摘要:目的 ?觀察臭氧水局部注射治療跟痛癥的臨床療效。方法 ?選擇 2016年6月~2018年8月在我院疼痛科門診診斷為跟痛癥的患者72例,隨機分為對照組和觀察組,每組36例。觀察組采用臭氧水局部注射治療,對照組采用抗炎鎮(zhèn)痛液局部注射治療,記錄并比較兩組患者治療前和治療后1周、1個月、3個月視覺模擬評分(VAS)和臨床療效。結(jié)果 ?觀察組治療后1周、1個月、3個月的VAS評分分別為(4.18±0.81)分、(3.32±0.56)分、(2.43±0.47)分,均低于治療前的(6.88±0.76)分,對照組治療后1周、1個月、3個月的VAS評分分別為(4.94±0.65)分、(4.31±0.62)分、(3.89±0.52)分,均低于治療前的(6.64±0.52)分,差異均有統(tǒng)計學意義(P<0.05)。治療后1周、1個月、3個月時,觀察組VAS評分均低于對照組,差異有統(tǒng)計學意義(P<0.05)。治療3個月后,觀察組總有效率高于對照組(94.44% vs 80.56%),差異有統(tǒng)計學意義(P<0.05)。結(jié)論 ?臭氧水局部注射治療跟痛癥能更加顯著緩解患者癥狀,起效迅速,優(yōu)于抗炎鎮(zhèn)痛液注射治療。

Abstract:Objective ?To observe the clinical effect of ozone water local injection in the treatment of heel pain. Methods ?From June 2016 to August 2018, 72 patients with heel pain were randomly divided into control group (n=36) and observation group (n=36).The observation group was treated with ozone water local injection, and the control group was treated with local injection of anti-inflammatory and analgesic liquid. The visual analog score (VAS) and clinical efficacy of the two groups were recorded and compared before treatment and 1 week, 1 month and 3 months after treatment. Results ?The VAS scores of the observation group at 1 week, 1 month, and 3 months after treatment were (4.18±0.81), (3.32±0.56), and (2.43±0.47), respectively, which were lower than those before treatment (6.88±0.76). The VAS scores at 1 week, 1 month, and 3 months after treatment in the control group were (4.94±0.65), (4.31±0.62), and (3.89±0.52), respectively, which were lower than those before treatment (6.64±0.52), the difference was statistically significant (P<0.05). At 1 week, 1 month, and 3 months after treatment, the VAS scores of the observation group were lower than the control group,the difference was statistically significant (P<0.05). After 3 months of treatment, the total effective rate of the observation group was higher than that of the control group (94.44% vs 80.56%),the difference was statistically significant (P<0.05). Conclusion ?Local injection of ozone water can significantly relieve the symptoms of patients with heel pain, and it has a rapid onset of action, which is superior to anti-inflammatory analgesic injection.
目前,臭氧治療在軟組織勞損、骨關(guān)節(jié)疾病等炎性疼痛中應用廣泛,有研究表明[5]臭氧注射治療肌筋膜疼痛,能夠使肌肉、筋膜代謝再活化,中和酸中毒,增加三磷酸酰酐(ATP)合成、Ca2+的再攝取和水腫的重吸收;周友龍等[6]應用臭氧水沖洗治療膝骨關(guān)節(jié)炎,可提高合成軟骨基質(zhì)的能力,延緩軟骨退變,有增強軟骨再生修復的作用,抑制關(guān)節(jié)腔內(nèi)炎癥因子的釋放,清除氧自由基。臭氧水注射治療時須嚴格無菌操作,禁止軟組織內(nèi)高阻力注射,注射過程中注意病人反饋,復制疼痛,到達靶點回抽無異常后注射。本研究中采用跟骨內(nèi)側(cè)穿刺點,對痛點、筋膜間隙及足底外側(cè)神經(jīng)分支多個病因部位注射,既起到了清除局部炎癥,松解筋膜的作用,同時又對病灶神經(jīng)進行了阻滯和調(diào)節(jié)。近年來,應用機械物理效應治療跟痛癥的療效也得到驗證。有學者[7]采用射頻聯(lián)合抗炎鎮(zhèn)痛液注射治療跟痛癥,其近期、遠期效果均滿意,射頻治療的作用是消除炎癥,松解粘連,改善組織血循環(huán),調(diào)節(jié)神經(jīng)功能,與化學藥物注射聯(lián)合能起到協(xié)同作用,臭氧水局部注射聯(lián)合物理治療方式治療跟痛癥仍有待進一步研究。周增華等[8]使用沖擊波治療跟痛癥,通過直接抑制神經(jīng)末梢,降低痛覺感受器的敏感性,達到鎮(zhèn)痛作用,還起到了松解病變組織粘連,改善局部血液循環(huán),加速組織活性物質(zhì)生成和疼痛代謝產(chǎn)物排泄的作用。
臭氧水局部注射治療跟痛癥,可消除病灶區(qū)域炎癥,改善局部血液循環(huán),調(diào)節(jié)神經(jīng)敏感性,從而減輕疼痛。本研究結(jié)果顯示,治療后1周、1個月、3個月時兩組VAS評分均低于治療前,差異有統(tǒng)計學意義(P<0.05);治療后1周、1個月、3個月時,觀察組VAS評分低于對照組,差異有統(tǒng)計學意義(P<0.05)。治療3個月后,觀察組總有效率高于對照組(94.44% vs 80.56%),差異有統(tǒng)計學意義(P<0.05)。
綜上所述,臭氧水局部注射治療跟痛癥療效顯著,起效迅速,副作用極低,優(yōu)于單純抗炎鎮(zhèn)痛液注射治療。
