[摘要] 目的 探討同型半胱氨酸(Hcy)、超敏C-反應(yīng)蛋白(hs-CRP)和D-二聚體(DD)聯(lián)合檢測(cè)在腦卒中診斷和預(yù)后判斷中的應(yīng)用價(jià)值。 方法 選取本院2013年5月~2014年9月收治的224例腦卒中(腦出血96例,腦梗死128例)患者作為研究對(duì)象,選取同期的115例健康體檢者作為健康對(duì)照組,比較各組的Hcy、hs-CRP和DD水平。 結(jié)果 腦卒中組的Hcy、hs-CRP和DD水平顯著高于健康對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。腦出血組與腦梗死組的Hcy、hs-CRP和DD水平顯著高于健康對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。腦卒中組的Hcy、hs-CRP與DD聯(lián)合檢測(cè)的陽性率為91.5%,顯著高于健康對(duì)照組的3.5%,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。腦卒中組Hcy、hs-CRP、DD檢測(cè)的陽性率顯著高于健康對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。 結(jié)論 Hcy水平是腦卒中的重要預(yù)測(cè)因子,Hcy、hs-CRP和DD聯(lián)合檢測(cè)有助于腦卒中的早期診治,并且對(duì)判斷預(yù)后也有重要的臨床價(jià)值。

[Abstract] Objective To explore the application value of combined detection of homocysteine (Hcy),hypersensitive C- reactive protein (hs-CRP) and D-dimer (DD) in the diagnosis and prognosis of stroke. Methods 224 cases of stroke (96 cases of cerebral hemorrhage,128 cases of cerebral infarction) from May 2013 to September 2014 in our hospital were selected as the research object,115 healthy persons in the same period were selected as the healthy control group.The level of Hcy,hs-CRP and DD in the each group was compared. Results The level of Hcy,hs-CRP and DD in the stroke group was higher than that in the healthy control goup,with significant difference (P<0.05).The level of Hcy,hs-CRP and DD in the cerebral hemorrhage group and the cerebral infarction group was higher than that in the healthy control goup,with significant difference (P<0.05).The positive rate of combined detection of Hcy,hs-CRP and DD in the stroke group was 91.5%,which was higher than 3.5% in the healthy control goup,with significant difference (P<0.05).The positive rate of Hcy,hs-CRP and DD in the stroke group was higher than that in the healthy control goup,with significant difference (P<0.05). Conclusion Hcy level is an important predictor of stroke,combined detection of Hcy,hs-CRP and DD is helpful for early diagnosis and treatment of stroke,and which also has important clinical value in judging prognosis.

腦卒中病情嚴(yán)重,預(yù)后差,是一種嚴(yán)重影響人類健康的常見病。研究顯示[1],血清同型半胱氨酸(homocysteine,Hcy)和超敏C反應(yīng)蛋白(hypersensitive C-reactive protein,hs-CRP)指標(biāo)對(duì)于腦卒中發(fā)病的預(yù)防及病情分析具有重要意義。Hcy和hs-CRP是動(dòng)脈硬化和缺血性腦卒中的獨(dú)立危險(xiǎn)因素[2],并且血液成分(高凝狀態(tài))和血流動(dòng)力學(xué)的改變也是其發(fā)病的重要原因之一[3]。D二聚體(D-Dmier,DD)是纖維蛋白降解產(chǎn)物中的最小片段,是反映凝血和纖溶活化的分子標(biāo)志物。本研究通過檢測(cè)Hcy、hs-CRP和DD水平,探討其在腦卒中診斷及預(yù)后判斷中的應(yīng)用價(jià)值。
1 資料與方法
1.1 一般資料
選取本院神經(jīng)內(nèi)科2013年5月~2014年9月收治的224例腦卒中患者作為研究對(duì)象,均參照1995年全國第四屆腦血管病學(xué)術(shù)會(huì)議通過的診斷標(biāo)準(zhǔn)[4]確診,排除肝、腎功能不全等患者,其中腦出血96例,腦梗死128例。腦出血組中,男54例,女42例;年齡46~77歲,平均63.2歲。腦梗死組中,男75例,女53例;年齡43~83歲,平均68.7歲。兩組的性別、年齡等一般資料比較,差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。選取同期的115例健康體檢者作為健康對(duì)照組,其中男64例,女51例;年齡47~76歲,平均62.5歲。本研究經(jīng)本院醫(yī)學(xué)倫理委員會(huì)批準(zhǔn),均取得患者知情同意。
