      【摘要】 目的:探討不同入路全髖關(guān)節(jié)置換術(shù)治療GardenⅢ、Ⅳ型股骨頸骨折效果及對(duì)肌肉組織損傷、骨代謝的影響。方法:選擇濟(jì)寧市第二人民醫(yī)院2018年1月-2020年6月收治的股骨頸骨折行全髖關(guān)節(jié)置換術(shù)(THA)的患者93例,按照手術(shù)方式不同將患者分為DAA組45例與PLA組48例,DAA組給予直接前入路THA,PLA組采用后外側(cè)入路THA。比較兩組手術(shù)相關(guān)指標(biāo)、疼痛及髖關(guān)節(jié)功能評(píng)分、并發(fā)癥發(fā)生情況、肌肉組織損傷及骨代謝指標(biāo)。結(jié)果:與PLA組比較,DAA組手術(shù)時(shí)間更長(zhǎng)、術(shù)中出血量更少、切口長(zhǎng)度更短,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05),兩組髖臼前傾角及髖臼外展角比較,差異均無統(tǒng)計(jì)學(xué)意義(P>0.05)。兩組術(shù)前視覺模擬評(píng)分(VAS)及Harris評(píng)分比較,差異均無統(tǒng)計(jì)學(xué)意義(P>0.05);兩組術(shù)后VAS評(píng)分及Harris評(píng)分均優(yōu)于術(shù)前,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05);術(shù)后1、3 d,DAA組VAS評(píng)分均低于PLA組(P<0.05);術(shù)后2周及3個(gè)月,DAA組Harris評(píng)分均高于PLA組(P<0.05),術(shù)后6個(gè)月兩組Harris評(píng)分比較,差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。兩組并發(fā)癥發(fā)生率比較,差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。兩組術(shù)前肌酸激酶(CK)、C反應(yīng)蛋白(CRP)比較,差異均無統(tǒng)計(jì)學(xué)意義(P>0.05),術(shù)后1、3 d,兩組CK、CRP較術(shù)前均明顯升高,且PLA組均高于DAA組,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。兩組術(shù)前Ⅰ型前膠原氨基前肽(PINP)、Ⅰ型前膠原羧基端肽(PICP)及抗酒石酸酸性磷酸酶-5b(TRACP-5b)比較,差異均無統(tǒng)計(jì)學(xué)意義(P>0.05),術(shù)后3個(gè)月兩組PINP、PICP、TRACP-5b均優(yōu)于術(shù)前,且DAA組均優(yōu)于PLA組,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論:直接前入路全髖關(guān)節(jié)置換術(shù)治療GardenⅢ、Ⅳ型股骨頸骨折有利于髖關(guān)節(jié)功能早期恢復(fù),減少患者疼痛及肌肉組織損傷,改善患者骨代謝。

      [Abstract] Objective: To investigate the effect of different approaches of total hip arthroplasty in the treatment of Garden type Ⅲ and Ⅳ femoral neck fractures and its influence on muscle tissue injury and bone metabolism. Method: A total of 93 patients with femoral neck fracture who underwent total hip arthroplasty (THA) in Jining NO.2 People’s Hospital from January 2018 to June 2020 were selected and divided into DAA group (n=45) and PLA group (n=48) according to different operation methods. DAA group was given direct anterior approach THA and PLA group was given posterolateral approach THA. The operation related indexes, pain and hip function scores, complications, muscle tissue injury and bone metabolism indexes were compared between the two groups. Result: Compared with PLA group, the operation time was longer, the amount of intraoperative bleeding was less and the length of incision was shorter in DAA group, the differences were statistically significant (P<0.05), but there were no significant differences in acetabular anteversion and abduction angle between the two groups (P>0.05). There were no significant differences in preoperative VAS score and Harris score between the two groups (P>0.05); the postoperative visual analogue scale (VAS) scores and Harris scores of the two groups were better than those before operation, and the differences were statistically significant (P<0.05); the VAS scores of DAA group were lower than those of PLA group 1 and 3 days after operation (P<0.05); the Harris scores of DAA group were higher than those of PLA group at 2 weeks and 3 months after operation (P<0.05), there was no significant difference in Harris score between the two groups at 6 months after operation (P>0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). There were no significant differences in creatine kinase (CK) and C-reactive protein (CRP) between the two groups before operation (P>0.05), but CK and CRP were significantly increased in the two groups 1 and 3 days after operation, and those of PLA group were higher than those of DAA group (P<0.05). There were no significant differences in N-terminal peptide of type Ⅰ procollagen (PINP), carboxyl terminal propeptide of type Ⅰ procollagen (PICP) and tartrate resistant acid phosphatase-5b (TRACP-5b)between the two groups before operation (P>0.05), 3 months after operation, PINP, PICP and TRACP-5b in the two groups were better than those before operation, and those of DAA group were better than those of PLA group, the differences were statistically significant (P<0.05). Conclusion: Direct anterior total hip arthroplasty in the treatment of Garden type Ⅲ and Ⅳ femoral neck fractures is conducive to early recovery of hip function, reduce pain and muscle tissue damage, and improve bone metabolism.

