīng)束膜和神經(jīng)外膜,表面僅有一層或兩層不連續(xù)的扁平鞘膜細(xì)胞[7],較周圍神經(jīng)更易損傷。因此,術(shù)中監(jiān)測(cè)腦神經(jīng)功能完整性即顯得十分重要[11]。
本研究監(jiān)測(cè)18例海綿竇斜坡腫瘤涉及的上瞼提肌、上斜肌、咀嚼肌和眼外肌 f?EMG 和 t?EMG,其中2例患者術(shù)后出現(xiàn)腦神經(jīng)功能障礙,腦神經(jīng)損傷發(fā)生率為2/18,并于術(shù)后3個(gè)月功能恢復(fù)正常,表明術(shù)中神經(jīng)電生理監(jiān)測(cè)可有效降低腦神經(jīng)損傷發(fā)生率。有研究顯示,顱底外科手術(shù)中,采用不同術(shù)中監(jiān)測(cè)的腦神經(jīng)損傷發(fā)生率為 2%~ 47%[12?15],而未行神經(jīng)電生理監(jiān)測(cè)的患者,其腦神經(jīng)損傷發(fā)生率為14%~ 68%[12,16]。
f?EMG監(jiān)測(cè)是對(duì)支配肌肉的神經(jīng)進(jìn)行機(jī)械牽拉、擠壓、熱等因素刺激時(shí)出現(xiàn)的自發(fā)性神經(jīng)張力放電[9]。f?EMG 是神經(jīng)損傷的敏感指標(biāo),但其出現(xiàn)時(shí)并不意味神經(jīng)損傷,其缺失亦不能保證無神經(jīng)損傷。Nelson等[17]認(rèn)為,神經(jīng)橫斷可能不產(chǎn)生神經(jīng)張力放電;神經(jīng)一旦切斷,如果遠(yuǎn)端受刺激,仍可以記錄到誘發(fā)反應(yīng);如果遠(yuǎn)端有機(jī)械刺激,即可激發(fā)神經(jīng)張力放電。這種現(xiàn)象可能使神經(jīng)電生理學(xué)專家和神經(jīng)外科醫(yī)師對(duì)腦神經(jīng)的連續(xù)性產(chǎn)生誤解。然而,t?EMG與 f?EMG相結(jié)合,可提高監(jiān)測(cè)腦神經(jīng)的連續(xù)性和準(zhǔn)確性。術(shù)中如果懷疑腦神經(jīng)切斷,電刺激神經(jīng)遠(yuǎn)端無法產(chǎn)生誘發(fā)電位。此外,損傷的腦神經(jīng)有更高的刺激閾值[17]。據(jù)此有助于神經(jīng)外科醫(yī)師判斷腦神經(jīng)結(jié)構(gòu)的完整性。f?EMG可實(shí)時(shí)反映神經(jīng)肌肉狀態(tài),具有時(shí)限性,且受諸多因素(如電凝、電刀)的干擾,導(dǎo)致肌電圖波形變化萬千,有一定的局限性。這就更需要有臨床經(jīng)驗(yàn)的神經(jīng)電生理科醫(yī)師能夠準(zhǔn)確辨別異常波形是否與手術(shù)相關(guān),方能給術(shù)者提供準(zhǔn)確客觀的信息以指導(dǎo)手術(shù)。
綜上所述,內(nèi)鏡下擴(kuò)大經(jīng)鼻蝶海綿竇斜坡區(qū)腫瘤切除術(shù)中采用神經(jīng)電生理監(jiān)測(cè)動(dòng)眼神經(jīng)、滑車神經(jīng)、三叉神經(jīng)和外展神經(jīng)是一種安全、有效的方法,通過監(jiān)測(cè)和評(píng)估術(shù)中f?EMG和t?EMG,使神經(jīng)外科醫(yī)師在腫瘤切除過程中改變手術(shù)操作和策略,減少腦神經(jīng)損傷。