      隨著年齡的增長(zhǎng),股骨頸骨折的發(fā)生率呈指數(shù)增長(zhǎng)[1],尤其在老年人群中,由于骨質(zhì)疏松性骨密度變化、髖周肌群退化、反應(yīng)能力退化等原因,在受到撞擊等外力作用時(shí)易發(fā)生骨折[2]。全髖關(guān)節(jié)置換術(shù)(THA)是治療股骨頸骨折的重要方法,可顯著地緩解疼痛和改善功能[3]。關(guān)于THA的最佳入路至今仍存在爭(zhēng)議,理想的術(shù)式應(yīng)是術(shù)中損傷小、術(shù)后功能恢復(fù)好且并發(fā)癥少,后外側(cè)入路(PLA)是最常用的入路,但其主要缺點(diǎn)為易發(fā)生后脫位[4];直接前入路(DAA)為肌間隙及神經(jīng)間隙入路,術(shù)中醫(yī)源性肌肉損傷更少,術(shù)后恢復(fù)更快更好[5-6],但有研究指出其術(shù)后并發(fā)癥發(fā)生率更高[7-8]。因此,本研究就DAA與PLA對(duì)GardenⅢ、Ⅳ型股骨頸骨折患者手術(shù)相關(guān)指標(biāo)、疼痛及髖關(guān)節(jié)功能評(píng)分、術(shù)后并發(fā)癥、肌肉組織損傷、骨代謝的影響展開研究,報(bào)道如下。

      1 資料與方法

      1.1 一般資料 選擇濟(jì)寧市第二人民醫(yī)院2018年1月-2020年6月收治的股骨頸骨折行全髖關(guān)節(jié)置換術(shù)的患者93例。納入標(biāo)準(zhǔn):(1)確診為股骨頸骨折且為GardenⅢ、Ⅳ型;(2)初次行全髖關(guān)節(jié)置換術(shù);(3)臨床資料完整;(4)配合本研究。排除標(biāo)準(zhǔn):(1)精神疾病史或溝通障礙;(2)體重指數(shù)(BMI)>30 kg/m;(3)合并其他影響本研究的疾病,如神經(jīng)肌肉病變、既往髖部手術(shù)史、感染等。按照手術(shù)方式不同將患者分為DAA組45例與PLA組48例。本研究經(jīng)醫(yī)學(xué)倫理委員會(huì)批準(zhǔn)同意?;颊邔?duì)本研究知情同意。

      1.2 方法 兩組均采用全身麻醉。DAA組給予直接前入路THA,患者取仰臥位,髖關(guān)節(jié)后伸,于髂前上棘外側(cè)及遠(yuǎn)端2~3 cm處向腓骨頭方向作一8~10 cm切口,暴露并“T”形切開髖關(guān)節(jié)囊,利用拉鉤充分顯露關(guān)節(jié)囊內(nèi)股骨頸并截?cái)啵〕龉晒穷^,置入Hoffman拉鉤暴露并去除髖臼周圍增生贅骨,后植入髖臼假體,患肢內(nèi)收外旋位置于健肢下方,完全暴露股骨近端,植入股骨假體并安裝合適的股骨頭,復(fù)位髖關(guān)節(jié),檢查下肢長(zhǎng)度、活動(dòng)度及人工髖關(guān)節(jié)穩(wěn)定性后縫合關(guān)閉傷口。PLA組采用后外側(cè)入路THA,患者取側(cè)臥位,以大轉(zhuǎn)子頂點(diǎn)為中心在其后外側(cè)做一長(zhǎng)10~12 cm的弧形切口,依次切開皮下組織、淺筋膜、深筋膜、臀大肌及外旋肌群,取出股骨頭,去除髖臼周圍增生贅骨,后植入髖臼假體,髖關(guān)節(jié)屈曲內(nèi)收內(nèi)旋,之后操作同DAA組。兩組術(shù)后均給予常規(guī)治療并進(jìn)行相應(yīng)康復(fù)鍛煉。

      1.3 觀察指標(biāo)及評(píng)價(jià)標(biāo)準(zhǔn) (1)手術(shù)指標(biāo):手術(shù)時(shí)間、術(shù)中出血量、切口長(zhǎng)度,X線片測(cè)量髖臼前傾角及外展角;(2)疼痛及髖關(guān)節(jié)功能:于術(shù)前及術(shù)后1、3 d進(jìn)行VAS評(píng)分,滿分10分,得分越高疼痛越嚴(yán)重;于術(shù)前、術(shù)后2周及3、6個(gè)月時(shí)評(píng)價(jià)髖關(guān)節(jié)功能,采用Harris評(píng)分,滿分100分,得分越高功能越好;(3)并發(fā)癥發(fā)生情況;(4)肌肉組織損傷情況:術(shù)前及術(shù)后1、3 d抽取患者晨起空腹靜脈血送檢驗(yàn)科檢測(cè)血清肌肉損傷標(biāo)志物肌酸激酶(CK)及炎癥反應(yīng)標(biāo)志物C反應(yīng)蛋白(CRP);(5)骨代謝:術(shù)前及術(shù)后3個(gè)月抽取患者晨起空腹靜脈血送檢驗(yàn)科檢測(cè)血清Ⅰ型前膠原氨基前肽(PⅠNP)、Ⅰ型前膠原羧基端肽(PICP)及抗酒石酸酸性磷酸酶-5b(TRACP-5b)水平。

      1.4 統(tǒng)計(jì)學(xué)處理 所得數(shù)據(jù)采用SPSS 23.0進(jìn)行統(tǒng)計(jì)學(xué)分析,符合正態(tài)分布的計(jì)量資料以(x±s)表示,組內(nèi)比較用配對(duì)t檢驗(yàn),組間比較采用獨(dú)立樣本t檢驗(yàn);計(jì)數(shù)資料以率(%)表示,比較用χ檢驗(yàn)或Fisher確切概率法,P<0.05為差異有統(tǒng)計(jì)學(xué)意義。

      2 結(jié)果

      2.1 兩組一般資料比較 兩組年齡、性別、Garden分型等一般資料比較,差異均無統(tǒng)計(jì)學(xué)意義(P>0.05),具有可比性,見表1。

      2.2 兩組手術(shù)指標(biāo)比較 與PLA組比較,DAA組手術(shù)時(shí)間更長(zhǎng)、術(shù)中出血量更少、切口長(zhǎng)度更短,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05);兩組髖臼前傾角及髖臼外展角比較,差異均無統(tǒng)計(jì)學(xué)意義(P>0.05)。見表2。

      2.3 兩組VAS評(píng)分及髖關(guān)節(jié)功能評(píng)分比較 兩組術(shù)前VAS評(píng)分及Harris評(píng)分比較,差異均無統(tǒng)計(jì)學(xué)意義(P>0.05);術(shù)后兩組VAS評(píng)分及Harris評(píng)分均優(yōu)于術(shù)前,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05);術(shù)后1、3 d,DAA組VAS評(píng)分均低于PLA組(P<0.05);術(shù)后2周及3個(gè)月,DAA組Harris評(píng)分均高于PLA組(P<0.05),術(shù)后6個(gè)月,兩組Harris評(píng)分比較,差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。見表3。

      2.4 兩組并發(fā)癥發(fā)生情況比較 DAA組中,2例出現(xiàn)股外側(cè)皮神經(jīng)支配區(qū)域的感覺障礙,PLA組中1例由于活動(dòng)不當(dāng)造成髖關(guān)節(jié)后脫位,神經(jīng)麻痹2例,兩組出現(xiàn)并發(fā)癥的患者均經(jīng)對(duì)癥治療后恢復(fù),兩組均無感染、下肢深靜脈血栓等其他并發(fā)癥發(fā)生。DAA組并發(fā)癥發(fā)生率為4.44%(2/45),PLA組為6.25%(3/48),兩組比較差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。

      2.5 兩組肌肉組織損傷情況比較 兩組術(shù)前CK、CRP比較,差異均無統(tǒng)計(jì)學(xué)意義(P>0.05);術(shù)后1、3 d,兩組CK、CRP較術(shù)前均明顯升高,且PLA組均高于DAA組,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。見表4。

      2.6 兩組骨代謝情況比較 兩組術(shù)前PINP、PICP、TRACP-5b比較,差異均無統(tǒng)計(jì)學(xué)意義(P>0.05);術(shù)后3個(gè)月兩組PINP、PICP、TRACP-5b均優(yōu)于術(shù)前,且DAA組均優(yōu)于PLA組,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。見表5。

      3 討論

      對(duì)于存在移位的GardenⅢ、Ⅳ型股骨頸骨折,尤其是老年患者,首選人工關(guān)節(jié)置換術(shù)[9]。隨著髖關(guān)節(jié)置換術(shù)的普及,手術(shù)入路的選擇也成為關(guān)注的焦點(diǎn)。直接前入路最初由Carl Heuter于1881年提出,由Smith Peterson大量使用推廣該技術(shù)[10]。在國內(nèi)DAA發(fā)展較晚,且手術(shù)難度大、學(xué)習(xí)曲線長(zhǎng),因此發(fā)展受到一定限制[11],但近年來逐漸受到國內(nèi)學(xué)者關(guān)注,并逐漸推廣應(yīng)用。

      本研究中,PLA組手術(shù)時(shí)間短于DAA組,可能由于后外側(cè)入路切斷了外旋肌群,手術(shù)視野顯露良好,易于手術(shù)操作;而直接前入路為神經(jīng)肌肉間隙入路,切口小,對(duì)組織損傷小,因此術(shù)中出血量少,術(shù)后疼痛更輕,與以往文獻(xiàn)[12-13]研究結(jié)果一致。一項(xiàng)Meta分析顯示,隨訪90 d,DAA組髖關(guān)節(jié)功能恢復(fù)更好[6],另有研究顯示,術(shù)后4周DAA組Harris評(píng)分更高,而長(zhǎng)期隨訪兩組Harris評(píng)分無差異[8,14],本研究亦得出相似結(jié)果,術(shù)后2周及3個(gè)月DAA組Harris評(píng)分均高于PLA組(P<0.05),術(shù)后6個(gè)月兩組Harris評(píng)分差異無統(tǒng)計(jì)學(xué)意義(P>0.05),說明直接前入路有利于髖關(guān)節(jié)功能術(shù)后早期恢復(fù)。

      髖關(guān)節(jié)脫位是THA的主要并發(fā)癥,初次全髖關(guān)節(jié)置換術(shù)后發(fā)生率為0.3%~10%[3];有研究認(rèn)為,股外側(cè)皮神經(jīng)損傷更常見,可能與手術(shù)中牽拉時(shí)間較長(zhǎng)或暴力牽拉有關(guān)[15-16]。本研究中DAA組2例出現(xiàn)股外側(cè)皮神經(jīng)支配區(qū)域的感覺障礙,PLA組中1例髖關(guān)節(jié)后脫位,神經(jīng)麻痹2例,兩組并發(fā)癥發(fā)生率差異無統(tǒng)計(jì)學(xué)意義,說明直接前入路手術(shù)安全性較高。CRP、CK均是組織損傷的重要標(biāo)志物,CK是反映肌肉損傷程度的重要指標(biāo),周甲彬等[17]研究顯示,DAA組術(shù)后CRP、CK均低于PLA組,與本研究結(jié)果一致,而Rykov等[18]則發(fā)現(xiàn)兩種術(shù)式對(duì)CRP、CK的影響無差異,分析原因可能與手術(shù)熟練度有關(guān)。PINP、PICP為骨形成的重要指標(biāo),而TRACP-5b為骨吸收重要指標(biāo),兩組術(shù)后上述指標(biāo)均較術(shù)前改善,且DAA組變化更顯著,說明DAA組骨代謝更好,分析原因可能為DAA組患者疼痛輕,功能恢復(fù)快,有利于患者更早地完成康復(fù)鍛煉,從而改善早期骨代謝。

      綜上所述,直接前入路全髖關(guān)節(jié)置換術(shù)治療GardenⅢ、Ⅳ型股骨頸骨折有利于髖關(guān)節(jié)功能早期恢復(fù),減少患者疼痛及肌肉組織損傷,改善患者骨代謝。
